Effectiveness of scenario-based simulation teaching on advance care planning competencies in geriatric nurses: A quasi-experimental study.

AIM: To compare the effectiveness of scenario-based simulation versus case-based teaching in improving geriatric nurses' multiple competencies for Advance Care Planning (ACP). BACKGROUND: Geriatric nurses are pivotal in ACP but often lack adequate preparedness, necessitating training that addresses both knowledge and behavioral-attitudinal components. Simulation-based education offers a safe environment to develop such complex clinical skills. DESIGN: A quasi-experimental, non-equivalent control group pretest-posttest design. METHOD: Within a provincial geriatric nursing training program in

STRENGTH and the Health Care Team: Changing Interprofessional and Client-Centered Practices.

BACKGROUND: Client-centered practice in stroke rehabilitation is strongly influenced by interprofessional team functioning and collaborative goal setting. The hospital context is problematic for client-centered practice and reduces the ability of the health care team and client with stroke to appreciate the impact of stroke on functioning within the home environment. OBJECTIVE: The purpose of this study was to trial Stroke Rehabilitation Enhancing and Guiding Transition Home (STRENGTH), an approach to rehabilitation that provides clients, carers, and hospital-based therapists with weekly oppo

Prevalence of hyperaldosteronism in primary care patients with resistant hypertension.

INTRODUCTION: Because hyperaldosteronism is the most common curable reason for secondary hypertension, screening is recommended. However, prevalence among general practice patients and feasibility of screening is still unclear. A design to assess prevalence in general practice and barriers against screening was created. METHODS: This was an open, observational pilot study and focus group. In 2 general practices, all patients with arterial hypertension were included. Those with resistant hypertension (>140/90 mm Hg and taking ≥3 antihypertensive drugs) were eligible for screening. The design a

Standardization as a mechanism to improve safety in health care.

BACKGROUND: A growing body of literature shows that when patterns of care are widely divergent, clinical outcomes suffer and, as a result, safety may be compromised. A multispecialty group at Luther Midelfort, Mayo Health System (LM, MHS) initiated efforts to reduce variance in the clinical practice patterns of providers. The pilot initiative, which entailed standardization of a sliding-scale insulin protocol, served as a template throughout the LM, MHS for reducing variance and enhancing safety. STANDARDIZING INSULIN ADMINISTRATION: A single sliding-scale insulin protocol for regular insulin

Knowledge transfer in family physicians managing patients with acute low back pain: a prospective randomized control trial.

BACKGROUND CONTEXT: The process through which new scientific developments are incorporated into clinical practice is referred to as "knowledge transfer" and is currently the subject of great interest in many areas of clinical medicine. Family physicians managing patients with acute low back pain have been shown to have a poor overall rate of concordance with clinical practice guideline-recommended treatments. New methods need to be developed to help physicians bridge the guideline implementation gap. PURPOSE: To determine the efficacy of a knowledge transfer method that communicates clinical

Training for Change: Developing Clinical Debriefing Champions in a Maternity Unit.

BACKGROUND: Clinical debriefing (CD) is a structured reflective conversation that supports experiential learning, enhances teamwork and contributes to patient safety. Despite established evidence for its benefits, implementation of CD within clinical environments remains challenging, with cultural barriers frequently cited as problematic. As part of a wider programme to embed CD within a busy maternity unit, a 1-day course in CD facilitation was delivered to an interprofessional group to develop debriefing skills and empower a group of staff to commit to CD practice. APPROACH: Course design w

The effect of using intra-oral camera as an educational aid for oral health promotion in children.

PURPOSE: The aim of this study was to evaluate the effect of using an intraoral camera (IOC) as an educational aid for promoting oral health among 7-10-year-old children in India. MATERIALS AND METHODS: A randomized trial was conducted involving 260 children from private schools. The intervention group (n = 130) received IOC-based oral health education, while the control group (n = 130) received verbal education. Both groups were assessed for knowledge, attitude, and practice (KAP) towards oral health, as well as the DMFT/deft index at baseline and after 12 months. Statistical analyses were p

Breast implant illness revisited: A cohort study of health symptoms in women with implant-based reconstruction.

BACKGROUND: This study investigated the association between silicone breast implants and health symptoms believed to be related to breast implant illness (BII) in women who underwent implant-based breast reconstruction after breast cancer. Although there is increasing evidence of an association between BII and breast augmentation cases, it is unclear in women with implant-based breast reconstruction. METHODS: This retrospective cohort study involved 254 women who underwent implant-based reconstruction after breast cancer treatment and a control group comprising 487 women who underwent breast-

"Practice so that the skill does not disappear": mixed methods evaluation of simulator-based learning for midwives in Uganda.

BACKGROUND: Postpartum hemorrhage and neonatal asphyxia are leading causes of maternal and neonatal mortality, respectively, that occur relatively rarely in low-volume health facilities in sub-Saharan Africa. Rare occurrence of cases may limit the readiness and skills that individual birth attendants have to address complications. Evidence suggests that simulator-based training and practice sessions can help birth attendants maintain these life-saving skills; one approach is called "low-dose, high-frequency" (LDHF). The objective of this evaluation is to determine the facilitating factors and
