What is this chocolate milk in my circuit? A cause of acute clotting of a continuous renal replacement circuit: Questions.

INTRODUCTION: One of the greatest problems associated with continuous renal replacement therapy (CRRT) is the early clotting of filters. A literature search revealed three case reports of lipemic blood causing recurrent clotting and reduced CRRT circuit survival time in adult patients, but no reports of cases in children. DIAGNOSIS/TREATMENT: A 23-month-old male infant with Martinez-Frias syndrome and multivisceral transplant was admitted to the hospital with severe sepsis and hemolytic anemia. He developed acute kidney injury, fluid overload and electrolyte imbalances requiring CRRT and was

Laparoscopic management of congenital duodenal atresia or stenosis: A single-center early experience.

BACKGROUND: The background is to review our experience with laparoscopic repair of congenital duodenal atresia or stenosis (CDAS) and compare postoperative outcome with a group control of laparotomy repair. METHODS: Retrospective chart review of all cases of CDAS undergoing laparoscopic surgery at our institution between July 2013 and May 2014 and comparison with a group control of open operation performed between 2007 and 2010. Data were compared using Fisher's exact test for qualitative values and Mann-Whitney test for quantitative values. P values less than 0,05 were considered statistical

[Caudal-epidural injection of adjuvants and local anaesthetics in the newborns undergoing surgery].

In 45 newborns, operated on for the inborn failures of gut, the markers of stress and indices of central hemodynamics were studied up. General anaesthesia was applied together with caudal-epidural injection of local anaesthetics in 15 newborns, in 15--the adjuvants--klophelline and promedol--were added to local anaesthetics, in 15--a standard atharalgesia was used. The best and most durable effect was noted while application of anesthesia in combination with adjuvants.

Rectal and sigmoid atresia: transanal approach.

We report 2 patients with rectal and low sigmoid atresia operated on, respectively, at 6 and 3 months of age using the transanal approach, similar to the transanal technique for Hirschsprung disease, after exploratory laparotomy with colostomy at birth. There were no intraoperative or postoperative complications after a follow-up time of 2 years. After closure of the colostomy, both patients had no fecal incontinence. The transanal approach is a safe and effective technique in the management of rectal and sigmoid atresia.

Severe congenital skin defects in a newborn. Case report and relevance of several obstetrical parameters.

The clinical, laboratory, histologic and autopsy findings are reported from a live-born male infant with severe congenital skin defects (CSD) who survived for 2 days. The family history revealed consanguinity of the (Turkish) parents. The patient was compared with 10 cases from the literature with the most severe form of CSD. The combination of severe CSD, parental consanguinity and gastrointestinal atresia was found in 3 of these 11 cases, including our own patient. Differentiation from an atypical form of epidermolysis bullosa, complicated by pyloric atresia, is difficult. The mechanism of t…

The alimentary tract malformations in the rabbit thalidomide embryopathy.

Among 906 thalidomide exposed young from 6 different rabbit strains, 141 major malformations involving the alimentary tract were found. All but 4 of the defects can confidently be regarded as a drug effect. The lesions occurred in the derivatives of the foregut, hindgut and umbilicus. Foregut anomalies (68 specimens) comprised hiatus hernia, megaoesophagus, prepyloric notching of the greater curvature of the stomach and deficient septum formation between oesophagus and trachea. In many rabbits all 4 lesions were present together. Hindgut malformations (61 specimens) were made up of rectal atre

Congenital anomalies of the tubular gastrointestinal tract.

Congenital anomalies of the tubular gastrointestinal tract are an important cause of morbidity not only in infants, but also in children and adults. The gastrointestinal (GI) tract, composed of all three primitive germ layers, develops early during embryogenesis. Two major steps in its development are the formation of the gut tube (giving rise to the foregut, the midgut and the hindgut), and the formation of individual organs with specialized cell types. Formation of an intact and functioning GI tract is under strict control from various molecular pathways. Disruption of any of these crucial m

Duodenal Stenosis: A Diagnostic Challenge in a Neonate With Poor Weight Gain.

Cases of isolated duodenal stenosis in the neonatal period are minimally reported in pediatric literature. Causes of small bowel obstruction such as duodenal atresia or malrotation with midgut volvulus have been well documented and are often diagnosed due to their acute clinical presentation. Duodenal stenosis, however, causes an incomplete intestinal obstruction with a more indolent and varying clinical presentation thus making it a diagnostic challenge. We present a neonate with a unique case of congenital duodenal stenosis. The neonate presented with poor weight gain and frequent "spit-ups"

Oesophageal atresia: The growth gap.

Poor growth is an under-recognised yet significant long-term sequelae of oesophageal atresia (OA) repair. Few studies have specifically explored the reasons for growth impairment in this complex cohort. The association between poor growth with younger age and fundoplication appears to have the strongest supportive evidence, highlighting the need for early involvement of a dietitian and speech pathologist, and consideration of optimal medical reflux management prior to referring for anti-reflux surgery. However, it remains difficult to reach conclusions regarding other factors which may negativ

Abdominal Wall Defects.

The 2 most common congenital abdominal wall defects are gastroschisis and omphalocele. Both are usually diagnosed prenatally with fetal ultr … igher likelihood of abnormalities related to the gastrointestinal tract, with the most common being intestinal atresia. As such, the prognosis in patients with omphalocele is primarily affected by the severity and number of other anomalies and the prognosis for gastroschisis is correlated with the amount and function of the bowel. Because of these distinctions, thes …

Pathogenic implications of remnant vitelline structures in gastroschisis.

PURPOSE: The pathogenesis of gastroschisis is unknown. It may be helpful in understanding its pathogenesis to know the structural relationships among umbilical components including umbilical vessels, urachus, and vitelline structures, and thus, the authors investigated the remnants of vitelline structures in a series of cases of gastroschisis. METHODS: Medical records of 41 cases with gastroschisis treated in our institute from 1979 to 2009 were retrospectively reviewed. RESULTS: Paraumbilical bands, possible remnants of vitelline structures, were observed in 4 cases (9.8%). All 4 bands were

Histological study of timing and embryology of notochordal abnormalities in rat exposed in utero to Doxorubicin.

Experimental Doxorubicin-exposure in utero is correlated with foetal oesophageal atresia, tracheo-oesophageal fistula, axial alterations. While gastrointestinal and respiratory defects have been largely investigated, only sporadic data have been published to date on notochordal and vertebral defects. The aim of this work was the study of the genesis of chordal and vertebral abnormalities in rat embryos and foetuses exposed to Doxorubicin and the study of their correlation with oesophageal and tracheal defects. For this purpose, pregnant rats were i.p. injected with saline (control) or with 4 m

Short bowel syndrome in infants and children: an overview.

Short bowel syndrome is a spectrum of malnutrition resulting from inadequate bowel length. In infant and pediatric patients, the most common causes are necrotizing enterocolitis, abdominal wall defects, jejunal ileal atresia, and mid gut volvulus. There appear to be regional variations in etiology. Since the publication of Wilmore's classic monograph in 1972, there have been significant improvements in monitoring and nutritional support. In the modern era, survival rate ranges from 80% to 94%, and the presence or absence the ileal cecal valve appears to not impact on mortality rate, but does s

[Esophageal replacement using the large intestine in children].

Oesophageal replacement with an interposed graft of the large intestine on a vascular pedicle between the cervical oesophagus and oesophageal stump over the diaphragm was performed in four children for Vogt's classification Type-I and II oesophageal atresia. A tracheooesophageal fistula was closed in three, and gastrostomy was established in four neonates to provide nutrition. Oesophageal replacement was performed in patients aged 2 years and 5 months to 5 years and 4 months. One girl died following oesophageal replacement for bilateral pneumonia. A non-functioning oesophageus was extirpated f

Efficacy of hepatobiliary scintigraphy in the differential diagnosis of neonatal cholestasis and postoperative follow-up after hepatoporto-digestive anastomosis.

In the differential diagnosis of unclear neonatal and infant cholestasis the formerly applied 131-I-Rose Bengal scintigraphy has been widely abandoned in favour of hepatobiliary scintigraphy with 99mTc-labelled IDA derivatives excreted with the bile. Our results in 32 jaundiced children ranging from ages 2 weeks to 32 weeks show a high sensitivity of scintigraphy in proving an occlusion in 92% with a clearly lower specificity of 79%, if based on the demonstration of marked bile in the intestine as the only criterion. Diagnostic accuracy, however, can be further increased in additional criteria

A New Percutaneous Biliary Path With a Biodegradable Stent for Intrahepatic Biliary Stenosis After Living Donor Pediatric Liver Transplant.

Biliary tract complications are a significant source of morbidity following living donor liver transplant, especially in pediatric patients. These complications are influenced by graft type, bile duct anatomy, and reconstruction techniques. For a hepaticojejunostomy, percutaneous intervention is the first option preferred. Here, we present a case report of the successful use of a percutaneous biodegradable stent to manage an intrahepatic biliary stricture in an 8-month-old patient who had previously undergone living donor liver transplant for biliary atresia, thereby creating a new biliary pat

Development and validation of risk models for mortality and morbidity in 12 major pediatric surgical procedures: A study from the National Clinical Database-Pediatric of Japan.

PURPOSE: To establish and validate risk models of mortality and morbidity associated with 12 major pediatric surgical procedures using the National Clinical Database-Pediatric (NCD-P) data. METHODS: We used the NCD-P data for the development and validation datasets. By using multivariate logistic regression to analyze the development dataset, we created a prediction model for 30-day mortality and morbidity in 12 major pediatric surgical procedures, including tracheoplasty, pneumonectomy, fundoplication, total/subtotal excision of malignant tumor, and surgeries for Hirschsprung disease, anorec
