Exploring drug checking services for people who use drugs: Pre-implementation study.

BACKGROUND: The growing proportion of adulterants in the street drug supply has increased overdose risk and other negative health outcomes f … , we sought to understand facilitators and barriers to FTIR among potential participants at a local harm reduction agency. METHODS: Potential DCS participants were recruited (n = 25) into three focus groups (n = 8-9) and one individual interview. Semi-structured interview guides informed by the Consolidated Framework for Implementation Research (CFIR) were used …

"All it's gonna tell you is if it has fentanyl or not:" Perceptions of fentanyl and drug checking among first responders and people who use drugs.

INTRODUCTION: Fentanyl contamination in the unregulated drug supply contributes to high rates of opioid overdose among people who use drugs (PWUD). Drug checking is a harm reduction strategy that provides PWUD with information about the contents of their substances. Although drug checking has broad utility, this study focused specifically on drug checking to test for fentanyl, with a particular emphasis on fentanyl test strips (FTS) and point-of-care testing at community service organizations. This study assessed first responders and PWUDs' perceptions of fentanyl, drug checking, and associate…

Knowledge of Good Samaritan Laws and Beliefs About Arrests Among Persons Who Inject Drugs a Year After Policy Change in Baltimore, Maryland.

OBJECTIVES: Delivering and receiving prompt medical care during an overdose are imperative to ensure survival. Good Samaritan laws encourage people to call 911 during an overdose by providing immunity from selected drug arrests (eg, low-level possession). However, it is unclear whether persons who inject drugs (PWID) are aware of and understand these laws and their implications. We examined awareness among PWID of the 2015 Good Samaritan law in Maryland and their beliefs about whether they could be arrested for calling 911 or having an overdose. METHODS: We surveyed 298 PWID in Baltimore, Mar

Expanding low-threshold buprenorphine to justice-involved individuals through mobile treatment: Addressing a critical care gap.

BACKGROUND: Opioid use disorder (OUD) is highly prevalent among justice-involved individuals. While risk for overdose and other adverse consequences of opioid use are heightened among this population, most justice-involved individuals and other high-risk groups experience multiple barriers to engagement in opioid agonist treatment. METHODS: This paper describes the development of Project Connections at Re-Entry (PCARE), a low-threshold buprenorphine treatment program that engages vulnerable patients in care through a mobile van parked directly outside the Baltimore City Jail. Patients are ref

Overdose mortality rates in Croatia and factors associated with self-reported drug overdose among persons who inject drugs in three Croatian cities.

BACKGROUND: Drug overdose is the major cause of morbidity and mortality among persons who inject drugs (PWID). We assessed factors associated with the non-fatal drug overdose among PWID in three Croatian cities and national trends of overdose-related mortality (OM), and rates of uptake of opioid agonist drug treatment (OAT). METHODS: We used a respondent-driven sampling method to recruit 830 PWID in Zagreb, Split and Rijeka in 2014/2015. Participants completed behavioural questionnaires that included questions about overdose history, and we used Poisson regression to assess factors associated

Fatal and non-fatal overdose among opiate users in South Wales: A qualitative study of peer responses.

BACKGROUND: Overdose is a major cause of death among injecting drug users in Wales. Few studies, however, have explored the overdose respons … peers. However, a number of micro- and macro-level factors impeded the successful implementation of harm reduction techniques in response to an overdose. At micro level, the social setting of injecting drug use, peer group drug use norms and difficulties in identifying an overdose were linked to ineffective response. Macro-level factors including laws governing …

Reward Dependence and Harm Avoidance among Patients with Binge-Purge Type Eating Disorders.

INTRODUCTION: The Cloninger's Psychobiological Model of Temperament and Character includes temperamental dimensions such as reward dependence (RD) and harm avoidance (HA). Studies of RD differentiate between sensitivity to reward (SR) versus to punishment (SP). We investigated the interrelationship between HA and RD in acutely ill patients with binge/purge (B/P) type eating disorders (EDs) and following symptomatic stabilization. METHODS: Fifty patients with B/P EDs were assessed at admission to inpatient treatment, 36 of whom were reassessed at discharge. Thirty-six controls were similarly a

Social phobia, fear of negative evaluation and harm avoidance.

This naturalistic, prospective investigation examined the role of fear of negative evaluation and the personality trait of harm avoidance in the anxiety levels of treated social phobia patients. One hundred and fifty-seven patients with DSM-IV social phobia were assessed before starting treatment and were then followed for up to two years. As expected, greater fear of negative evaluation and higher scores of harm avoidance were associated with greater anxiety at the 6 month follow-up, and harm avoidance remained a significant predictor at 24 months. However, no evidence was found for an intera

Non-Medical Opioid Use in Ethiopia: An Exploratory Descriptive Study.

BACKGROUND: Non-medical opioid use is linked to severe consequences such as addiction, overdose, and infectious diseases, including human immunodeficiency virus (HIV), hepatitis B virus (HBV), and hepatitis C virus (HCV), particularly through injection practices. In Ethiopia, limited evidence exists on non-medical opioid use, and harm-reduction programs are absent. This study aimed to explore the experiences of non-medical opioid users seeking healthcare in Addis Ababa, Ethiopia. METHODS: A phenomenological qualitative study design was employed, complemented by descriptive quantitative data.

Motivations underlying co-use of benzodiazepines and opioids in the UK: a qualitative study.

BACKGROUND: Drug-related deaths have substantially increased over the past decade in the UK, particularly in Scotland. Co-use of opioids and benzodiazepines (prescribed and/or illicit) is contributing to rising mortality. This study identified motivations in people's co-use with the aim of informing prescribing and harm reduction interventions to address drug-related deaths. METHODS: We interviewed 48 people who co-use opioids and benzodiazepines and/or z-drugs (zopiclone and zolpidem) in Glasgow (n = 28), Teesside (n = 10) and Bristol (n = 10). Most participants self-identified as male (n = …

Unit costs of opioid agonist therapy: a global systematic review and cost extrapolation.

BACKGROUND: People who inject drugs (PWID) face elevated risks of HIV and viral hepatitis transmission. Opioid agonist therapy (OAT) like methadone and buprenorphine reduce drug-related harms and improve health outcomes among PWID. Despite effectiveness, global OAT coverage remains suboptimal. This study estimated the global unit cost of OAT provision to inform resource allocation and scale-up of harm reduction services. METHODS: We conducted a systematic review to identify cost estimates of OAT provision and cost drivers. Data were extracted from peer-reviewed and grey literature sources. Mi…

Protective Behavioral Strategies for Psychedelic Use: A Mini Review of the Evidence.

BACKGROUND: Approximately 8.5 million Americans over the age of 12 endorsed past year psychedelic use in 2022, with 1.4 million individuals initiating use during this time. Although emerging evidence indicates there may be beneficial aspects of psychedelic use, there is a need to understand how individuals might mitigate potential risks within nonclinical contexts, such as through the use of protective behavioral strategies (PBS). METHOD: PubMed and Google Scholar databases were searched to identify articles reporting on PBS (i.e., harm reduction strategies implemented at the individual level…
