Long-term health-related quality of life (HRQOL) and functional outcomes following mandibular and maxillary reconstruction are lacking. To determine these outcomes, a cross-sectional study of patients with a history of cancer who underwent jaw reconstruction was undertaken. Participants were identified from a database of jaw reconstruction procedures at the Chris O'Brien Lifehouse (Sydney, Australia). Eligible patients had at least one month follow-up, were aged ≥18 years at surgery, and had history of malignancy. HRQOL was measured using the FACE-Q Head and Neck Cancer Module (FACE-Q H&N). Fu

BACKGROUND: Reconstruction in a vessel-depleted neck is challenging. The success rates can be markedly decreased because of unavailability of suitable recipient vessels. In order to obtain a reliable flow, recipient vessels away from the zone of fibrosis, radiation, or infection need to be explored. The aim of this report is to present our experience and clinical outcomes using the retrograde flow coming from the distal transverse cervical artery (TCA) as a source for arterial inflow for complex head and neck reconstruction in patients with a vessel-depleted neck.
METHODS: Between July 2010 a

The use of free vascularized bone grafts for mandibular reconstruction has led to a consideration of the new entity of recurrent tumour on bone grafts. Exeresis of the mandible was performed in 4 patients with bone tumours: osteosarcoma 1, osteochondroma 1, carcinoma 1 and ameloblastoma 1. A free vascularized bone graft was used for the reconstruction. Tumours recurred with a delay of 10 to 18 months after the initial reconstruction. The frequency of tumoural recurrence can be measured in our series of 46 patients (24 with fibular grafts, 15 with scapular grafts, 4 with radius grafts and 3 wit…

BACKGROUND: Knowledge about the anatomy and morphology of the root canal system is essential for successful surgical and non-surgical root canal treatments. However, precise assessment of the root morphology and anatomy is not often possible on two-dimensional radiographs. This study aimed to investigate the association of root morphology of mandibular second molars on panoramic-like and axial views of cone-beam computed tomography (CBCT).
METHODS: This cross-sectional study evaluated 1,231 CBCT scans of mandibular second molars obtained between October 2018 and February 2022 that were retrie

A 49-year-old female presented with a T4 N2 Mo moderately differentiated squamous cell carcinoma of the right cheek mucosa and involving the mandible. Patient treatment included resection of the tumour with partial mandibulectomy, maxillectomy and a free osteocutaneous fibular flap to reconstruct the bony mandibular and soft tissue intraoral defects. Six weeks postoperatively the patient developed a stress fracture of the right tibia, a complication which has not been reported in the literature previously. Pre- and postoperative measures should be considered to avoid such a complication.

The recurrence of a solitary bone cyst of the mandibular condyle in a costochondral bone graft is reported. A solitary bone cyst of the right condylar head and neck of a 10-year-old boy was treated by total resection and immediate reconstruction with a costochondral bone graft. Two years after the first operation, a recurrence of the solitary bone cyst within the bone graft was noted. An open treatment was performed. A review of the literature on solitary bone cysts and recurrences of solitary bone cysts shows that the case reported is unique. Possible reasons for the recurrence are discussed.

This syndrome is made up of a constellation of congenitally malformed facial structures which arises from the first and second branchial arches. A case of hemifacial microsomia, with the physical characteristics of unilateral left underdevelopment of the external and middle ear, mandible and zygoma is presented. The treatment took place in two stages: Firstly the auricle was reconstructed with the method of tissue expander and silicone frame and secondly the mandibular prognathism and laterognathism with sagittal splitting was corrected. We present our case and the results that have been gaine

Objective: This study presents an innovative articular fossa prosthesis generated by the envelope surface of condyle movement, and compares its mandible movements, muscle activities, and joint reaction forces with two temporomandibular joint (TMJ) prostheses using multibody musculoskeletal simulation. Methods: A healthy 23-year-old female was recruited for this study. Cone-beam computed tomographic (CBCT) was performed to reconstruct the mandibular bone geometry. A customized TMJ fossa prosthesis was designed based on the subject-specific envelope surface of condyle movement (ESCM). Mandibular

Treatment of gunshot injuries to the maxillofacial region requires a multidisciplinary approach with specialist collaboration. The primary goals of treatment are to stabilize fractures, control bleeding, reconstruct the affected tissues, and restore function. This case report discusses a patient's treatment sequence resulting from a gunshot injury to the right posterior mandible. The initial stabilization and debridement, followed by mandibular augmentation, and finally prosthodontic restoration of lost teeth and the alveolar process are illustrated. Understanding the consequences of gunshot i

We performed sliding transport distraction osteogenesis (STDO) of an alveolar segment containing an unerupted third molar in the mandible of a 22-year-old man with a benign cementoblastoma. Marginal mandibulectomy including the tumour and the right mandibular second premolar and first and second molars was done. STDO was performed to horizontally reconstruct the alveolar ridge and to restore occlusion with the use of the third molar. After forward horizontal distraction of the alveolar segment, the third molar spontaneously erupted and was gradually moved to the position previously occupied by

A variation of the pectoralis major myocutaneous flap in head and neck reconstruction is presented. The defect to be covered was a large, intraoral, U-shaped defect extending from the left tonsillar region around the alveolar ridge and floor of mouth to the right soft palate and buccal mucosa. Nearly the entire tongue was intact, and a 4 cm left mandibular segmental defect existed. A single left pectoralis major myocutaneous island flap was split longitudinally to resurface the defect. Implications of vertical splitting of a myocutaneous flap are discussed with respect to blood supply and limi

BACKGROUND: Reconstruction of the tongue and floor of mouth after total/subtotal glossectomy poses a major challenge for reconstructive surgeons. Speech and deglutition after total glossectomy are usually significantly impaired, affecting the quality of life of these patients. Maintaining an adequate volume of the reconstruction is paramount to optimize speech and swallowing function postoperatively.
AIM: To report a novel free flap reconstructive technique based on the subscapular system.
METHODS: A preliminary cases series of patients undergoing reconstruction with scapular tip-thoracodors

UNLABELLED: Primary malignant tumors of the sternum are rare among bone tumors. Even with radical resection, the survival rate for sternal tumors remains low. Resection often results in significant bone defects in the chest wall, and reconstruction must provide adequate protection for pulmonary and respiratory structures. Flexible materials have historically been used for sternal reconstructions following failed sternotomies in cardiac surgery. Although these have had some success, they fail to provide adequate support for patients undergoing reconstruction secondary to tumor resection, who ar

In the last two decades, the anterolateral thigh flap has emerged as one of the most popular reconstructive options for multiple body sites. Based on a perforator flap harvest concept, the flap encompasses the advantages of versatility, pliability, and potential for composite tissue replacement. Although numerous anatomical variations exist, these are well-described, and flap safety remains uncompromised if certain anatomical boundaries are respected. Careful preoperative planning and identification of perforators remain the cornerstone of successful flap harvest. Once perforators are identifi

Temporomandibular joint ankylosis is a debilitating condition characterized by abnormal bony fusion within the joint, resulting in restricted jaw movement, facial asymmetry, and functional impairment. It commonly arises from trauma but may also result from infection, inflammation, or systemic factors. Diagnosis relies on detailed imaging, via computed tomography scans, to guide surgical planning. Management requires careful airway considerations and a structured surgical approach, with osteoarthrectomy forming the cornerstone of treatment. Reconstruction aims to restore mandibular height and f

We report the long-term functional and aesthetic outcomes of a male patient with bilateral severe Tessier no. 7 clefts associated with mandibular hypoplasia, cleft palate, right soft-palate hypoplasia, and congenital bilateral facial nerve palsy. The patient required enteral nutrition from birth because oral feeding was impossible. Through staged reconstruction, including oral floor muscle release, bilateral commissuroplasty, palatoplasty, pharyngeal flap surgery, tracheostomy, and later tracheostomy closure, he ultimately achieved full oral intake, phonation, and social adaptation in adulthoo

Temporomandibular joint ankyloses are a fusion of the mandibular condyle to the base of skull. Surgical advances have stemmed from innovation in computer planning, guidance, and intraoperative navigation, allowing surgeons to restore form and function with greater precision, predictability, and safety. Preoperative computer virtual surgical planning used the computed tomography scan data to render a 3-dimensional image that can be used for surgical simulations and fabrication of intraoperative aids. Temporomandibular joint reconstruction should be considered as a predictable option in the mana

The use of erbium, chromium: yttrium-scandium-gallium-garnet (Er,Cr:YSGG) lasers and CAD/CAM technology in dentistry is becoming more accepted and prevalent. Using these two technologies together, full-mouth dental reconstruction was attempted to evaluate their effectiveness. The authors prepared all remaining maxillary and mandibular teeth without the use of a dental drill or an anesthetic; fabrication and insertion of the 28 all-ceramic CAD/CAM restorations were completed in two appointments. When used appropriately, practitioners can combine Er,Cr:YSGG losers and CAD/CAM technology to deliv

The fibular free flap (FFF) approach is the preferred option for reconstruction of segmental mandibular deficiencies. Fixed dental prostheses are recommended for patients who have undergone FFF jaw surgery to improve quality of life, but treatment of these patients with an implant-supported prosthesis is challenging because of the thin bone and thick soft tissue in the fibula. This case history reports the use of a bone-level implant guide fabricated with a computer-aided system and milled customized long abutments for implant placement and prosthodontic procedures.
