BACKGROUND: Surgical fixation of humeral shaft fractures is widely considered a relative indication for polytraumatized patients to improve mobility and expedite care. This study aimed to determine whether operative treatment of humeral shaft fractures improves short term outcomes in polytrauma (PT] patients.
METHODS: Using the National Trauma Data Bank, PT patients with humeral shaft fractures were identified from 2010-2015. Three PT groups were analyzed: Group 1 - PT with nonoperative humeral shaft fracture, Group 2 - PT with humeral fixation on Day 1, and Group 3 - PT with humeral fixation

Distal humeral coronal shear fractures are rare and account for around 1% of all fractures at the elbow level and 6% of the distal humeral fractures. Many studies have reported on the associated injuries involving the lateral collateral ligament injuries and radial head fractures in up to 40% and 30% of humeral fractures, but this combination of coronal shear fracture of the trochlea and capitellum associated with olecranon fracture is unusual. Here, we report two cases of distal humerus coronal shear fracture with olecranon fracture treated surgically and establish and discuss the pathomechan…

Minimally invasive plate osteosynthesis (MIPO) has been used for humeral shaft fractures, but concerns exist about soft tissue injuries. The purpose of this study was to report the surgical technique and clinical outcomes of MIPO using a helical plate for metadiaphyseal complex humeral shaft fractures. Twelve patients with acute displacement involving proximal and middle third humeral shaft fractures (AO type C) were treated using the MIPO technique with a helical plate. Fracture union, complications, and functional outcomes were evaluated using the Constant-Murley score and Mayo Elbow Perform

Humeral head fractures are commonly found in elderly patients. A high rate of spontaneous rotator cuff tears in this patient group may endanger good results following surgical treatment. 70 patients > 65 years with dislocated humeral head fractures were prospectively studied and treated surgically following a standardised treatment protocol. In all cases the rotator cuff was examined intraoperatively. A tear of the rotator cuff was documented in 4 cases (5%). The prevalence of rotator cuff tears in elderly patients with dislocated humeral head fractures is significantly smaller than expected r

INTRODUCTION: In this study, we reviewed proximal humeral fractures surgically treated with open reduction and internal fixation (ORIF) to test the null hypothesis that there is no association between fracture characteristics and surgeon characteristics in outcomes after surgical management of displaced proximal humeral fractures with ORIF.
METHODS: A retrospective review of surgically treated proximal humeral fractures was done at a tertiary-level hospital. The data were organized into two categories: fracture characteristics and surgeon characteristics.
RESULTS: There were 314 proximal hum…

Humeral shaft fractures constitute only 3% of fractures in children younger than age 16 years. They are most common in children younger than 3 and older than 12 years old. They can be classified according to the fracture pattern, location, and tissues damaged. Fractures resulting from minor trauma may be caused by an occult unicameral bone cyst. Each age group requires different diagnosis, treatment, and prognosis. Fractures at birth are seen mostly with macrosomic and breech presentation. In children younger than 3 years, humeral fractures often are linked to child abuse. In those older than …

Shoulder dislocations associated with a displaced fracture of the humeral head or glenoid require different treatment than shoulder dislocations without fracture. If the humeral head is fractured, two possible complications must be considered:impairment of the subacromial gliding mechanism and insufficient blood supply to the humeral head. In glenoid fractures, instability may be induced. The degree of instability depends on the size of the fragment. In fractures of the humeral head, in particular of the greater tuberosity, we differentiate between the so-called en bloc fracture and the so-cal

BACKGROUND: This study evaluated treatment patterns for long bone fractures and factors that contribute to use of intramedullary nails (IMN).
METHODS: Patients from IBM MarketScan® Research Commercial and Medicare Databases with femoral/tibial/humeral fractures during inpatient admission between January 2016-July 2019 were identified. Patients were categorized by treatment (i.e., non-surgical/internal fixation [extramedullary internal fixation/plating]/IMN/external fixation). Four-year rates of IMN were reported by fracture type. Logistic regression evaluated factors contributing to IMN use.

INTRODUCTION: While recommendations for early exploration and nerve repair in cases of open fractures of the humeral shaft associated with radial nerve palsy are clear, the therapeutic algorithm for the management of closed humeral shaft fractures complicated by radial nerve palsy is still uncertain. The purpose of this study was to determine whether patients with complete sensory and motor radial nerve palsy following a closed fracture of the humeral shaft should be surgically explored.
PATIENTS AND METHODS: Twenty-five patients with closed humeral shaft fractures complicated by complete rad

BACKGROUND: We report a rare case in which closed reduction was successfully obtained for iatrogenically displaced fracture-dislocation of the humeral anatomical neck with a favorable clinical outcome.
CASE PRESENTATION: A 53-year old postman suffered from shoulder dislocation with an undisplaced fracture of the humeral anatomical neck which was initially undiagnosed. After the first attempt to reduce the dislocation of the shoulder joint by Stimson's method, complete displacement of the fractured humeral anatomical neck occurred. By closed reduction under general anesthesia, the displaced hu

Nonunion is one of the most common and challenging complications of distal humeral fractures. Newer internal fixation principles and techniques have improved the ability of orthopaedic surgeons to achieve stable fixation of complex distal humeral fractures. However, some fractures will fail to unite, leaving the patient with an unstable, dysfunctional, and often painful upper extremity that requires additional surgery.

Application of humeral nail in the treatment of humeral shaft fractures is a relatively novel method of fracture fixation. Its application at Clinical Department of Traumatology, Sestre milosrdnice University Hospital Center began in 2001. The operative procedure should be performed using a minimally invasive technique without fracture opening in order to maintain optimal conditions for fracture healing including hematoma management. However, in everyday practice it is often impossible to obtain satisfactory fracture reduction using the closed procedure, so open reduction and additional fixati

Open humeral shaft fractures comprise approximately 2% of all fractures of the humerus. Nearly 20% of open humeral shaft fractures will develop deep infection, increasing the risk of nonunion regardless of treatment method. Recalcitrant septic nonunion of the humeral shaft is a complex and challenging problem. Operative treatment should aim to eradicate infection, address bony defects, and establish a stable construct that affords early motion. We describe the case of a 38-year-old male with a recalcitrant humeral shaft septic nonunion following fixation of an open humeral shaft fracture. Mana

Compartment syndrome (CS) can present differently in children than in adults. Increased need for analgesics is the first sign of evolving CS in children. Children with supracondylar humeral fractures, floating elbow injuries, operatively treated forearm fractures, and tibial fractures are at high risk for CS. Elbow flexion beyond 90° in supracondylar humeral fractures and closed treatment of forearm fractures in floating elbow injuries are associated with increased risk for CS. Prompt diagnosis and treatment with fasciotomy in children result in excellent long-term outcomes.
