Anatomic variation of the spinal accessory innervation pattern of the sternocleidomastoid muscle: A cadaveric study.

Several studies have described the anatomical course of the spinal accessory nerve (SAN) and its associated landmarks. In particular, the SA … more commonly branches without penetration (54%) in a clinical study of Indian patients undergoing neck dissection. We performed a cadaveric study on 31 subjects using an anterior triangle approach to visualize the anatomy of the SAN as it courses towards the SCM. All the subjects were White and included 35% male (N=11) and 65% female (N=20). Our study shows tha …

Contralateral migration of Kirschner wire from right acromioclavicular joint to left side of neck: a case report.

BACKGROUND: Kirschner wire migration is one of the most common complications after internal fixation of fracture or dislocation in the shoul … ansversal, descendant, and then ascendant directions. The Kirschner wire was removed by exploratory neck dissection under C-arm fluoroscopy assistance without complication. CONCLUSION: Wire migration may occur after surgical treatment with or without clinical complaint. Several hypotheses for the mechanism of wire migration have been postulated, including muscul …

Is open surgery for head and neck cancers truly declining?

In the past two decades, major modifications in the way we treat head and neck cancers, due to advances in technology and medical oncology, … an increase in the incidence of oral cavity, thyroid and skin cancer, an increase in the number of neck dissections performed, and an increase in salvage surgery and free flap reconstruction. The use of minimally invasive, technology-based surgery-with the use of lasers, operating microscopes, endoscopes, robots and image guidance-has increased. Technology, epid …

Updated definition of level VI lymph node classification in the neck.

CONCLUSION: This update will enable us to precisely address the involvement pattern of level VI and to standardize treatment procedures in order to refine their indications and eventually improve their results and avoid treatment morbidity. BACKGROUND: The neck level classification is being used worldwide to describe the lymph nodes status of the neck. It provides standardized data to properly evaluate and then improve our protocols for the management of neck metastasis in an evidence-based medical manner. Although level VI treatment is challenging in cancer of the larynx, pharynx, trachea, e

Spontaneous Vertebral Artery Dissection with Anti-cardiolipin Antibody Positivity.

A 29-year-old woman with no medical history visited our hospital with a sudden onset of headache. Magnetic resonance imaging and angiography of the head and neck demonstrated an occlusion and intramural hematoma in the right vertebral artery. We diagnosed vertebral artery dissection and provided treatment to reduce her headache and control her blood pressure. The patient's symptoms and imaging findings gradually improved. Blood biochemistry was positive for anticardiolipin antibodies. Anticardiolipin antibodies are often associated with thromboembolism but rarely associated with the developmen

Preauricular anteroparotid transmasseteric approach of the mandibular condyle: The natural evolution after cervical dissections? An anatomical feasibility study.

The purpose of this study was to evaluate the reliability and reproducibility of a novel approach for base and neck fractures of the mandible. An anatomical study was conducted on 22 given to Science subjects. A rhytidectomy-like approach was performed, followed by a transmasseteric anteroparotid dissection. Measures were taken of the incision length and bone exposure at every step. The identification of a ramus of the facial nerve during dissection was noted. The subjects were classified according to their facial adiposity. A correlation coefficient was measured between incision length and bo

Internal carotid artery dissection after inferior alveolar nerve block for third molar dental care presented as hypoglossal nerve palsy.

Cervicocephalic artery dissections occurring during dental care have exceptionally been reported. We describe a case of internal carotid artery dissection, presenting as hypoglossal nerve palsy, occurring immediately after difficult inferior alveolar nerve anesthetic block for third inferior molar dental care. Carotid dissection was successfully treated with anticoagulation therapy while hypoglossal nerve palsy recovered with carotid dissection shrinkage. The etiopathogenetic mechanisms of this singular form of internal carotid artery dissection are discussed. The possibility of internal carot

Occlusion of the vertebral artery secondary to dissection of the subclavian artery--case report.

A 34-year-old man presented with occlusion of the left vertebral artery (VA) secondary to dissection of the left subclavian artery manifesting as vertigo, nausea, vomiting, and neck pain. On admission, he was alert with left limb and truncal ataxia. Magnetic resonance (MR) imaging and MR angiography showed left cerebellar infarction and occlusion of the left VA. Conventional angiography and three-dimensional computed tomography (3D-CT) angiography showed stenosis with thrombosed pseudo-lumen of the left subclavian artery, and occlusion of the left VA. Presumably the idiopathic dissection of th

Successful treatment of severe facial lymphedema by lymphovenous anastomosis.

BACKGROUND: Facial edema is a common complication after neck dissection and/or chemoradiotherapy for head and neck cancer. Edema subsides spontaneously in most cases but sometimes persists, in which case surgical intervention is required. We report a case of severe facial edema that showed significant improvement upon lymphovenous anastomosis (LVA). METHODS: A 66-year-old man with oral floor cancer developed progressive facial lymphedema after tumor resection, bilateral neck dissections, chemoradiotherapy, and fibular and rectus abdominis musculocutaneous flap transfer. Initial anatomic investigations of the I-gel airway: a novel supraglottic airway without inflatable cuff.

The I-gel airway is a novel supraglottic airway that uses an anatomically designed mask made of a gel-like thermoplastic elastomer. We studied the positioning and mechanics of this new device in 65 non-embalmed cadavers with 73 endoscopies (eight had repeat insertion), 16 neck dissections, and six neck radiographs. A full view of the glottis (percentage of glottic opening score 100%) occurred in 44/73 insertions, whereas only 3/73 insertions had epiglottis-only views. Including the eight repeat insertions with a different size, a glottic opening score of > 50% was obtained in all 65 cadavers. …

Craniocervical arterial dissection in children: clinical and radiographic presentation and outcome.

Craniocervical arterial dissection is a recognized cause of arterial ischemic stroke in children. Whether children with craniocervical arterial dissection have dissection characteristics different from those of adults is unclear. A retrospective review of children, 1 month to 18 years of age, with dissection from two Canadian pediatric ischemic stroke registry centers was conducted. From 213 patients with arterial ischemic stroke, 16 (7.5%) were identified with dissection, 37.5% had warning symptoms, and 50% had a history of head or neck trauma. The clinical presentation included headache (44%

[Spontaneous dissection and stenosis of the vertebral artery].

The case of a 21-year-old female patient with neck pain and brain stem symptomatology after appendectomy is reported. Duplex sonographic findings were compatible with right vertebral artery dissection and occlusion of the vessel. Angiography confirmed the diagnosis. The patient recovered within weeks and was free of symptoms 6 months after the acute episode. Dissection of the vertebral artery can occur with or without minor trauma. Associations with fibromuscular dysplasia, arterial hypertension and the use of oral contraceptives have been reported. As in our own patient, dissection can appear

Bilateral clavicular attachment of omohyoid muscle.

Omohyoid muscle present in cervical region is of particular importance to anatomists as it defines anteriorly the carotid triangle and divid … ace was measured 3.3cm in length on left side and 3.6cm on right side. The omohyoid is important in neck dissections because it is considered as an ideal landmark for level III and IV lymph node metastases. Knowledge of variations of this muscle is very important for surgeries in neck region because of its close relation to the internal jugular vein and brachial …

Preservation of the facial artery in excision of the submandibular salivary gland.

BACKGROUND: The accepted method for submandibular gland excision traditionally includes ligation of the facial artery (FA) as suggested by a … tumor and in two cases of chronic sialadenitis. One patient had a postoperative hematoma following neck dissection requiring re-exploration. The source of bleeding was not found to be related to the FA. Contrary to accepted methodology, the FA may readily be preserved in surgery of the submandibular salivary glands. We suggest preservation of the FA in all cases …

Entry closure using the Najuta stent graft without supra-aortic vessel revascularization of a chronic type B aortic dissection with an aberrant right subclavian artery.

We report a case of entry closure using the Najuta fenestrated stent graft without supra-aortic vessel revascularization in a patient with chronic type B aortic dissection and an aberrant right subclavian artery. The Najuta fenestrated stent graft was inserted and deployed at zone 0. Complete preservation of the neck vessels and entry closure were achieved. This approach is a less invasive yet effective option for treating chronic type B aortic dissection with an aberrant subclavian artery.

Spontaneous carotid artery dissection.

Symptomatic spontaneous internal carotid artery dissection is responsible for up to 20% of strokes in patients under age 45 years. A carotid artery dissection occurs when arterial walls separate and create a false lumen. Risk factors include neck trauma, recent infection, family history, smoking, hypertension, oral contraceptives, migraine headaches, and connective tissue disease. Dissections can produce local pain, facial paralysis, and pupillary defects. Treatment includes anticoagulation and, in the event of pharmaceutical failure or recurrence, surgery.

[Reconstruction of the left common carotid artery for dissective occlusion detected by intra-operative ultrasonography].

We describe a case of Stanford type A acute aortic dissection. Replacement of the ascending aorta and aortic arch was performed using an "arch 1st technique". Following the completion of replacement, hypotension of the left superficial temporal artery pressure was detected. Ultrasonography revealed dissection of the left common carotid artery (LCCA) and compressive occlusion of the true lumen. Reconstruction of the LCCA was performed in the neck. The patient did well after the operation without any neurological abnormalities.

Traumatic carotid artery dissection causing blindness.

A case of delayed postoperative visual loss due to bilateral traumatic carotid artery dissection is presented. In patients with a major craniofacial injury due to a high-speed motor vehicle accident, we suggest that carotid artery duplex ultrasonography be used in the initial evaluation for possible carotid artery dissection. Magnetic resonance imaging of the head and neck with magnetic resonance angiography should be performed subsequently if indicated. Early diagnosis and initiation of therapy can minimize complications.

Postpartum internal carotid and vertebral arterial dissections.

OBJECTIVE: Headache and neck pain are common postpartum symptoms. However, these symptoms can rarely be associated with internal carotid artery and vertebral artery dissections. We aimed to review five cases of postpartum cervical artery dissection and to review the clinical course of previously reported cases. METHODS: Patients with postpartum dissections diagnosed at our institution since 2005 were identified through a database maintained by the senior author, and their clinical data were reviewed. Additionally, a literature search for previously reported cases was performed, and the clinic
