[Transposition of Great Artery].

Transposition of the great artery is one of common congenital cardiac disease resulting cyanosis. Death occurs easily in untreated patients with transposition and intact ventricular septal defect (VSD) in infancy at a few days of age when posterior descending coronary artery (PDA) closed. Since there are 2 parallel circulations, flow from pulmonary to systemic circulation is necessary for systemic oxygenation, and Balloon atrial septostomy or prostaglandin infusion should be performed especially if patient do not have VSD. Although the advent of fetal echocardiography, it is difficult to diagn

[Coronarographic and ventriculographic aspects of 76 cases of unstable angina (authors transl)].

-- Coronarographic and ventriculographic aspects of 76 patients with unstable angina were analyzed. 8 cases (10%) had normal coronary arteries, 9 (11.8%) had slight stenotic lesions (less than 50%), 59 (77.6%) had serious stenosis. The stenosis concerned only one principal branch in 22 patients (37.2%), 2 branches in 24 (40%), 3 branches in 13 (22%). In 77-79% of cases, ventricular contractility was normal. A collateral circulation was found in 58% of cases, but no link was discovered between the coronaric lesions and the clinical type of angina. The course of the illness was studied during th

Pharmacological treatment of chronic stable angina pectoris.

Chronic stable angina is the most common manifestation of ischaemic heart disease in the developed world and is associated with impaired quality of life and increased mortality. The pathogenesis of stable angina is complex and often, albeit not always, involves flow-limiting epicardial coronary artery stenoses (atheromatous plaques) that reduce the ability of the coronary circulation to deliver appropriate blood supply to the myocardium. The coronary microcirculation can also play an important role. An imbalance between myocardial oxygen supply and metabolic oxygen demand causes the symptoms o…

Three-dimensional transmural mechanical interaction between the coronary vasculature and passive myocardium in the dog.

The "garden hose" effect of coronary perfusion on diastolic left ventricular (LV) mechanics has been proposed to cause changes in systolic function by altering diastolic sarcomere length. We measured transmural distributions of three-dimensional shape change using radiopaque markers implanted in the LV free wall of eight isolated arrested canine hearts as functions of coronary arterial perfusion pressure (Pp) and LV pressure (PLV) and related these deformations to the local muscle fiber architecture. Increased Pp from 0 to 110 mm Hg produced a 10% reduction in LV chamber volume (P < .01) and 2

Regional myocardial blood flow in experimental myocardial infarction after pretreatment with aspirin.

The effects of aspirin on myocardial blood flow in an area of ischemia were studied in 12 baboons. In each, a diagonal branch of the left anterior descending coronary artery was ligated. Six of the baboons received aspirin (2 X 600 mg orally, 12 hours and 1 hour before ligation); the other six did not receive aspirin and served as a control group. The extent of myocardial ischemia was delineated with an electrode wire grid on the surface of the anterior left ventricular wall. The maximal area circumscribed by electrodes with 2 mV or more ST segment elevation was compared with the area of reduc

Transcatheter Aortic Valve Replacement in Type B Aortic Dissection.

BACKGROUND: Long non-coding RNAs (lncRNAs) have been reported to modulate cardiovascular diseases, and expression dynamics of lncRNAs in the bloodstream were proposed to be potential biomarkers for clinical diagnosis. However, few cardiovascular diseases-related circulating lncRNAs were identified and their prediction power has not been investigated in depth. Here we report a new circulating lncRNA, atherosclerotic plaque pathogenesis associated transcript (APPAT), and evaluated its role and predicting ability in atherosclerotic development. METHODS: APPAT was analyzed and screened by high-th

Systemic Venous Rerouting Through the Coronary Sinus for ccTGA With Bilateral SVCs.

The double-switch operation for congenitally corrected transposition of the great arteries, in which the morphologic left ventricle is restored to the systemic circulation, is an effective surgical option. This case report describes an atrial switch technique without using supplemental material during the double-switch operation for congenitally corrected transposition of the great arteries associated with persistent left superior vena cava, with the systemic venous blood flow rerouted through the enlarged coronary sinus.

Hybrid approach combining off-pump CABG with transapical aortic valve implantation via median sternotomy.

We report the case of a 75-year-old patient diagnosed with severe aortic stenosis and two-vessel coronary artery disease. Due to multiple comorbidities including chronic renal insufficiency, stroke and pulmonary hypertension (EuroSCORE: 34%; STS mortality risk: 14.9%), he was not a candidate for conventional aortic valve surgery. He underwent a novel hybrid treatment approach combining off-pump CABG and transapical aortic valve implantation via a median sternotomy. Extracorporeal circulation could be entirely avoided.

Gut Dubosiella newyorkensis alleviates noise-related atherosclerosis by reducing N1-methyl-4-pyridone-3-carboxamide.

Environmental noise is ubiquitous in daily life and has emerged as a major risk factor for cardiovascular diseases. Nevertheless, the association between noise exposure and coronary heart disease, as well as the underlying molecular mechanisms, remains poorly understood. Here, analyses of UK Biobank (UKB) data revealed that noise exposure was associated with an increased risk of coronary heart disease. We further demonstrated that noise exposure elevated circulating levels of N1-methyl-4-pyridone-3-carboxamide (4PY) in both humans and rodents. Accumulation of 4PY suppressed endothelial NR4A1 e

Improved recovery from laryngeal-stimulation-induced apnea by alpha-adrenergic blockade in adult rabbits.

The apnea triggered by laryngeal stimulation (LS) is associated with a redistribution of blood flow towards the heart and the brain that depends upon peripheral vasoconstriction. The circulatory component of the response to LS has been shown to be reduced following injection of large dose of beta-adrenergic agonist that more intriguingly blunted the ventilatory response as well. To test whether the reduction of LS-induced apnea could be related to the decreased magnitude of the peripheral vasoconstruction, the effects of the alpha-adrenergic antagonist urapidil on the ventilatory and blood pre

The use of angiotensin-converting enzyme inhibitors in congestive heart failure.

The development of angiotensin-converting enzyme (ACE) inhibitors has been important in furthering our understanding of the pathophysiology of congestive heart failure and improving the care of these patients. ACE inhibitors have been shown to improve the longevity and quality of life of patients with congestive heart failure. They decrease neurohumoral over-activation, they restore baroreceptor reactivity, and in many cases they increase sodium and water excretion. If hypovolemia and an excessive decrease in renal perfusion pressure can be avoided, ACE inhibitors generally do not cause a dete

Pulsed Doppler echocardiography in the preoperative evaluation of total anomalous pulmonary venous connection.

Pulmonary venous flow was evaluated by pulsed Doppler echocardiography in 38 patients with total anomalous pulmonary venous connection. Twenty-nine of these 38 had no associated intracardiac anomaly (Group I), and 9 had complex intracardiac anatomy associated with low pulmonary blood flow (Group II). In Group I the drainage was infracardiac in nine, supracardiac in seven, intracardiac in eight and mixed in five. In both groups, in those with venous obstruction the flow in the individual pulmonary veins and ascending or descending vein was nonphasic, varying only with respiration. Flow in the a

New developments in cardiopulmonary resuscitation.

The traditional concept of cardiopulmonary resuscitation (CPR) is that the heart is squeezed between the sternum and the spine during external cardiac massage to create a pressure gradient that forces blood from the heart to the periphery. Although the heart may actually be squeezed in some persons by this maneuver, a newer view of CPR holds that closed-chest compression produces a generalized rise in intrathoracic pressure that is applied to the pulmonary vascular bed as well as the heart. Thus, the heart does not serve primarily as a pump during external cardiac massage but instead acts as a

One origin, diverse courses: coronary CTA evaluation of single coronary artery variants from the right sinus of Valsalva.

Single coronary artery (SCA) anomalies, in which the entire coronary circulation arises from a solitary ostium, are exceedingly rare congenital variants with variable clinical implications depending on their morphologic course. Recognition of malignant trajectories, particularly interarterial courses, is critical given their association with myocardial ischemia and sudden cardiac death (SCD). This study aimed to describe the anatomical diversity, clinical presentations, and prognostic implications of SCA originating from the right sinus of Valsalva (RSV), with emphasis on the diagnostic value …
