The value of early and late 99mTc-methylene diphosphonate scintigrams of hands in patients with rheumatoid arthritis and other inflammatory rheumatic diseases.

Scintigrams of hands performed immediately after injection of 555 MBq 99mTc-methylene disphosphonate (99mTc-MDP) have been found to be the same and to have the same diagnostic value as those performed with 99mTcO4 both in persons without arthritic changes and in patients with inflammatory rheumatic diseases. Increased accumulation at this point is caused by hypervascularity due to inflammation, bone accretion being still negligible. Additional exposure to radiation from 99mTcO4 scintigraphy can therefore be avoided. The late scintigrams performed 3 h after 99mTc-MDP injection in the same patie

Humeroradioulnar Synostosis in a Patient with Underlying Congenital Scoliosis: A Case Report.

INTRODUCTION: Synostoses, or fusions, of the bones of the elbow are rare orthopedic occurrences that result from abnormal segmentation of the humerus, radius, and/or ulna during fetal development. The upper extremity begins as a single cartilage anlage in utero that segments into a distinct humerus, radius, and ulna and when disrupted, synostoses of the elbow occur. The most uncommon synostosis reported is the humeroradioulnar synostosis, in which all three bones of the elbow are fused. These synostoses often occur in conjunction with other orthopedic anomalies, as well as defects of other bod

Pathogenesis of Bone Alterations in Gaucher Disease: The Role of Immune System.

Gaucher, the most prevalent lysosomal disorder, is an autosomal recessive inherited disorder due to a deficiency of glucocerebrosidase. Glucocerebrosidase deficiency leads to the accumulation of glucosylceramide primarily in cells of mononuclear-macrophage lineage. Clinical alterations are visceral, hematological, and skeletal. Bone disorder in Gaucher disease produces defects on bone metabolism and structure and patients suffer from bone pain and crisis. Skeletal problems include osteopenia, osteoporosis, osteolytic lesions, and osteonecrosis. On the other hand a chronic stimulation of the im

[The relationship between bone and glucose/lipid metabolism].

The fracture risks are increased in patients with lifestyle-related diseases such as diabetes mellitus and dyslipidemia. Bone has been recognized as an endocrine organ to regulate glucose and fat metabolism. Hyperglycemia, advanced glycation end products (AGEs) , and insulin signal are involved in diabetes-related bone disease. Previous studies suggest that hypercholesterolemia may increase the risk of fractures. Adipokines such as leptin and adiponectin derived from fat tissue, which are important regulators for glucose and lipid metabolism, regulate bone metabolism. On the other hand, it has

Rare localizations of bone sarcoidosis: two case reports and review of the literature.

Sarcoidosis is a multisystem disease of unknown origin. Granulomatous bone involvement has an overall incidence of 1-13%. This incidence is probably underestimated in certain patient series because bone involvement is often asymptomatic. The small bones of hands and feet are the most common localizations, while skull, knee, rib, pelvic and sternal localizations are rarely reported. Here we describe some interesting cases of chronic sarcoidosis with unusual bone localizations observed at our regional referral centre for sarcoidosis. We also review the literature to underline the complexity of t

Management of the Hand Tumors.

Tumors in the hand are relatively uncommon but 95% are benign. Tumors occurring in the hand, forearm and arm often have unique growth patterns and potential for metastasis that may be different from those seen elsewhere in the body. Secondary metastatic tumors in the hand are very rare (0.1%). Diagnosis is mainly clinical, but X-ray, USG and MRI help as a diagnostic aid. The aim of the study was to early diagnosis, see the pattern and proper management of the hand tumor and ensure good hand function. This prospective study was done from January 2004 to July 2019. We found 220 hand tumors in th

Enchondroma of the scaphoid: a case report.

Enchondroma represents a common bone tumor of the hand. The scaphoid is a rare location. We report the case of a scaphoid enchondroma presenting as chronic wrist pain following a relatively minor trauma. The diagnosis was suggested by radiological study and then confirmed by histological study after biopsy. Enucleation and vascularized bone graft with osteosynthesis provided good results. The clinical, radiological and therapeutic aspects of this rare condition are discussed.

[Greig's syndrome. Neonatal radiologic manifestations].

A newborn infant with postaxial polydactyly of the hands, preaxial polydactyly of the feet, syndactyly and craniofacial dysmorphism (Greig's Syndrome) is reported. The radiographic manifestations in the neonatal period are described. The advanced bone age, the unusual shape of the iliac bones and the supernumerary prehallux toe are new radiological findings in Greig's Syndrome at this age. The diffuse distribution of skeletal anomalies suggests a more complex problem of the intrauterine process of ossification.

"Fever in the Hand".

A 14-year-old female adolescent was being treated for malnutrition secondary to anorexia nervosa. She complained of severe "shooting" pain, swelling, and color and temperature changes in her left (dominant) hand. She had recently suffered a fall during a hockey game and pounded on the table during a "temper tantrum." On exam, the left hand was cyanotic, edematous, and cold, with severe hyperesthesia. A bone scan showed marked reduction in flow to the left hand. She underwent eight sympathetic ganglion blocks resulting in transitory Horner's syndrome. Favorable plethysmographic changes occurred

[Changes in certain biochemical markers of bone turnover in rheumatoid arthritis patients treated with short-term low dose glucocorticosteroids].

Osteoporosis associated with rheumatoid arthritis (RA) is induced by chronic inflammation. Glucocorticosteriods (GCS) applied in the treatment of RA chronically reduce production of proinflammatory cytokines (IL-1, IL-6, and TNF) which are potent stimulators of bone resorption. On the other hand they directly reduce bone mass by inhibition of osteoblast. In order to assess bone turnover the following parameters have been measured: Alkaline phosphatase (AP), alkaline phosphatase-bone formation (AP-B), deoxypirydynoline (Dpd) and carboxyterminal telopeptides of type I collagen (CTx). Based on th

Tumor resection and antibodies to parathyroid hormone-related protein cause similar changes on bone histomorphometry in hypercalcemia of cancer.

Bone resorption is increased in both humoral hypercalcemia of malignancy (HHM) and primary hyperparathyroidism. On the other hand, bone formation parameters are increased in primary hyperparathyroidism and decreased in HHM. Recently, a PTH-related protein (PTHrP) has been shown to be responsible for the hypercalcemia in the syndrome of HHM. In the present study we evaluated the effects of a neutralizing antiserum to PTHrP on bone histomorphometric parameters in hypercalcemic athymic mice bearing a human squamous cell lung cancer. These effects were compared to those of tumor resection. Similar

Microsurgical two-stage reconstruction of the thumb. Case report.

A 40-year-old patient presented with traumatic amputation of the thumb and index finger. The second metacarpal bone fragment was transposed to lengthen the first metacarpal bone. A free flap from the lateral forearm was used to reconstruct the thenar region. Later, the second free toe-to-hand transfer was done from which he made a good recovery, achieving active flexion and extension in all joints.

[Metacarpal shaft reconstruction using medial femoral condyle flap: A case report].

We report the technique applied to reconstruct a whole shaft defect of the 4th metacarpal bone in a 22-year-old women after aneurysmal bone cyst resection. Local invasion leads to possible poor revascularization possibilities, justifying the use of a vascularized bone transfer. Surgical procedure consisted in a 5-centimeter free medial femoral bone flap transfer. Two months after surgery, no after effect was found at donor site and bone consolidation was complete. Bone fixation allowed early active motion and a complete recovery of flexion-extension range was present 1 year after surgery. Free

Regulation of osteoclast differentiation and function by interleukin-1.

Interleukin-1 (IL-1) is a multifunctional cytokine that regulates various cellular and tissue functions. Among tissues, bone is the most sensitive to IL-1. IL-1 is a potent cytokine for bone resorption and participates in the multiple steps of osteoclast recruitment, such as differentiation, multinucleation, activation, and survival. On the other hand, considerable evidence has been accumulated over the past 10 years to indicate that this cytokine plays key roles in pathological bone destruction in a variety of human diseases, including rheumatoid arthritis, osteoporosis, and periodontal disea

[Association of mycosis fungoides and Jaccoud syndrome].

We report here a case of a 49 year-old woman who developed a unilateral Jaccoud's arthropathy 9 years after a mycosis fungoides of the left hand. This rheumatic affection, described in 1866, is a deformation of the hands or of the feet linked to a periarticular disease without any bone lesion. Many causes have been registered but the most frequent etiology is the systemic lupus erythematosus. The mycosis fungoides had never been reported as yet to be responsible to the Jaccoud's arthropathy.

[Skeletal thumb reconstruction by vascularized metacarpal II transposition. Anatomic study and clinical case reports].

In complex hand injuries with destruction of the index finger, improved function is achieved by resection of the second metacarpal. We examined the possible use of the second metacarpal for reconstruction of simultaneous bony thumb defects. Its vascular anatomy was studied by dissection of 104 cadaver hands that had been injected with "Kleistermasse". Six arterial patterns were found, all of them useful for transposition of the second metacarpal into a thumb defect. The use of this new vascularized pedicle bone graft is demonstrated.

[The type and frequency of hand skeletal anomalies and their significance for body height growth].

The frequency of minor skeletal anomalies of the hand, such as brachymesophalangia, pseudo- and cone-shaped-epiphyses, as well as shortness of the fourth metacarpal bone were investigated in almost 5000 healthy children, undergoing orthodontic treatment. In addition body length was compared to the percentile curves of Kunze. Surprisingly, only 42% of all hand films demonstrated normal morphology. Certain anomalies could be found more frequently in girls than in boys and these seemed to exert an inhibitory effect on growth potential.

Burden of carpal tunnel syndrome and its associated factors among construction industry workers in Gondar town, Ethiopia.

BACKGROUND: Carpal tunnel syndrome is characterized by symptoms such as pain, numbness, or tingling on the anterior surface of the index, middle, or radial half of the ring finger, which is frequently associated with weakness of hand grip, and nocturnal pain and/or numbness resulting from compression of the median nerve at the carpal tunnel between the carpal bones and the transverse ligament. The construction industry involves many activities aside from the building process, such as landscaping, painting, electrical supply, plastering, paving, and telecommunications. Performing such tasks inv
