Peripapillary structural and microvascular alterations in early dysthyroid optic neuropathy.

BACKGROUND: To explore the changes in blood supply and structure around the optic nerve head (ONH) in thyroid-associated ophthalmopathy (TAO) patients with suspected dysthyroid optic neuropathy (DON). METHODS: TAO patients [19 with DON; 24 non-DON (NDON); 20 with equivocal DON (EDON)], and 34 control subjects were examined. Optical coherence tomography angiography (OCTA) was used to obtain peripapillary retinal nerve fiber layer (p-RNFL) and vessel density parameters, including the ONH whole image vessel density (ONH-wiVD) and the radial peripapillary capillary vessel density (RPC-VD) in earl

Feedback Enhances Training and Elicits Biometric Responses in Novel Flexible Nasolaryngoscopy Model.

OBJECTIVES: To demonstrate the feasibility and impact of auditory feedback in flexible fiberoptic nasolaryngoscopy (FNL) training with a novel model incorporating audio triggers based on known anatomical loci of negative patient feedback. METHODS: This proof-of-concept study used a CT-based 3D-printed and silicone-casted model of nares to mainstem bronchi to teach FNL during an otolaryngology simulation training. The spatially tracked laryngoscope tip triggered simulated responses (pain, gag, cough) when co-localized to specific anatomic landmarks: the nasal septum, epiglottis, and trachea. P

Reliability and Accuracy of Remote Fiberoptic Nasopharyngolaryngoscopy in the Pediatric Population.

INTRODUCTION: Telemedicine is an increasingly prevalent component of medical practice. In otolaryngology, there is the potential for telemedicine services to be performed in conjunction with device use, such as with a nasolaryngoscope. This study evaluates the reliability of remote examinations of the upper airway through an iPhone recording using a coupling device attached to a nasopharyngolaryngoscope (NPL). METHODS: A prospective, blinded study was performed for pediatric patients requiring an NPL during an office visit. The NPL was performed using a coupling device attached to a smartphon

Awake Fiberoptic Intubation in Cervical Spine Injury: A Comparison between Atomized Local Anesthesia versus Airway Nerve Blocks.

Background In cooperative patients with cervical spine injury, awake fiberoptic intubation is an excellent option for elective and semi urgent situations. It allows documentation of neurologic examination before and after intubation and surgical positioning. We have compared anesthesia of airway by nerve block and the local anesthesia atomizer undergoing awake fiberoptic intubation in cervical spine injury patients, in terms of the intubation time and discomfort. Objective To compare the intubation time and discomfort in patient with cervical spine injury with anticipated difficult airway pote

Transnasal injection laryngoplasty.

OBJECTIVES: The objective of this study was to evaluate the role of transnasal fiber-optic injection laryngoplasty in the treatment of glottic insufficiency. METHODS: Video recordings of 16 patients who underwent fiber-optic injection laryngoplasty for the treatment of glottic insufficiency were reviewed. Outcome measures included perceptual evaluation using the GRBAS (grading, roughness, breathiness, asthenia, straining) classification, laryngeal videostroboscopy, and frame-by-frame analysis to assess the extent, shape, and duration of glottic closure. RESULTS: Nine patients were ultimately

A randomised comparison of the self-pressurised air-QTM intubating laryngeal airway with the LMA Unique™ in children.

We conducted a randomised trial comparing the self-pressurised air-Q™ intubating laryngeal airway (air-Q SP) with the LMA-Unique in 60 children undergoing surgery. Outcomes measured were airway leak pressure, ease and time for insertion, fibreoptic examination, incidence of gastric insufflation and complications. Median (IQR [range]) time to successful device placement was faster with the air-Q SP (12 (10-15 [5-18])) s than with the LMA-Unique (14 (12-17 [6-22]) s; p=0.05). There were no statistically significant differences between the air-Q SP and LMA-Unique in initial airway leak pressures

General anaesthesia in an adult patient with Morquio syndrom with emphasis on airway issues.

Patients with Morquio syndrome possess a number of characteristics which may complicate an anaesthetic procedure. The most important is that a deposition of mucopolysaccharides in the soft tissues of the oro-pharynx distorts the airway, making the airway management difficult, while the atlanto-axial instability puts these patients at risk of subluxation and quadriparesis. As the endotracheal intubation in Morquio syndrome patients may be difficult or even impossible, we recommend the technique of awake fiberoptic intubation to be considered. Our approach to awake fiberoptic intubation in an ad

Fiber optic bronchoscopy in patients with acute hypoxemic respiratory failure requiring noninvasive ventilation--a feasibility study.

INTRODUCTION: Noninvasive ventilation (NIV) is a standard procedure in selected patients with acute respiratory failure. Previous studies have used noninvasive ventilation to ensure adequate gas exchange during fiberoptic bronchoscopy in spontaneously breathing hypoxemic patients, thus avoiding endotracheal intubation. However, it is unknown whether bronchoscopy can be performed safely in patients with acute hypoxemic respiratory failure already in need of NIV prior to the decision for bronchoscopy. METHODS: We prospectively investigated 40 consecutive, critically ill, adult patients with acu

A comparative study of endotracheal tube positioning methods in children: safety from neck movement.

BACKGROUND: The unexpected displacement of the endotracheal tube (ETT) as a result of neck movements can cause endobronchial intubation and accidental extubation. The ETT is subject to movement even after its proper placement has been confirmed either clinically or radiographically. METHODS: One-hundred-seven children (2-8 yr) were divided randomly into three groups. In Group I, the ETT was entered into the main bronchus and withdrawn until equal sounds in both lung were heard, and then withdrawn 2 cm. In Group II, the ETT position was determined by placing the prescribed marks on the ETT at

Responsive Contingency Planning: a novel system for anticipated difficulty in airway management in dental abscess.

BACKGROUND: This study is of a novel system for management of anticipated difficult airway (Responsive Contingency Planning). It is based on the notion that almost all problems in airway management have already been experienced, so they can be anticipated and prepared for using 'worst case' planning. METHODS: Anaesthetic colleagues were introduced to the new system. Thirty-two patients with dental abscess were recruited for anaesthetic airway management based around the new scheme. Data collection involved a preoperative assessment of problems specific to dental abscess, fascia-spaces involve

Glottic insertion of the ProSeal LMA occurs in 6% of cases: a review of 627 patients.

PURPOSE: Glottic insertion of the ProSeal Laryngeal Mask Airway (PLMA) has received little attention in the anesthesiology literature. We investigated the incidence and depth of insertion associated with this important cause for a failed insertion attempt with the PLMA. METHODS: With Institutional Review Board approval, we reviewed 15-months' use of the PLMA. Diagnosis of glottic insertion involved a test with children's bubble solution placed on the drain tube port, as well as a fibreoptic examination of the airway of patients experiencing airway obstruction. Patients were anesthetized and p

In vitro and in vivo evaluation of the thermal patterns and lesions of catheter ablation with a microwave monopole antenna.

BACKGROUND: Radiofrequency (RF) catheter ablation is an effective treatment for supraventricular tachycardia. The effectiveness of the technique is at times limited by the small lesion size produced by RF energy delivery. Previous reports have indicated that microwave energy is capable of producing a larger volume of heated tissue than radiofrequency energy, raising the possibility that microwave energy may offer a potential alternative energy source to radiofrequency for the substrate ablation of certain arrhythmias such as ventricular tachycardia or atrial flutter. METHODS: The present stud

Use of a remifentanil-propofol mixture for pediatric flexible fiberoptic bronchoscopy sedation.

BACKGROUND: Flexible fiberoptic bronchoscopy is an important diagnostic tool for pediatric pulmonologists. Because of its favorable respiratory profile, ketamine has become a popular sedative for this procedure, but may be associated with unpleasant emergence reactions in the older child. Remifentanil is a newer, ultra-short acting opioid that has been shown to provide effective sedation and cough suppression for fiberoptic bronchoscopy when combined with intermittent propofol boluses. However, delivery of these agents as a combined, single infusion has not been described. METHODS: Children >

Combined antegrade and retrograde endoscopic approach for the management of urinary diversion-associated pathology.

BACKGROUND: Endourologic management of stones and strictures in patients with a urinary diversion is often cumbersome because of the absence of standard anatomic landmarks. We report on our technique of minimally invasive management of urinary diversion-associated pathology by means of a combined antegrade and retrograde approach. PATIENTS AND METHODS: Five patients with urinary diversion-associated pathology were treated at our institution between May 1997 and October 1998. Their problems were: an obstructing ureteral stone in a man with ureterosigmoidostomy performed for bladder extrophy; t

Comparison of upper-airway evaluations during wakefulness and sleep.

The location of upper-airway obstruction during sleep has been predicted by using waking measures that include physical examination and endoscopic Müller's maneuver. However, this prediction remains speculative. To objectively evaluate these clinical measures, 22 patients with severe obstructive sleep apnea had upper-airway studies during nonsedated sleep with solid-state manometry and videoendoscopy. Objective tongue-base obstruction was measured with four methods, each identifying a different aspect of airway collapse during sleep. Manometry identified tongue obstruction during (1) early and

Pediatric bronchoscopy during a 17-year period.

From 1969 through 1985, 1032 pediatric bronchoscopies were performed on a total of 748 children under 16 years of age. Of the children 27.4% were under 6 months of age and the smallest patient weighed 600 g. Rigid bronchoscopes with a diameter ranging from 2.5 to 6 mm were used. A flexible fiberoptic bronchoscope with a diameter of 3.5 mm was used only occasionally, mostly for the diagnosis of laryngeal dynamics. All the endoscopies were performed under general anesthesia, except for some laryngoscopies performed with a flexible bronchoscope. The most common indications for bronchoscopy were s

The use of intermittent and continuous recordings of jugular venous bulb oxygen saturation in the unconscious patient.

Monitoring clinical signs in unconscious patients provides only late information about cerebral deterioration. Ischaemia and hypoxia are the mechanisms of much of the damage. Cerebral blood flow (CBF) measurements provide direct evidence of ischaemia but are intermittent values for what may be an unstable situation. Continuous recordings of CBF and oxygenation are more likely to reveal harmful tendencies to ischaemia and hypoxia at an early stage than intermittent readings. We report our experience with intermittent and also continuous recording of the jugular venous bulb oxygen saturation (JV

Contribution of modern imaging in rectosigmoid neoplastic disease (comparison with fiberoptic endoscopy).

Double-contrast enema (DCE) remains the primary diagnostic exploration. Its specific method and interpretation require skillful handling. Unlike colonoscopy, DCE results in false-positives, the frequency of which varies widely in function of the skill of the interpretation. False negatives reach a significant level from a threshold of size which can be fixed at 5 mm. Despite these drawbacks, double-contrast enema, which is an exploration involving a two level of invasiveness and which provides a good panoramic image of the colon should remain the primary exploration in routine check-ups.
