Complications of inflammatory bowel disease.

Complications in inflammatory bowel disease determine the severity of disease as well as the complexities of medical or surgical treatment opportunities. Therefore, in known inflammatory bowel disease, the prevention, the early detection and the adequate therapeutic response to certain complications are important goals in the follow-up of inflammatory bowel disease patients. Disease complications are separated into intestinal and extraintestinal complications. Intestinal complications are somewhat disease specific, which means that they occur exclusively in either Crohn's disease or ulcerative

[Surgical treatment of ileus in the radiation-injured intestine].

Between the years 1977 to 1990, ten patients were operated for ileus due to intestinal damage induced by radiotherapy. The patients had received an average radiation dose of 50.2 GY. Average lapse of time between radiotherapy and operation for ileus was 15.5 months. The operation procedures included were five lysis of adhesions, four by-pass operations, three small intestine resections, one large intestine resection, three transversostomies and one ileostomy. Three patients suffered postoperative complications; one got bronchopneumonia, one suffered prolonged paralytic ileus and one suffered a

[Diverticular disease of the colon (author's transl)].

Admission diagnosis of 100 patients with diverticular disease of the colon indicated that the clinical features of the disease is still relatively unknown. The most important cardinal symptoms are abdominal complaints, irregular stools and/or bleeding from the large intestine. These symptoms occur with about equql frequency in patients with diverticular disease, whether or not there was evidence of inflammatory complications. If there is clinically manifest diverticulum formation of the colon, the term "diverticular disease" seems appropriate, since transition between diverticulosis and divert

[Intraoperative placement of transnasal small intestinal feeding tube during the surgery in 5 cases with high position intestinal obstruction and postoperative feeding].

OBJECTIVE: To explore the value of employing the small intestinal feeding tube in treating high position intestinal obstruction of newborn infant. METHOD: Five newborn infants (3 males and 2 females; 1 premature infant and 4 fully-mature infants; 2 had membranous atresia of duodenum, 1 had annular pancreas, and 2 had proximal small intestine atresia; 1 infant had malrotation). The duodenal membrane-like atresia and the blind-end of small intestine were removed and intestinal anastomosis was performed, which was combined with intestinal malrotation removal. Before the intestinal anastomosis su

Magnetic resonance imaging of Crohn disease: early recognition of treatment response and relapse.

A patient with active Crohn disease was evaluated by MRI at admission, clinical remission, and a new relapse. The MRI-estimated disease extension correlated with surgical findings, whereas ultrasonography underestimated and a small bowel series overestimated the extension. MRI disclosed the disappearance of intestinal edema at the time of clinical remission and, in contrast to ultrasonography, showed an abscess and a fistula, confirmed by surgery, at the new relapse.

The vesicosigmoidal fistula: diagnosis and surgical treatment.

The authors report their experience in the diagnosis and treatment of 10 cases of vesicosigmoidal fistula. In accordance with the literature the most frequent presenting symptoms were of urinary origin. For the diagnosis urography was helpful, cystography and cystoscopy allowed the observation of a fistulous orifice, while barium enema confirmed, or removed probable doubts about the nature of the intestinal pathology. Surgical treatment varied from one-stage to multi-stage procedures.

Congenital intrathoracic bronchopulmonary foregut malformations in childhood.

Bronchopulmonary foregut malformations (BPFM) are a group of related disorders. They include tracheoesophageal fistula with or without atresia, bronchopulmonary sequestration, intestinal duplication cyst, bronchogenic cyst, aberrant systemic arterial supply to the lung, and bronchial mucosal rests in the esophagus. This paper describes 12 cases which illustrate the spectrum of clinical and radiographic features of these anomalies. The embryogenesis of these malformations is reviewed and appropriate radiographic investigation is discussed.

Ostomy complicatıons in patients with anorectal malformations.

PURPOSE: Ostomy is an important step in the treatment of patients with anorectal malformation (ARM). Sometimes this basic surgical procedure may cause a devastating complication. METHODS: The medical reports of the patients with ARM who had ostomy in the past were inspected. How many of them were operated in this center or sent from others, the type of ARM, what type of ostomy performed and which place of the bowel used and the complication type and rate related to ostomy and ostomy closure were evaluated. RESULTS: Nearly, 157 of 230 patients with ARM had ostomy during twenty-two years were

Acute gastrointestinal complications of infrarenal aortic aneurysm repair.

Acute gastrointestinal complications developed in 31 of 472 patients following aortic aneurysmectomy (6.6 per cent). In order of frequency these were: ischaemic intestine in nine patients, mechanical or paralytic ileus in eight patients, peptic ulceration in seven patients, undiagnosed gastrointestinal bleeding in five patients and paraprosthetic fistula in two patients. The risk of developing peptic ulcer complications was not significantly increased in patients with a previous history of peptic ulcer disease. The risk of developing an ischaemic intestine was increased if the distal limb of a

Congenital bronchobiliary fistula in a newborn: a case report.

BACKGROUND: Congenital bronchobiliary fistula (CBBF) is a developmental structural abnormality between the trachea or bronchus and the biliary tree, with non-specific clinical manifestations that mainly present as repeated cough and dyspnea, but it is often misdiagnosed as pneumonia. If not promptly diagnosed and effectively treated, CBBF can be fatal. Currently, radical surgery is the only treatment option for CBBF. CASE PRESENTATION: An 8-day-old full-term Han Chinese female infant presented with coughing accompanied by sputum and shortness of breath with worsening dyspnea for 4 days. Yello

Prosthetic Hip Joint Infection of Digestive Origin, Diagnosis of a Rare but Serious Condition: A Case Report.

INTRODUCTION: Prosthetic joint infection (PJI) is a relatively infrequent occurrence; however, it always poses a significant risk to the patient's functional outcome. The origin of PJI is often a topic of debate. In this case, we present a PJI that resulted from a digestive fistula passing through an iliopsoas abscess, which can be regarded as an unusual consequence of rare conditions. CASE REPORT: A 72-year-old man was brought to the emergency department with pain in his right hip and a functional impairment of his right lower limb. This had been ongoing for 3 weeks and he had no history of

[Diagnosis and treatment of billiary ileus].

INTRODUCTION: Biliary ileus represents only 14% of mechanical obstructions of the gastrointestinal tract. However, the rate of non-strangulated small bowel obstructions reaches as much as 25% in patients over 65 years of age. Usually, a pressure necrosis is created by a large gallstone that passes through the developed biliodigestive fistula, subsequently obturating the gastrointestinal tract. CASE REPORTS: 35 patients underwent a surgical procedure at the Department of Surgery, Teaching Hospital Pilsen for biliary ileus from January 1, 2000 to January 31, 2015. Mean age was 79 years (median

Surgical management of small-bowel radiation enteritis.

We treated 17 patients with severe small-bowel radiation enteritis surgically. Fourteen patients were female. Gynecologic malignant lesions (cervical, ovarian, and endometrial) were the most frequent sites of the primary tumors for which radiation was given. Thirteen patients had bowel obstruction, and the remainder had enterovaginal fistulae. Intestinal bypass, rather than resection, was the preferred approach in the treatment of these patients, and was used in 11 cases. Successful palliation was provided in nine patients, with minimal morbidity and no operative deaths. Follow-up at eight to

A Rare Case of Gastrojejunocutaneous Fistula after Percutaneous Endoscopic Gastrostomy.

We report a rare case of fistulation to the jejunum after percutaneous endoscopic gastrostomy (PEG). An 85-year-old man with previous cerebral infarction and swallowing disturbance underwent PEG. Nine months later, he developed a high fever and discharge around the gastrostomy button. He was diagnosed with aspiration pneumonia, and administered antibiotic therapy. Examination showed digestive fluid around the gastrostomy button. Gastrointestinal contrast-enhanced CT revealed a gastrojejunocutaneous fistula. The button was removed, and the fistula closed naturally. PEG was performed again. The

[Clinical study of enterovesical fistulas].

We conducted a retrospective review of 16 patients who were diagnosed with enterovesical fistula in our hospital between January 2000 and July 2013. The patient's median age was 74 years old and 4 were female. Most of the chief complaints were pneumaturia and fecaluria. There was a vesicosigmoidal fistula in 12 patients, an ileovesical fistula in 2, and a rectovesical fistula in 2. The main underlying cause was diverticulitis in 9 patients and a sigmoid colon carcinoma in 3. Diagnoses were made based on the findings of cystoscopy, barium enema, abdominal computed tomography and so on. Treatmen

[A case of cecovesical and rectovesical fistulas secondary to mucinous adenocarcinoma of the cecum].

A hundred and nine cases of cecovesical fistula are described in the literature. Although its origin is usually reported to be appendicitis, we present a rare case of cecovesical and rectovesical fistulas secondary to mucinous adenocarcinoma of the cecum. An 84-year-old man was referred to our hospital with the chief complaints of fever and cloudy urine. An enterovesical fistula was seen on cystography, and cystoscopy showed a lot of mucinous fluid in the bladder and the papillary tumor arising from the enteric mucosa was identified through the fistula. Transurethral biopsy of the tumor reveal

[Cutaneo-reno-colonic fistula in tubercular kidney with staghorn calculus: report of a case].

We report a rare case of colorenocutaneous fistula, in a patient with a stone of the left kidney. The fistula was successfully managed by single stage surgery (nephrectomy, fistulectomy and suture of colon). Histopathological examination revealed renal tuberculosis. Surgery was completed by an anti-tubercular treatment.
