Observation with long follow-up of the first 200 cases of intraocular injection of silicone combined with vitrectomy led us to discover three major complications: cataract, keratopathy and hypertonia, the last two found only in aphakic eyes. No toxic action of silicone could be proved to explain these complications. They seem to be related to physical properties of the silicone which we use, and in particular its gravity lower than one. Because there are psychological and emotional problems, as well as optical ones, when patients are adapting to aphakic corrections, it is necessary for the optometrist to establish rapport and provide suitable explanations. Suggestions are made concerning discussions about the optical changes associated with use of spectacles, implants, and contact lenses. The author advises the use of a cataract lens package in which a series of lenses is designed prior to cataract surgery.

The authors refit 20 former hard lens aphakic eyes with the Bausch & Lomb SOFLENS. Ten patients required less soft lens power than the power of the hard contact lens formerly used; five patients required the same soft lenspower; and five patients required more power with the soft lens than with the previous hard lens used. These findings were different than what would have been expected on the basis of the Back-Vertex Power Chart distributed by Bausch & Lomb.

The paper presents the results of Ahmed's drainage implanted in 19 patients with different forms of posttraumatic glaucoma: 14 and 5 eyes with contusion and wound glaucoma, respectively. Bilateral congenital cataract is the most common cause of treatable childhood blindness. Nuclear cataract is usually present at birth and is nonprogressive, whereas lamellar cataract usually develops later and is progressive.
