Advancing equity in applied health and care research training: a reflexive perspective on the NIHR INSIGHT programme.

Health inequities in the United Kingdom disproportionately affect racialised, globally marginalized, and working-class communities. Research training programmes shape who becomes a researcher, which questions are prioritized and which knowledge is recognized as legitimate. This Perspective provides a reflexive, practice-informed account of the NIHR Inspiring Students into Research Training (INSIGHT) Programme across three South East England institutions: the University of Kent (MSc Applied Health and Care Research), the University of Brighton (MRes), and the University of Chichester (MRes). Brazil's Neurosurgical Paradox: Advanced Capacity Amid Territorial Inequity.

Neurosurgical care has undergone remarkable technological advancement over recent decades, yet equitable access remains uneven across health systems. This study examines the structural determinants of neurosurgical access in Brazil, a middle-income country with a universal public health system (Sistema Único de Saúde), highlighting the paradox between workforce expansion and persistent territorial inequities. Between 2015 and 2025, Brazil's neurosurgical workforce increased by 50.9%, rising from 2875 to 4340 specialists, with national density growing from 1.43 to 2.04 per 100 000 inhabitants.

Awards, keynotes and gender equity in coastal geoscience and engineering: A 50-year perspective.

Awards and invited keynotes are critical markers of scholarly achievement that shape visibility, career advancement and retention in academia. Yet extensive evidence across science, technology, engineering, mathematics and medicine shows that women remain underrepresented among recipients of these honors. To evaluate equity within the coastal geosciences and engineering (CGE) community, we compiled the gender distribution of 1,138 awards and invited keynotes granted by professional societies and conferences relevant to CGE over the past 50 years. We additionally reviewed publicly available nom

Workforce Implications of Increased Referrals to Hereditary Cancer Services in Canada: A Scenario-Based Analysis.

Over the last decade, utilization of clinical genetics services has grown rapidly, putting increasing pressure on the workforce available to deliver genetic healthcare. To highlight the policy challenges facing Canadian health systems, a needs-based workforce requirements model was developed to determine the number of Canadian patients in 2030 for whom an assessment of hereditary cancer risk would be indicated according to current standards and the numbers of genetic counsellors, clinical geneticists and other physicians with expertise in genetics needed to provide care under a diverse set of

Academic-practice partnerships in public health workforce development in China: a comparative analysis of 12 purposively selected cases.

BACKGROUND: Academic-practice partnerships (APPs) are increasingly used in public health workforce development, but evidence on their institutional drivers and varied development pathways in China remains limited. This study examines how institutional pressures, interorganizational resource relationships, and boundary-spanning practices are reflected in the development of APPs in China. METHODS: An exploratory multiple-case comparative design was adopted. Twelve APP cases were purposively selected across Chinese regions with diverse resource endowments. Drawing on institutional isomorphism, R

Mapping System Vulnerabilities in Anaesthesia Safety: A Role-Based Inquiry in Response to Fatal Fungal Meningitis Outbreaks in Mexico.

OBJECTIVES: Unsafe anaesthesia practices in Mexico have contributed to repeated outbreaks of fatal fungal meningitis, exposing broader health system vulnerabilities. This study aimed to analyse how structural weaknesses in perioperative care, spanning governance, workforce, and institutional practices, reflect deep gaps in health planning, regulation, clinical oversight and hospital management. METHODS: We applied a participatory, role-based systems mapping approach. A two-phase design method combined a survey to identify perceived system failures with structured, guided interprofessional, pa

Medical School Admissions: Focusing on Producing a Physician Workforce That Addresses the Needs of the United States.

The aging population, burnout, and earlier retirement of physicians along with the static number of training positions are likely to worsen the current physician shortage. There is an urgent need to transform the process for selecting medical students. In this Invited Commentary, the authors suggest that to build the physician workforce that the United States needs for the future, academic medicine should focus on building capacity in 3 overarching areas. First, medical schools need to develop a more diverse pool of capable applicants that better matches the demographic characteristics of heal

Efforts in Undergraduate Medical Education to Improve Socioeconomic Status Diversity.

PURPOSE: Despite the importance of socioeconomic status (SES) diversity within the health care workforce, little progress has been made toward this elusive goal. Although centuries of structural disadvantage underlie this stagnation, medical schools are well positioned to increase enrollment and retention of students from low-SES backgrounds. In this research report, the authors investigate existing efforts in undergraduate medical education to improve SES diversity. METHOD: The authors analyzed the Association of American Medical Colleges Medical School Admissions Requirements database, the

Nursing the Nuu-chah-nulth Way: Communities Driving Nursing Policy Priorities.

Rural and remote Indigenous communities face unique challenges, and they must drive solutions for sustaining and maintaining distinct nursing practices. Resourcing Indigenous community needs and aspirations for health depends on sustainable funding and an appropriately resourced nursing workforce. An Indigenous community-engaged research team led a program of study exploring Indigenous systems of care with three distinct communities. We used Indigenous research methodologies to identify obstacles to care and ways to advance nursing and healthcare delivery according to unique values and demogra

Seeing ourselves in anatomy: Embedding equity, diversity, and inclusivity to transform learning in a UK BSc curriculum.

Embedding intersectional equity, diversity, and inclusion (EDI) in anatomy education is essential to reflect the growing diversity of student populations, support all learners, and prepare graduates for a more inclusive, socially responsive global workforce. Anatomical textbooks and models have typically focused on white, cis-gender, able-bodied individuals who perpetuates biases and underrepresentation, impacting learners' professional development. We embedded five workshops in the junior honors BSc anatomy curriculum, which all adopted an intersectional EDI approach. These were strongly anch

Virtual learning modalities and clinical competence in baccalaureate nursing education: An integrative review.

BACKGROUND: The COVID-19 pandemic accelerated the use of digital education in nursing, including simulation, virtual reality (VR), telehealth, and AI-supported tools. Despite increased access and engagement, concerns remain about preparation for clinical practice. AIM: To examine associations between virtual learning modalities and clinical competence and workforce readiness among undergraduate nursing students. METHODS: The authors conducted an integrative review guided by PRISMA and SWiM and used Whittemore and Knafl's five-step methodology to synthesize evidence from diverse study designs

Digital Health Innovation by Design: A Logic Model Scaffold for Rural, Regional, and Remote Settings.

Digital health innovations (DHIs) have the potential to transform access, continuity, and quality of healthcare in rural, regional, and remote (RRR) settings, yet they often fall short in practice. Barriers extend beyond infrastructure and technology to include workforce challenges and the complex realities of delivering care across diverse geographic, cultural, and social contexts. Effective DHIs must therefore be designed with local needs and systemic constraints in mind. Conventional logic models can align inputs and activities with intended outcomes, but their linear and static assumptions
