Radiologic Image-Guided Tube Stoma Insertion in Neonatal Short Bowel Syndrome: First Case Report in the Literature.

Background: The management of neonatal short bowel syndrome can be challenging, and it is critical that these babies are managed in a multidisciplinary team setting with specialists who are experienced in the complex management of these children. One of the surgical strategies, initially published by the Bianchi team in Manchester, UK, is controlled tissue expansion program (CTE) which is done via the insertion of catheters into the proximal and distal bowel in the form of tube stomas. The clamping of the proximal tube allows for an increase in length and circumferential diameter of the proxim

Comparison of enterotomy leak pressure among fresh, cooled, and frozen-thawed porcine jejunal segments.

OBJECTIVE To determine whether stored (cooled or frozen-thawed) jejunal segments can be used to obtain dependable leak pressure data after enterotomy closure. SAMPLE 36 jejunal segments from 3 juvenile pigs. PROCEDURES Jejunal segments were harvested from euthanized pigs and assigned to 1 of 3 treatment groups (n = 12 segments/group) as follows: fresh (used within 4 hours after collection), cooled (stored overnight at 5°C before use), and frozen-thawed (frozen at -12°C for 8 days and thawed at room temperature [23°C] for 1 hour before use). Jejunal segments were suspended and 2-cm enterotomy i

State of Play: Eight Decades of Surgery for Esophageal Atresia.

AIM: Surgical expertise and advances in technical equipment and perioperative management have led to enormous progress in survival and morbidity of patients with esophageal atresia (EA) in the last decades. We aimed to analyze the available literature on surgical outcome of EA for the past 80 years. MATERIALS AND METHODS: A PubMed literature search was conducted for the years 1944 to 2017 using the keywords "esophageal/oesophageal atresia," "outcome," "experience," "management," and "follow-up/follow up." Reports on long-gap EA only, non-English articles, case reports, and reviews without ori

Risk factors and prediction model for inpatient surgical site infection after major abdominal surgery.

BACKGROUND: Surgical site infections (SSIs) are a common source of postoperative morbidity and a marker of surgical quality. The ability to predict the incidence of SSIs is limited and most models have poor predictive value. We sought to identify risk factors associated with SSIs and develop a prediction model for SSIs after major abdominal surgery. METHODS: A total of 1744 patients undergoing pancreatic, hepatobiliary, and colorectal resections between January 1, 2010 and August 31, 2013 at Johns Hopkins Hospital were identified. Risk factors for any inpatient SSI (superficial and deep) were

Surgical techniques for esophageal replacement in children.

PURPOSE: Surgical techniques for esophageal replacement (ER) in children include colon interposition, gastric tube, gastric transposition, and jejunal interposition. This review evaluates the merits and demerits of each. METHOD: Surgical techniques, complications, and outcome of ER are reviewed over last seven decades. RESULTS: Colon interposition is the time-tested procedure with minimal and less serious complications. Long-term complications include reflux, halitosis, colonic segment dilatation, and anastomotic stricture, sometimes requiring surgical interventions especially for dilatation

Feasibility and impact on surgical outcomes of modified double-stapling technique for patients undergoing laparoscopic anterior resection.

BACKGROUND: Anastomotic leakage is a major cause of postoperative morbidity and mortality in the treatment of colorectal cancer. The aim of this study was to investigate the modified double-stapling technique (MDST), as an alternative for conventional double-stapling technique (DST), and whether it could reduce the anastomotic leakage rate in laparoscopic anterior resection (Lapa-AR). STUDY DESIGN: Between March 2009 and October 2010, a total of 189 patients who underwent Lapa-AR for the treatment of adenocarcinoma of the sigmoid colon or rectosigmoid colon were divided into the MDST group (n

Eating behavior in rats subjected to vagotomy, sleeve gastrectomy, and duodenal switch.

BACKGROUND/AIM: Food intake, eating behavior, and metabolic parameters in rats that underwent bilateral truncal vagotomy, sleeve gastrectomy, and duodenal switch procedures were examined. METHODS: Rats were subjected to bilateral truncal vagotomy plus pyloroplasty (VTPP), pyloroplasty (PP), laparotomy, sleeve gastrectomy (SG), or duodenal switch (DS; with and without SG). RESULTS: VTPP, but neither PP nor laparotomy, reduced body weight (BW; 10%) transiently (1 week postoperatively). SG reduced BW (10%) for 6 weeks, while DS alone or SG followed by DS led to a continuous BW loss from 15% at

Expressions of connexin and par-3 in the distal margin of rectal cancer after ultra-low anterior resection.

This study examined the expression of connexin and protease-activated receptor 3 (par-3) in the distal resection margin of rectal cancer and the correlation of the expression of the two proteins with tumor relapse. A total of 40 patients with rectal cancer underwent ultra-low anterior resection with curved cutter stapler. The pathological specimens were divided into 3 groups in terms of sampling sites: tumor group, 2.0-cm group (in which the tissues were harvested 2.0 cm distal to the tumor tissues), 3.0-cm group (in which the tissues were taken 3.0 cm away from the tumor tissues). All the sam

[Gastrectomy with end-loop jejunogastroplasty in stomach cancer].

Experience with surgical treatment of 147 patients after gastrectomy with jejunogastroplasty and creation of end-loop esophageal-intestinal anastomosis is analyzed. Original methods of jejunogastroplasty are based on end-loop construction of esophageal-intestinal anastomosis simulating anatomic-functional analog of lost cardia and pylorus and replacing reservoir function of the stomach. One year and later after surgery 83 patients with end-loop esophageal-intestinal anastomosis and 73 patients with anastomosis were examined. It is demonstrated that jejunogastroplasty with end-loop anastomosis

The fistulectome: a new device for treatment of complex anal fistulas by "Core-Out" fistulectomy.

PURPOSE: In an attempt to improve the quality of life of patients with high anal fistula, we developed a new mechanical device, a "fistulectome," which excises the fistula tract in a totally controlled manner, particularly useful in the treatment of high anal fistulas. The "fistulectomy set" consists of a flexible shaft, cannulation and fixation guides, an incisor mouth, and a handle, which is simultaneously used for motor housing. METHODS: The principle of the treatment is to excise approximately 2-mm thickness of the fistula tract circumferentially, which in fact is a "coring-out" procedure

Gastrointestinal pseudo-obstruction: report of a patient with postoperative pseudo-obstruction.

Postoperative pseudo-obstruction is a rare state of protracted gastrointestinal paresis that may progress to paralysis without the presence of obstructive lesions. Pseudo-obstruction is usually, but not exclusively, associated with an abdominal operative procedure (laparotomy), however, it may occasionally occur following extra-abdominal operations. As differentiated from the usual, 'physiologic'postoperative paresis, pseudo-obstruction persists for more than 7 days. The pathogenesis of postoperative pseudo-obstruction is complex and as yet partially unknown. Whereas the 'physiologic' postoper

Early enteral nutrition in gastrointestinal surgery: a pilot study.

There is still some concern about the safety of early enteral nutrition (EN) to patients with recent anastomoses. A pilot trial was carried out on a prospective basis to evaluate the tolerance and clinical outcome of 56 patients who received early EN following gastrointestinal (GI) surgery. A continuous infusion of an elemental, peptide-based diet was administered using a nasointestinal feeding tube placed beyond the pylorus by the operating surgeon. Tube feeds were started at 6.07 +/- 4.99 h after surgery and advanced as tolerated to a rate of 60 mL/h on the third postoperative day. Patients
