[Application of improved regional citrate anticoagulation in continuous hemofiltration in children].

Objective: To investigate the application of regional citrate anticoagulation with calcium hemofiltration basic solution in continuous hemof … ns of citrate anticoagulation, such as hypocalcaemia, metabolic alkalosis, citrate accumulation and hypernatremia were observed. Result: Continuous hemofiltration was applied in 18 patients for 734.5 hours, and the average useful life of filter was (25±11)h.There was no obvious clotting event. There were 168 groups of datum of the blood gas analysis, electrolyt …

Hemodiafiltration improves performance of 24 hour ex situ normothermic liver machine perfusion.

BACKGROUND & AIMS: Performing ex situ normothermic machine perfusion (NMP) for ≥24 h represents an opportunity to evaluate and treat livers, … Incorporation of HDF during NMP maintained stable pH and electrolyte levels, effectively preventing hypernatremia, hypochloremia, and hypocalcemia developing without HDF. HDF cleared metabolic wastes (e.g. urea) and inflammatory cytokines (IL-1B, IL-2, IL-6, IL-8, and IL-18), resulting in reduced injury and oxidative stress markers after 24 h (Suzuki score 0.8 …

Associations between day of admission, admission hyponatremia and hospital outcomes in medical patients: A retrospective multicenter cohort study.

INTRODUCTION: We investigated the relationship between admission day, serum sodium concentration, and outcomes in medical inpatients. Hypona … abia (January 1, 2016 - May 9, 2022). PARTICIPANTS: 43,361 adult medical admissions. Patients with hypernatremia (n = 1,892) or LOS > 30 days (n = 2,988) were excluded. INTERVENTIONS: Admissions were categorized by admission day (Sunday - Saturday) and serum sodium concentration (normonatremia and hyponatremia with severity sub-classification). MAIN OUTCOME …

Prevalence, risk factors and prognosis of hypernatraemia during hospitalisation in internal medicine.

INTRODUCTION: Hypernatraemia in hospitalised patients is less common and less studied than hyponatraemia, although it also seems to be associated with a poor prognosis. The present study evaluates its prevalence, risk factors and prognosis in an internal medicine department. METHODS: Full hospital stays over 28 months in a 36-bed internal medicine department were analysed retrospectively. Patients with at least one plasma sodium ≥ 150 mmol/l were compared first with all other patients and then individually with sex- and age-matched normonatraemic controls. RESULTS: Plasma sodium ≥ 150 mmol÷l

Preventing venous thrombosis in critically ill children: what is the right approach?

BACKGROUND: The incidence of venous thromboembolic (VTE) events in children has increased in recent years (J Neurosurg, 101, 2004, 32; J Thromb Haemost, 1, 2003, 1443) yet there is currently no consensus as to what VTE prophylaxis, if any, should be applied to the pediatric population. OBJECTIVES/AIMS: Our aim was to audit current practice in pediatric VTE prophylaxis across England and Wales and to advocate simple measures for prevention. We illustrate the importance of the condition with a series of cases from the South West Paediatric Burns and Neurosurgical Services based in Bristol. MET

Clinical data and factors associated with poor outcome in pneumococcal meningitis.

We carried out a 4-year study of 159 children (ages 1 month-14 years) with pneumococcal meningitis. The study was divided into two periods: the retrospective period (1998-2000: 107 patients), and the prospective period (2001-2002: 52 patients). About 2/3 of the children were under 2 years of age: 72 (45%) were under 1 year of age and 38 (24%) had meningitis during the second year of life. One-third of the patients had signs of otitis media; convulsions were more frequent in patients under 1 year compared with older patients (34.7 vs. 14.9%; P=0.004); 13/159 children (8.2%) died; 93/159 (58.5%)

From treating UTI to agranulocytosis.

The growing problem of antibacterial resistance has resulted in increased interest in fosfomycin, especially its parenteral formulation. It is a broad-spectrum antibiotic with both in vivo and in vitro activity against a wide range of bacteria, including MDR and XDR bacteria. Due to its high tissue penetration, fosfomycin may be used in a broad range of tissues and targets, including the CNS, soft tissue, bone, lungs, and abscess fluid. The most commonly reported adverse effects are hypokalemia and hypernatremia. We describe a case of agranulocytosis in a young patient with chronic kidney dise…

ENDOCRINOLOGY IN THE TIME OF COVID-19: Management of diabetes insipidus and hyponatraemia.

COVID-19 has changed the nature of medical consultations, emphasizing virtual patient counseling, with relevance for patients with diabetes insipidus (DI) or hyponatraemia. The main complication of desmopressin treatment in DI is dilutional hyponatraemia. Since plasma sodium monitoring is not always possible in times of COVID-19, we recommend to delay the desmopressin dose once a week until aquaresis occurs allowing excess retained water to be excreted. Patients should measure their body weight daily. Patients with DI admitted to the hospital with COVID-19 have a high risk for mortality due to

Clinical insights into adipsic diabetes insipidus: a large case series.

OBJECTIVE: Adipsic diabetes insipidus (DI) causes significant hypernatraemia. Morbidity and mortality data for patients with adipsic DI have been previously published as single case reports, rather than as formal trials or case series from units with established management protocols. Our objective was to describe morbidity and mortality in patients with adipsic DI attending a tertiary referral centre, representing the largest reported series of adipsic DI, and to suggest management protocols for such patients, based on our extensive experience of this condition. DESIGN: Arginine vasopressin (

Osmotic Demyelination Syndrome in Children.

Osmotic demyelination syndrome is an acute demyelination process that usually occurs several days following an osmotic stress. This syndrome is rare in adults (0.4% to 0.56%) and even more uncommon in children. We performed a review of all reported pediatric osmotic demyelination syndrome patients from 1960 to 2018. Among all 106 cases, 49 presented with isolated central pontine myelinolysis, 30 with isolated extrapontine myelinolysis, and 27 with combined central pontine myelinolysis and extrapontine myelinolysis. There was no gender preponderance, and the highest prevalence was noted between
