[Principles for the control of sexually transmitted infections in tropical areas].

Owing to their frequency and their own complications, sexually transmitted infections (STI) constitute a very classical health problem, especially in developing areas. The interest for those diseases has been on the rise since the beginning of the AIDS pandemic. Conventional sexually transmitted diseases (such as syphilis, chancroid, infections due to Neisseria gonorrhoeae and to Chlamydia trachomatis) are potent cofactors of HIV transmission. It has been established that the treatment of patients with STI was able to lower the HIV incidence at the community level. Today the fight against STI

Doppler assessment of the fetal venous system.

This article describes the achievements in Doppler measurements of the fetal venous circulation with emphasis on the clinical impact of these techniques. In rhesus isoimmunization, fetal venous flow assessment gives useful information on the fetal haematologic condition and on the impact of blood transfusion. In first trimester fetuses, Doppler evaluation of the ductus venosus and umbilical vein could attribute to the detection of cardiac defects and/or chromosomal abnormalities. The inferior vena cava flow velocity waveform could be studied in cases of fetal arrhythmias to be able to diagnose

Overproduction of CXC chemokines CXCL1, CXCL9, CXCL10 and CXCL12 in β-thalassemia major or patients.

BACKGROUNDS AND OBJECTIVES: B-thalassemia major is one of the most frequent hematological genetic disorders, worldwide. Chemokines are the main components of the immune system and play fundamental roles in pathogenesis of inflammatory disorders. Therefore, the present study aimed to examine whether serum CXC chemokines are altered in b-thalassemia major patients. DESIGN AND SETTINGS: We enrolled 63 b-thalassemia patients and 80 controls in this cross-sectional study, which was performed during 2012-2013 in Kerman, Iran. METHODS: We enrolled 63 b-thalassemia patients and 80 controls in the pr

Specific activation of dendritic cells enhances clearance of Bacillus anthracis following infection.

Dendritic cells are potent activators of the immune system and have a key role in linking innate and adaptive immune responses. In the current study we have used ex vivo pulsed bone marrow dendritic cells (BMDC) in a novel adoptive transfer strategy to protect against challenge with Bacillus anthracis, in a murine model. Pre-pulsing murine BMDC with either recombinant Protective Antigen (PA) or CpG significantly upregulated expression of the activation markers CD40, CD80, CD86 and MHC-II. Passive transfusion of mice with pulsed BMDC, concurrently with active immunisation with rPA in alum, sign

Paroxysmal nocturnal haemoglobinuria with coexisting deficiency of the ninth component of complement: lack of massive haemolytic attack.

A 47-year-old woman with paroxysmal nocturnal haemoglobinuria (PNH) was found to have an inherited deficiency in the ninth complement component (C9). In complement-sensitivity lysis tests, 80% of her erythrocytes were markedly complement-sensitive (PNH-III). Laser cytofluorimetry with a monoclonal antibody against decay-accelerating factor (DAF) revealed that 95% of her erythrocytes were DAF-negative. Surprisingly, she has suffered only mild haemolysis and has never experienced massive spontaneous haemolysis. Gross haemoglobinuria and jaundice occurred only after receiving postoperative transf

Extreme juvenile gigantomastia with bilateral excision of 16.5 kg (27% of body weight) in a 15-year-old adolescent: a case report.

Juvenile gigantomastia is a rare clinical entity associated with major functional and psychological consequences. We report the case of a 15-year-old adolescent with a body mass index of 24.2 kg/m2 who presented with extreme bilateral gigantomastia progressing over 2 years. Reduction mammoplasty using the Thorek technique was performed, with a total excision of 16.5 kg (27% of body weight). The procedure was complicated by postoperative hypovolemic shock requiring blood transfusion and intensive care monitoring. Secondary partial necrosis involving both the nipple and areolar components of the

ABO incompatibility: A cause for neonatal alloimmune thrombocytopenia.

ABO antibodies are naturally occurring antibodies. The ABO antibodies found in the Group O individuals include anti-A and anti-B. In Group O individuals, it tends to be predominantly immunoglobulins G (IgG), although immunoglobulins M and IgA components are also present. Infants of Group O mothers are at higher risk for hemolytic disease of the fetus and new-born than those born to mothers with Group A or B because IgG readily cross the placenta. At the same time, abnormal high concentration of ABO antibody in mother can lead to destruction of platelets in neonates and leads to development of

Development and Validation of a Simplified Five-Variable Score for Predicting Massive Transfusion in Blunt-Trauma-Predominant Patients.

Background/Objectives: Early identification of trauma patients requiring massive transfusion (MT) is essential for timely activation of massive transfusion protocols. Existing models have limitations: the Assessment of Blood Consumption (ABC) score includes penetrating injury, which limits its use where blunt trauma predominates; the Trauma-Associated Severe Hemorrhage (TASH) score requires numerous weighted variables; and the Traumatic Bleeding Severity Score (TBSS) requires blood pressure measured after crystalloid infusion. This study aimed to develop and validate a simplified MT prediction

Relative distribution of West Nile virus RNA in blood compartments: implications for blood donor nucleic acid amplification technology screening.

BACKGROUND: Despite implementation of targeted individual-donor nucleic acid test (NAT) screening of blood donors for West Nile virus (WNV), three "breakthrough" WNV transfusion transmission cases were reported (2004-2008), suggesting that current plasma-based assays are unable to detect all WNV-infectious donations. A 2007 report found that 19 of 20 red blood cell components from WNV-infected donors contained 1 log higher viral load than plasma components. This study's aim was to further establish the value of screening whole blood relative to plasma for WNV RNA by generating differential vir

Brief note and evaluation of acute-radiation syndrome and treatment of a Tokai-mura criticality accident patient.

Patient A who was exposed to a critical dose of radiation developed skin lesions throughout the body surface, gastrointestinal disorder with massive diarrhea and prominent bleeding, which caused severe loss in body fluids. Gastrointestinal bleeding due to the deteriorated intestinal mucosa was considered to be one of the major causes of death, although infection did not develop, possibly because of SDD and aseptic intensive care, until terminal stages. Patient A ultimately developed respiratory and renal failure in addition to skin exudate and gastrointestinal bleeding, and died of multiple or

Haemovigilance and Epidemiological Surveillance of Transfusion-Transmitted Infections in Greece, 1996-2023: National Donor Screening Data and Recipient Investigations.

BACKGROUND: Surveillance of transfusion-transmitted infections (TTIs) supports blood safety, particularly for multi-transfused patients such as those with haemoglobinopathies. MATERIALS AND METHODS: Retrospective descriptive analysis of aggregated national surveillance data from Greece. We report aggregate infectious-marker positivity values from national surveillance summaries for 1996-2009. We also report NAT yields in 2007-2023, the transmission categories of HIV-positive donors in 2010-2019 and recipient investigations reported to the national haemovigilance system in 2005-2023. RESULTS:

Hepatitis E: An update on One Health and clinical medicine.

The hepatitis E virus (HEV) is one of the main causes of acute hepatitis and the de facto global burden is underestimated. HEV-related clinical complications are often undetected and are not considered in the differential diagnosis. Convincing findings from studies suggest that HEV is clinically relevant not only in developing countries but also in industrialized countries. Eight HEV genotypes (HEV-1 to HEV-8) with different human and animal hosts and other HEV-related viruses are in circulation. Transmission routes vary by genotype and location, with large waterborne outbreaks in developing c

Increased transfusion-free survival following auxiliary pig liver xenotransplantation.

BACKGROUND: Pig to baboon liver xenotransplantation typically results in severe thrombocytopenia and coagulation disturbances, culminating in death from hemorrhage within 9 days, in spite of continuous transfusions. We studied the contribution of anticoagulant production and clotting pathway deficiencies to fatal bleeding in baboon recipients of porcine livers. METHODS: By transplanting liver xenografts from α1,3-galactosyltransferase gene-knockout (GalT-KO) miniature swine donors into baboons as auxiliary organs, leaving the native liver in place, we provided the full spectrum of primate clo

Listeria monocytogenes in donated platelets: a potential transfusion-transmitted pathogen intercepted through screening.

BACKGROUND: Bacterial contamination of blood components is a potentially life-threatening complication of transfusions. In October 2005, the Centers for Disease Control and Prevention (CDC) noted four Listeria monocytogenes (Lm) isolates cultured from four different units of donated apheresis platelets (PLTs) among Lm isolates sent to the CDC National Listeria Reference Laboratory for subtyping as part of routine surveillance activities. STUDY DESIGN AND METHODS: We describe an investigation to determine possible common sources of infection among donors or factors associated with PLT collecti

Impact of transcription factors KLF1 and GATA1 on red blood cell antigen expression: a review.

KLF transcription factor 1 (KLF1) and GATA binding protein 1 (GATA1) are transcription factors (TFs) that initiate and regulate transcription of the genes involved in erythropoiesis. These TFs possess DNA-binding domains that recognize specific nucleotide sequences in genes, to which they bind and regulate transcription. Variants in the genes that encode either KLF1 or GATA1 can result in a range of hematologic phenotypes-from benign to severe forms of thrombocytopenia and anemia; they can also weaken the expression of blood group antigens. The Lutheran (LU) blood group system is susceptible t

[Typical causes and clinical presentations of traumatic and nontraumatic soft tissue and organ haemorrhage in emergency departments].

Acute bleeding often occurs in a wide variety of forms in clinical emergency medicine. In traumatic bleeding, soft tissue injuries to the head or extremities are common, while severe bleeding is less common in chest injuries or pelvic trauma. In nontraumatic emergencies, gastrointestinal bleeding is the leading cause, but many other bleeding entities are possible. Structured management helps to detect the critically ill patient, identify the possible source of bleeding and provide appropriate radiological diagnostics and therapy.
