A pilot study to investigate the combined use of Botulinum toxin type-a and ankle foot orthosis for the treatment of spastic foot in chronic hemiplegic patients.

BACKGROUND: Botulinum toxin is commonly used to treat spastic equinus foot. This treatment seems to improve gait in hemiplegic patients when used alone or combined with an ankle-foot orthosis. However, the nature and effects of this improvement have until now rarely been studied. The aim of this study was to quantify the impact of a Botulinum toxin injection in the triceps surae of hemiplegic patients with equinus foot, used either alone or in combination with an ankle-foot orthosis, on the kinematics and dynamics of the paretic lower limb, and to determine the advantage of combining an ankle-

What variables influence the ability of an AFO to improve function and when are they indicated?

BACKGROUND: Children with spina bifida often present with functional deficits of the lower limb associated with neurosegmental lesion levels and require orthotic management. The most used orthosis for children with spina bifida is the ankle-foot orthosis (AFO). The AFO improves ambulation and reduces energy cost while walking. Despite the apparent benefits of using an AFO, limited evidence documents the influence of factors predicting the ability of an AFO to improve function and when they are indicated. These variables include AFO design, footwear, AFO-footwear combination, and data acquisiti

Metatarsal bars more effective than metatarsal pads in reducing impulse on the second metatarsal head.

BACKGROUND: The second metatarsal head is commonly involved in cases of metatarsalgia. As part of the conservative treatment, metatarsal bars and metatarsal pads are often prescribed. OBJECTIVE: To compare the effectiveness of metatarsal bars and metatarsal pads in reducing impulse on the second metatarsal head. METHOD: Thirty-five healthy subjects were monitored with an insole scanning system during walking in four different conditions: (a) wearing shoes only, (b) shoes plus metatarsal pads and shoes plus metatarsal bars, placed either (c) perpendicular to the foot axis or (d) oblique to th

Use of foot orthoses and calf stretching for individuals with medial tibial stress syndrome.

Use of orthotics and calf stretching may alleviate symptoms in runners with medial tibial stress syndrome (MTSS). The objective of this study was to determine which patients with MTSS have a positive response to off-the-shelf foot orthoses and calf stretching based on selected clinical tests to establish a clinical prediction rule. This prospective cohort/predictive validity study enrolled 23 women and men aged 22 to 44 years with symptoms of MTSS. Interventions included off-the-shelf basic foot orthotics and calf stretching. Fifteen of the 23 runners had a 50% reduction of pain in 3 weeks of

Motor adaptation during dorsiflexion-assisted walking with a powered orthosis.

A robotic ankle-foot orthosis (AFO) that provides powered assistance could adjust to varying gait dynamics much better than a rigid AFO. To provide insight into how humans would adapt to a powered AFO, we studied the response of neurologically intact subjects walking with an active dorsiflexion assist orthosis proportionally controlled by tibialis anterior electromyography (EMG). We examined the two mechanical functions of ankle dorsiflexors in gait (power absorption at heel strike and power generation at toe-off) by recruiting two groups of healthy subjects: Group One, called Continuous Contr

Locating the axis of rotation when fitting an elbow orthosis: a comparison of measurement and palpation.

No other previously published studies consider the relative motion of orthotic components positioned on the upper arm and the forearm. This study therefore measured the location and direction of the axis of rotation of an orthotic component fixed to the forearm in relation to an orthotic component fixed to the upper arm, and compared the results with those obtained by palpation. A plane flexion or extension motion of the forearm component in relation to the component on the upper arm can be described as a pure rotation about a fixed centre. However, activation of the biceps or triceps shifts t

How can push-off be preserved during use of an ankle foot orthosis in children with hemiplegia? A prospective controlled study.

Several studies indicated that walking with an ankle foot orthosis (AFO) impaired third rocker. The purpose of this study was to evaluate the effects of two types of orthoses, with similar goal settings, on gait, in a homogeneous group of children, using both barefoot and shoe walking as control conditions. Fifteen children with hemiplegia, aged between 4 and 10 years, received two types of individually tuned AFOs: common posterior leaf-spring (PLS) and Dual Carbon Fiber Spring AFO (CFO) (with carbon fibre at the dorsal part of the orthosis). Both orthoses were expected to prevent plantar flex

Quadricepsplasty in arthrogryposis (amyoplasia): long-term follow-up.

Eight patients with arthrogryposis multiplex congenita (amyoplasia type) (11 knees) with knee hyperextension deformity underwent quadricepsplasty and were analyzed during an average follow-up period of 11 years and 2 months. The results were clinically analyzed based on gait pattern, range of movement, and orthotic requirements. Joint congruency was evaluated by radiography according to the Leveuf Pais classification. A satisfactory result was the correction of the deformity, articular congruency, sufficient range of movement, adequate gait pattern and no need for orthosis. A satisfactory outc

Use of anti-pronation taping to assess suitability of orthotic prescription: case report.

This case report describes a strategy for assessing the suitability of orthotic prescription for individual patients with lower limb overuse injuries. The case concerns a 32 year old male soccer player with a two-year history of Achilles tendinopathy. A functional assessment performed before, during, and after a trial period of anti-pronation taping showed that taping reduced symptoms markedly and resulted in a 10-fold increase in pain-free jogging distance. This was interpreted as an indication for favourable orthotic intervention. Subsequently, orthotic intervention was associated with a sim

Comparison of three ankle-foot orthosis configurations for children with spastic diplegia.

This study compared the functional efficacy of three commonly prescribed ankle-foot orthosis (AFO) configurations (solid [SAFO], hinged [HAFO], and posterior leaf spring [PLS]). Sixteen independently ambulatory children (10 males, six females; mean age 8 years 4 months, SD 2 years 4 months; range 4 years 4 months to 11 years 6 months) with spastic diplegia participated in this study. Four children were classified at level I of the Gross Motor Function Classification System (GMFCS; Palisano et al. 1997); the remaining 12 were at level II. Children were assessed barefoot (BF) at baseline (baseli

[Nonsurgical treatment of congenital dysplasia and dislocation of the hip].

Despite general ultrasound screening of the newborn, congenital dysplasia and dislocation of the hip are still among the most common congenital skeletal diseases.A differentiated and stage-adjusted therapy must be the aim because inadequate therapy can lead to necrosis of the femoral head or a residual dysplasia that both require further treatment. Now as before a correct and early diagnosis is absolutely necessary for the treatment of dysplasia of the hip.A once treated hip should be observed until completion of growth.

Stair locomotion in children with spastic hemiplegia: the impact of three different ankle foot orthosis (AFOs) configurations.

The purpose of this study was to investigate the impact of three different ankle foot orthoses (AFO) configurations on the function and kinematics of stair locomotion in children with spastic hemiplegia. Nineteen children were evaluated barefoot and with a hinged, posterior leaf spring (PLS) and solid AFO during stair ascent and descent. Stair specific items from the Pediatric Evaluation of Disability Inventory (PEDI) were used to evaluate function, while a motion measurement system was used to evaluate kinematics. The PEDI revealed no significant differences between AFOs and barefoot, althoug

A reciprocal walking orthosis hip joint for young paediatric patients with a variety of pathological conditions.

A growing trend in the use of reciprocal walking orthoses for infant paraplegic patients, and their application for control of the lower limbs in very young total body involved cerebral palsy patients, has created a need for smaller components. A prototype design of a hip joint has been produced which provides the following features: adjustable range of flexion/extension control; override on stops to permit sitting; high lateral rigidity; no lateral bearing play; very high rigidity in the sagittal plane; low friction bearings; high resistance to torque about the vertical axis. In addition a si
