Chronic recurrent multifocal osteomyelitis with a comprehensive approach to differential diagnosis of paediatric skull pain.

A girl in middle childhood was referred to rheumatology with a 1-month history of progressive skull pain, preceded by fleeting musculoskeletal symptoms. Apart from a scaly rash on her scalp, she was well, with moderately elevated inflammatory markers. Skull imaging (radiographs, CT and MRI) revealed osteolytic lesions, soft tissue swelling and pachymeningeal enhancement at frontal and temporal convexities. Langerhans cell histiocytosis, bone infection/inflammation or malignancy was considered. Skin and bone biopsies eventually ruled out mimicking diseases and confirmed the diagnosis of chronic

Hydroxyapatite cement in craniofacial skeletal reconstruction and its effects on the developing craniofacial skeleton.

OBJECTIVE: To assess the effects of hydroxyapatite cement (HAC) on the developing feline craniofacial skeleton. DESIGN: Fronto-orbital craniotomies were performed on 14 kittens and reconstructed by autograft or HAC. By design, animals in which the craniofacial skeleton was reconstructed with HAC also underwent obliteration of the left frontal sinus. After achievement of skeletal maturity, animals were sacrificed and compared by 11 standardized cranial measurements obtained by sliding caliper. Additional analyses included histological studies, histomorphometry, and computed tomography. SUBJEC

Infantile Osteopetrosis with a CLCN7 Variant: An Educational Case Highlighting Integrated Diagnosis and Variant Interpretation.

BACKGROUND: Infantile osteopetrosis is a rare inherited disorder caused by defective osteoclast-mediated bone resorption, resulting in increased bone density and marrow failure. The CLCN7 gene encodes a chloride channel essential for osteoclast acidification. CASE PRESENTATION: A 4-month-old male presented with failure to thrive, pallor, hepatosplenomegaly, and recurrent infections. Examination revealed frontal bossing and hypotonia. Laboratory evaluation showed anemia and persistent hypocalcemia (6.8 mg/dL; reference 8.5-10.5 mg/dL). Skeletal survey demonstrated diffuse osteosclerosis with a

Fontanellar bone - A rarity in pediatric cranial abnormalities.

BACKGROUND: Fontanelles, membranous gaps in the infant skull, are integral for accommodating the expansion of the skull by the growing brain postnatally. The anterior fontanelle, situated at the frontal-parietal bone intersection, typically closes gradually within the first two years. Fontanellar bone, an exceedingly rare ossification anomaly of the anterior fontanelle, clinically mimics craniosynostosis. CASE DESCRIPTION: We present the case of a 22-day-old male with an almost closed anterior fontanelle who underwent evaluation. Prenatal and postnatal history were unremarkable. Physical exam

Perilymphatic fistula and pneumolabyrinth without temporal bone fracture: a rare entity.

Post-traumatic pneumolabyrinth is an uncommon clinical entity, particularly in the absence of temporal bone fracture. We report the case of a patient who presented to our emergency department with a headache, sudden left hearing loss and severe dizziness which began after a traumatic brain injury 3 days earlier. On examination, the patient presented signs of left vestibulopathy, left sensorineural hearing loss and positive fistula test, normal otoscopy and without focal neurological signs. The audiometry confirmed profound left sensorineural hearing loss. Cranial CT revealed a right occipital

Less-lethal hybrid ammunition wounds: a forensic assessment introducing bullet-skin-bone entity.

Agencies all around the world now use less-lethal weapons with homogeneous missiles such as bean bag or rubber bullets. Contusions and sometimes significant morbidity have been reported. This study focuses on wounds caused by hybrid ammunition with the pathologists' flap-by-flap procedure. Twenty-four postmortem human subjects were used, and lesions caused on frontal, temporal, sternal, and left tibial regions by a 40-mm hybrid ammunition (33 g weight) were evaluated on various distance range. The 50% risk of fractures occurred at 79.2 m/sec on the forehead, 72.9 m/sec on the temporal, 72.5 m/

Valosin-containing protein Asp395Gly mutation in a patient with frontotemporal dementia: a case report.

BACKGROUND: Variants in the valosin-containing protein (VCP) gene were identified as one of the causes for inclusion body myopathy associated with Paget disease of the bone and frontotemporal dementia (FTD). Previously identified pathogenic variants in VCP are associated with frontotemporal lobar degeneration with TDP-43 inclusions (FTLD-TDP) pathologically, but p.Asp395Gly VCP was recently reported to cause familial FTD with tauopathy characterized by neurofibrillary tau tangles (NFT) and not FTLD-TDP. We describe the clinical and genetic findings of a patient with p.Asp395Gly valosin-contain

A new in utero sheep model for unilateral coronal craniosynostosis.

Several animal models have been designed in the past to analyze the pathophysiology and management of craniosynostosis, very few of which we … the right coronal suture occurred in all fetuses by 90 days gestation. In every animal, right-sided frontal bone flattening and supraorbital rim elevation were evident. Histologic analysis showed bony synostosis at the suture site without evidence of suture regeneration. By 140 days, this isolated suture fusion led to marked craniofacial abnormalities includin …

Frontal Sinus Fracture Management Meta-analysis: Endoscopic Versus Open Repair.

INTRODUCTION: Frontal sinus fracture management continues to be a point of controversy. Many systematic reviews have been done, but meta-analyses comparing various approaches to frontal sinus fractures are scarce. Our study focuses on open surgical repair versus endoscopic repair of frontal sinus fractures. MATERIALS AND METHODS: A MEDLINE search was performed to identify prospective RCTs, non-RCTs, guidelines, case-control, and observational studies published in English before April 2019. Search terms included (1) frontal sinus, (2) skull fractures, (3) frontal bone, alone or in combination.…

Blindness as a complication of monobloc frontofacial advancement with distraction.

The monobloc frontofacial osteotomy provides aesthetic and functional improvement in the treatment of various craniofacial deformities. This procedure, through highly complex, has had some significant associated complication, such as cerebrospinal fluid leakage, hematoma, infection, and bone resorption. Distraction has been successfully used to gradually elongate bone and soft tissue. This method seems to provide improved results over conventional surgery, with less morbidity. We present a case of a patient with Apert syndrome who underwent monobloc advancement using the Rigid External Device

Frontofacial osteotomies, advancement, and remodeling by distraction: an extended application of the technique.

The purpose of this clinical report is to present the distraction technique for advancement of the frontofacial skeleton as a unit. Our 14-year-old patient was diagnosed with Carpenter's syndrome and kleblattschädel deformity at birth. At other centers the patient underwent corrective surgeries, including repeated fronto-orbital advancement in an attempt to correct the residual deformity. This has resulted in bony malunion and recurrent deformity, and it has left the patient with no available donor sites for harvesting of bone graft. The patient had class III malocclusion, severe midfacial and

Management of embossment following the frontal osteoplastic operation.

The deformity in the bony contour of the forehead following the osteoplastic flap procedure on the frontal sinus, termed embossment, occurs in a small but definite number of cases. This complication arises from the combined processes of resorption and deposition of bone following disturbance of the periosteum in performing the operation. A simple and highly reliable method of cosmetic correction employing acrylic cranioplasty has been successfully utilized in a series of cases without complications. The details of the operative technique will be outlined.

The effects of fronto-parieto-squamosal suture fusion on cranial growth: an experimental study.

Craniosynostoses have been known for at least 20 centuries, but their etiopathogeneses are still unclear. There are three main theories to explain the etiology of craniosynostosis, Moss's theory being the most popular. According to Moss, the development of the neurocranial form is viewed as an integration of the growth of the skull base, the calvarial bones, the meninges, and the enclosed brain. Experimental studies, however, are generally focused on cranial vault suture synostosis. This study aims to demonstrate the effects of cranial base synostosis by performing fronto-parieto-squamosal sut

A combined frontoorbital and occipital advancement technique for use in total calvarial reconstruction.

The management of infants with bilateral coronal synostosis and resultant brachyturricephaly poses a significant therapeutic challenge. The application of total calvarial reconstruction to the treatment of this problem has represented a major recent innovation that has substantially improved the cosmetic results in this patient population. However, rigid fixation of the reconstructed calvaria is often required to maintain the correction achieved and to provide protection for the underlying brain. The requirement of extensive fixation constitutes a significant disadvantage for the use of this p

Myxoma of the skull and orbit.

An unusual case of myxoma of the skull extending into the orbital cavity is reported. Myxoma is a benign tumour of the soft tissues and maxillary bones, which often presents infiltrating growth and recurrences. Histologically it is characterized by abundant myxoid stroma and is often confused with other tumours which can present mucoid degeneration. Although diagnosis is impossible before histological examination, the possibility of a myxoma must be considered in the presence of extradural osteolytic poorly vascularized lesions.
