Management for Bony Mallet Thumb with a Single Extension Blocking Kirschner Wire.

INTRODUCTION: Mallet finger injury is defined by disruption of the terminal extensor tendon distal-to-distal interphalangeal (DIP) joint. While in the fingers, it is a relatively common injury, it is a rarely encountered entity when involving the thumb. Various conservative and operative treatment strategies have been reported for the management of mallet thumb with no consensus by clinicians. CASE REPORT: We present the case of a 27-year-old right hand dominant man with a left bony mallet thumb injury that occurred while playing hurling. Hurling is traditional Irish sport that is one of the

Congenital flexion deformities of the thumb.

A congenital flexion deformity of the IP joint of the thumb is usually due to a trigger finger. The diagnosis is based on the thumb being normal except for a fixed, flexed posture of the IP joint and a palpable nodule in the FPL tendon at the level of the A-1 pulley. Treatment can be postponed until after age 1, as spontaneous regression probably occurs in about 30 per cent of cases. After that age, surgical correction is the best approach to treatment and usually results in normal thumb function. Congenital soft tissue flexion deformities of the MP joint of the thumb vary from passively corre

The Foot as Donor Site for Functional Unit Reconstruction in the Hand with Toe Transfer Variations.

The foot is an ideal donor site for reconstruction of defects in the hand. Although standard toe transfer variants such as great or second toe transfer are commonly used, other variants can be designed, depending on the requirements of the defect. In this article, the authors illustrate the use of toe transfer variants to reconstruct different defects in the hand following functional and aesthetic units. Where possible, all 10 toes should be preserved. Specifically, we discuss reconstruction of pulp defects, nail complex defects, proximal interphalangeal joint defects, web space defects, compl

Histologically Proven Recurrent Synovitis after Nivolumab Treatment.

A 58-year-old woman with rheumatoid arthritis was diagnosed with methotrexate-associated Hodgkin lymphoma. After receiving several chemotherapy regimens, she started nivolumab treatment. Two weeks later, she was hospitalized with worsening finger, wrist, and elbow joint pain. A synovial biopsy of the wrist joint showed villous synovial proliferation and linear infiltration of CD68-/CD3-positive T cells (with more CD8 than CD4 T cells) but no CD20-positive B cells or CD138-positive macrophages. These findings corresponded to synovitis associated with immune-related adverse events, which are ind

Neuropathy of motor branch of median or ulnar nerve induced by midpalm ganglion.

Two cases of neuropathy of a motor branch caused by a midpalmal ganglion are presented. In the first case the ganglion originated from the midcarpal joint, protruded into the thenar muscle, and compressed the motor branch of the median nerve. In the second case the ganglion, distal to the fibrous arch of the hypothenar muscles, originated from the third carpometacarpal joint and compressed the motor branch of the ulnar nerve. In both cases muscle weakness and finger deformity recovered well after resection of the ganglion. This clinical condition is rare compared with carpal tunnel syndrome an

Trapeziometacarpal abnormalities in Ehlers-Danlos syndrome.

We studied 24 patients with Ehlers-Danlos syndrome with particular attention to the thumb and the trapeziometacarpal joint. The 11 males and 13 females had a mean age of 15.9 years. Symptomatic complaints included pinch, grip and twisting weakness; measurements showed dominant hand strength deficits of 71% for tip pinch, 55% for palmar pinch, 46% for key pinch, and 70% for grip. All finger joint ranges of motion were increased. Sixty-six percent of all the patients had trapeziometacarpal subluxation; 29% had dislocation. Sixteen percent had radiographic evidence of arthritis. Early occupationa…

The free triple chimeric dorsalis pedis flaps for repair of multifinger soft tissue defects: a report of two cases.

The reconstruction of complex hand injury such as multifinger soft tissue defect remains a challenging problem. Two cases of repair of multifinger injury with exposed bones using the free chimeric flaps based on the dorsalis pedis vessels are presented. Two male patients, 46 years old and 36 years old, suffered from a thermocompression injury to the dorsum of fingers resulting in soft tissue defects of multiple fingers. The chimeric free flap was designed and applied to cover the defects. The donor sites were covered by skin grafts. The postoperative courses were uneventful. Both patients were

An evaluation of a clinical method to assess malunion of little finger metacarpal fractures.

This study assessed the reliability, responsiveness and validity of two clinical measurements for the assessment of malunion of little finger metacarpal neck and shaft fractures. Both compared the relative lengths of the ring and little fingers in the injured and contralateral hands. One measurement was taken with the metacarpophalangeal joints extended (straight-MCP), and the other with them flexed to 90 degrees (90-MCP). Ninety-five percent of the differences between the relative lengths of the ring and little fingers in the two hands of 50 normal subjects were less than 3mm and the 95% limi

Finger-level avulsion-type injuries.

Fifty-nine avulsed fingers were managed between May 1986-December 2004. Of 59 cases, 39 (66%) were treated with microsurgical techniques, and the remaining 20 cases (34%) were treated in the conventional manner. Follow-up time varied between 9-18 years. All cases were classified according to the Kay-Wolff classification. Replantation was done in 26 fingers, and revascularization in 13 fingers. The survival rate was 100% in the revascularization cases, and 80.7% in the replantation cases. The total active range of motion was more than 90 degrees in the interphalangeal joints in 48% of cases, an

Anatomy of the flexor digitorum profundus insertion.

PURPOSE: To describe the anatomical insertion of the flexor digitorum profundus (FDP) on the distal phalanx. METHODS: The FDP insertion to the index, middle, ring, and little fingers were dissected in 10 fresh-frozen cadavers. The FDP tendon was dissected off the volar plate, which was elevated from proximal to distal, before the distal phalanx was disarticulated. The distal phalanx was then inked and the FDP was sharply dissected from bone. The insertion length, width, and distance of the insertion from the joint were measured and the insertion surface area and centroid of the FDP insertion

[Lumbar facet syndrome. Recommendation for staging before and after intra-articular injection treatment].

INTRODUCTION: Lumbar facet joint syndrome is normally based on spondylarthrosis. One of the most common methods of treating it is intra-articular injection of local anaesthetics and cortisone. In this prospective study our goal was to have an objective grading scale to assess the severity of lumbar facet syndrome before and after injection. METHODS: Thirty-two patients were treated by CT-guided intra-articular injections of 0.3 ml bupivacaine and 0.8 ml methylprednisolone. Five different parameters were scored in each patient: (1) finger-ground distance; (2) Schober index (10 cm distance alon

Traumatic correction of Linburg-Comstock anomaly: a case report.

Linburg-Comstock anomaly describes an anatomical variant of flexor tendons of the hand. Flexor pollicis longus (FPL) sends a connecting tendon to flexor digitorum profundus (FDP), causing simultaneous flexion at the distal interphalangeal joint (DIPJ) of the index finger when the interphalangeal joint (IPJ) of the thumb is flexed. Epidemiological studies have revealed a unilateral prevalence as high as 31% of individuals; however, the condition rarely causes symptoms. The anomaly can present with a restrictive flexor tenosynovitis, requiring explorative surgery to confirm the diagnosis and dis

Kinematical and functional improvements of trigger digits after sonographically assisted percutaneous release of the A1 pulley.

We present evidence of kinematic and functional improvements of the trigger digit after sonographically assisted percutaneous release using quantitative motion analyses and functional assessments, respectively. Twenty-two patients with idiopathic trigger digits, who received the surgery, were recruited. A motion tracking system was used to measure the assigned kinematics parameters at the presurgical visit and at 1, 4, and 12 weeks after surgery. The hand strength and functional outcome were also obtained using dynamometer and self-report questionnaire assessments. The hand strength, maximal w

[Transport of finger parts using an abdominal skin flap for finger and thumb reconstructive plastic surgery].

1st. With the aid of an abdominal or an inguinal flap it is possible to reconstruct a finger with sensibility by transporting islands of palmar skin with the corresponding nerve to any amputation stump of the hand. At least three operations are necessary. Parts of bones, joints and tendons may be included in the transported part from the hand. 2nd. These digits are rather bulky. Mobility depends on the preserved joints. 3rd. This method should only be used for thumb reconstruction or to achieve a simple two digit grip. 4th. In children growth of transported bones remains undisturbed.
