Magnesium: clinical considerations.

Magnesium is gaining recognition as a clinically important electrolyte. Hypomagnesemia has been associated with a variety of disorders including seizures, malignant ventricular dysrhythmias, and sudden death. The emergency department patients who are most likely to be magnesium deficient include alcoholics, patients who take diuretics, and those in diabetic ketoacidosis. Hypokalemia and hypocalcemia may represent unrecognized hypomagnesemia. Clinical trials and case reports also document increasing interest in magnesium as an effective therapeutic agent for potentially life-threatening problem

Magnesium depletion and pathogenesis of Alzheimer's disease.

Mg depletion, particularly in the hippocampus, appears to represent an important pathogenic factor in Alzheimer's disease. It is associated with high aluminium incorporation into brain neurones. This type of Mg deficit cannot respond to mere Mg supplementation, but requires correction of the dysregulation inducing this Mg depletion. Further research should seek to control the alterations of albumin, which may induce this brain Mg depletion.

Factors influencing magnesium absorption and metabolism in ruminants.

Magnesium is a nutrient required for all animals, but it is especially critical for ruminants. A physiological deficiency of Mg results in hypomagnesemic tetany. Typically, only female ruminants are affected, and the disturbance usually occurs during the early stages of lactation. Magnesium functions at three biochemical levels, as a cofactor at the enzymatic level, at the structural level in assembly of ribosomes, and at the whole cell level as a stabilizing force in membranes. Parathyroid hormone (PTH) can affect Mg metabolism by decreasing urinary Mg excretion and stimulating bone resorptio

Intra-/extracellular shifts of potassium after the administration of Mg in patients with cardiovascular diseases.

Body potassium status is often dosed on serum potassium determinations. However, this parameter is not an adequate guide to the body potassium status, unless several factors are taken into consideration, e.g. acid-base balance and serum creatinine level. Muscle magnesium content is another factor, probably operative through its activation of Na-K-ATPase, which produces the energy necessary for the active transport of potassium into the cell. Magnesium has a membrane-stabilizing effect as well, diminishing the outward movement of potassium from the cell. In case of magnesium deficiency, potassi…

Downbeat nystagmus. Long-term therapy with moderate-dose lithium carbonate.

Downbeat nystagmus developed in a 67-year-old hypomagnesemic woman while she was receiving lithium carbonate for depression. This nystagmus abated each time lithium carbonate therapy was withdrawn, and no alternative causes of nystagmus were demonstrated. However, this nystagmus occurred despite serum lithium carbonate levels in the nontoxic range. Total-body magnesium deficiency may have enhanced the toxic effect of lithium carbonate on cerebellomedullary connections.
