BACKGROUND: Anesthetic induction and maintenance with propofol are associated with decreased blood pressure that is, in part, due to decreased peripheral resistance. Several possible mechanisms whereby propofol could reduce peripheral resistance include a direct action of propofol on vascular smooth muscle, an inhibition of sympathetic activity to the vasculature, or both. This study examined these two possibilities in humans by measuring the forearm vascular responses to infusions of propofol into the brachial artery (study 1) and by determining the forearm arterial and venous responses to sy

BACKGROUND: Neuromuscular choristomas (NMC) are rare congenital lesions with differentiated muscle found within peripheral nerves. Patients often present with progressive neuropathy, undergrowth of the affected limb, and limb length discrepancy. In the sciatic nerve (the most common location of NMC), this may lead to specific manifestations in the distal limb, including progressive neuropathy, a shortened atrophic limb and a cavus foot. We hypothesized that the presence of NMC of the sciatic nerve (proximal to the sciatic notch) extending to the lumbosacral plexus could lead to abnormalities i

The developmental course of musculoskeletal deformities in neonatal brachial plexus palsy (NBPP) has not been studied extensively. The goals of this study were to: (1) evaluate a new animal model of NBPP, (2) characterize the development of musculoskeletal abnormalities in paralyzed shoulders, and (3) investigate the expression of myogenic and adipogenic genes in paralyzed rotator cuff muscles. Neonatal mice were divided into neurotomy and sham groups. The neurotomy group underwent surgical transection of the superior trunk of the brachial plexus within 24 h of birth. The sham group underwent

Contralateral transfer of the right, eighth ventral nerve branch (C8) (C8 cross-transfer - C8CT) was performed in 6 adult cats, in which the caudal part of the left brachial plexus (C8 and T1) had been severed, in order to mimic nerve root avulsion. Clinical and electrophysiological parameters, muscle contraction force measurements and histology were used to evaluate the effects of the surgery in a 14- to 36-month follow-up. The right forelimb (donor side) was clinically normal (no lameness) in all the cats at the end of the study. Electromyography performed 14 days after surgery revealed dene

A patient of ALS-like disorder in an HIV-1 clade-C-infected heterosexual male is being reported. A 37-year-old gentleman presented with subacute, progressive asymmetrical onset of weakness and wasting of upper limbs associated with brisk muscle stretch reflexes and without any sensory or sphincter involvement. While nerve conduction tests were normal, the EMG of proximal and distal limb muscles on both sides revealed evidence of denervation and reinnervation. Routine blood and urine tests and investigations for underlying causes of motor neuron disease were noncontributory. He was HIV-1, subty

The neuromuscular aspects of West Nile virus (WNV) infection have not been characterized in detail. We have studied a group of six patients with proven WNV infection. All cases presented with acute, severe, asymmetric, or monolimb weakness, with minimal or no sensory disturbance after a mild flu-like prodrome. Four cases also had facial weakness. Three of our cases had no encephalitic signs or symptoms despite cerebrospinal fluid pleocytosis. Electrophysiological studies showed severe denervation in paralyzed limb muscles, suggesting either motor neuron or multiple ventral nerve root damage. T

Sarcopenia, or 'frailty of the elderly', is a condition of low muscle mass which is prevalent in older adults and post-menopausal women, leading to falls or worsening after falls. Age-related muscle thinning contributes to falls and fractures. Fractures lead to downward spiral toward the loss of independence in the elderly and ultimately, increased health care expenditures of approximately 4 billion pounds a year. The diagnosis of sarcopenia is based on muscle mass, muscle strength, and physical performance. Sarcopenia is caused by imbalance between the anabolic and catabolic processes on the

INTRODUCTION AND HYPOTHESIS: The main risk factor for pelvic floor disorders is vaginal delivery, which may cause levator ani muscle (LAM) injury and denervation. LAM includes pubovisceral muscle (PVM, pubococcygeus), puborectalis muscle (PRM), and iliococcygeus muscle. We hypothesize that primiparous women with low pelvic floor muscle contraction have a reduced PVM cross-sectional area (CSA) compared to nulliparous women.
METHODS (SAMPLE SIZE AND STATISTICAL APPROACHES): This single-centre prospective observational study compared healthy nulliparous (n = 40) to primiparous (n = 40) women aft

BACKGROUND: Gastrointestinal dysfunction is one of the most common non-motor symptoms in Parkinson's disease (PD). The exact mechanisms behind these symptoms are not clearly understood. Studies in the well-established 6-hydroxydopamine (6-OHDA) lesioned rats of PD have shown altered contractility in isolated circular and longitudinal smooth muscle strips of distal colon. Contractile changes in proximal colon and distal ileum are nevertheless poorly studied. Moreover, segments may serve as better tissue preparations to understand the interplay between circular and longitudinal smooth muscle. Th

To search for the sequential compromise of the spinal cord, nerves, and skeletal muscle in mice chronically infected with Trypanosoma cruzi, animals were subjected to electromyographic investigation, end-plate recordings, and histological studies at 7, 15, 37, 60, 90, 120, 180, 270, and 360 days postinfection. Electromyographic studies showed signs of motor unit remodeling as early as 15 days postinfection, when diminished duration and amplitude of motor unit potentials pointing to a primary muscle involvement were found. Thereafter, certain features of denervation, reinnervation, and primary

Symptoms are related to attrition of the oversprouting motor neurons which after a period of time cannot support all their axonal sprouts, resulting in failure of re-reinnervation. In some …

Both Hg2+ and CH3Hg+ exert a depressive action on the twitch amplitude elicited by nerve stimulations more rapidly than that by muscle stimulation. CH3Hg+ was relatively more potent than Hg2+ in the nerve depressive action but was weaker on the muscle. Hg2+ induced a rapid phase and a slow phase of contracture in the diaphragm, while CH3Hg+ induced only a slow one. The biphasic contracture induced by Hg2+ could be inhibited by repetitive washout of the diaphragm with low Ca2+ (10(-3) mM) Krebs and also by treatment with glycerol to close T-tubule, while the slow contracture induced by CH3Hg+ w

Electromyographic and histopathological studies were performed on 112 skeletal muscles in 101 subjects with myopathy. The diagnostic significance of large action potentials (LAPs) in myopathy was studied. LAPs were defined as those action potentials with a duration of over 13 ms and an amplitude of over 3 mV (peak to peak). The following results were obtained: Most muscles with LAPs showed the grouped atrophy of small fibers of neuropathic change in addition to myopathic findings. Even in myopathy most LAPs reflected neuropathic change, except in thyrotoxic myopathy. LAPs were not related to a

BACKGROUND: Corneal protection is a priority in flaccid facial palsy patients. Denervation of the orbicularis oculi muscle results in weak palpebral closure and predisposes patients to severe corneal sequelae. While periorbital static procedures enhance corneal coverage in repose, voluntary closure is only regained through dynamic reinnervation of the muscle. This study aims to elucidate the added effect of dynamic reinnervation of the orbicularis oculi muscle on long-term corneal integrity as well as on dynamic closure of the palpebral aperture.
METHODS: Retrospective review was performed on

We report two males with progressive post-poliomyelitis muscular atrophy (PPMA) who developed slowly progressive weakness and atrophy in the previously unaffected muscles 50 years after acute poliomyelitis. Case 1 is a 61-year-old male who had suffered from poliomyelitis at the age of 3 years. After recovery from acute symptoms, paralysis remained in the right face, left upper extremity and right lower extremity. Paralysis had remained unchanged until the age of 55 years, when he felt increased fatigue and progressive muscle weakness of all extremities. Neurological examination revealed muscle

A 41-yr-old woman with hoarseness, multiple joint pain, and generalized myalgia was diagnosed with systemic lupus erythematosus (SLE) 7 mos before visiting our clinic. SLE-related vocal cord palsy of the left side was identified after otolaryngologic evaluation. We performed laryngeal electromyography (LEMG) on both cricothyroid and thyroarytenoid muscles. The findings indicated left recurrent laryngeal neuropathy with ongoing processes of denervation and reinnervation. Laryngeal involvement is a rare complication of SLE, but it is of clinical significance because serious consequence such as u

We report on the expression of growth associated protein (GAP)43 and neural cell adhesion molecule (NCAM) in congenital fibre type disproportion (CFTD) with myopathological additional signs of interstitial myositis. We assume that sarcolemmal GAP43 in developmental disordered myocytes plays a role in maintenance of growth morphology. In muscular dystrophy light microscopical evaluation reveals no GAP43 immunoreactivity in regenerating fibres. The expression of GAP43 seems to be a characteristic feature of CFTD. The expression of NCAM, particularly in the sarcolemma of small muscle fibres of CF

Hemiglossal paralysis due to lesions of the peripheral hypoglossal nerve leads to marked muscle atrophy with disturbed functioning in a minority of cases. Reinnervation from the unparalyzed, contralateral side may then be desired. In animal experiments on cats, a Z-plasty of the midportion of the tongue was carried out by transposing a portion of the normal tongue musculature into the opposite side following denervation 1 month previously by resection of the hypoglossal nerve. Electromyography was performed 13 months later. The tongue was then examined histochemically in serial sections in sea

Vocal cord positioning produced by selective laryngeal denervation remains a controversial issue in clinical laryngology. Previous studies fail to arrive at uniform conclusions for two important reasons: 1) failure to mark a reference sagittal plane from which to assess the degree of vocal cord lateralization, and 2) failure to recognize the influence of tracheostomy and respiratory positioning of the vocal cords. The present study makes use of photographic and electromyographic documentation in the assessment of the paralyzed cord. Physiologic inactivation of the cricothyroid muscle by trache
