The small intestine is an uncommon site of gastro-intestinal (GI) bleeding; however it is the commonest cause of obscure GI bleeding. It may require multiple blood transfusions, diagnostic procedures and repeated hospitalizations. Angiodysplasia is the commonest cause of obscure GI bleeding, particularly in the elderly. Inflammatory lesions and tumours are the usual causes of small intestinal bleeding in younger patients. An 80-year-old man with von Willebrand's disease was admitted with severe melaena. Despite suppletion with von Willebrand concentrate he continued to be dependent on blood transfusions. We describe our initial experience of a method which develops the role of endoscopic surgery in the treatment of early prostatic carcinoma. An "extended" transurethral resection of the prostate (TURP) under ultrasound control reduces the prostatic tissue to a thin residual capsule suitable for subsequent transmural coagulation by endoscopic YAG laser. Twenty patients with early disease were entered into the pilot study. BACKGROUND: Functional dyspepsia (FD) is a common disorder of gut-brain interaction characterized by postprandial distress and epigastric pain, which significantly impairs quality of life and increases healthcare burden. Although the Rome IV criteria and Japanese guidelines have refined its definition, the pathophysiology remains multifactorial, involving gastric motility abnormalities, visceral hypersensitivity, and autonomic dysfunction. Traditional diagnostic approaches, mainly based on clinical history and exclusion of organic diseases, are limited in elucidating the underlying mechanisms.
