Retrospective analysis of pectoralis major myocutaneous flap surgeries performed under war conditions.

AIM: To present our experiences in using a pectoralis major myocutaneous flap in the reconstruction of surgically created defects of the neck and lower part of the head during the war in Bosnia and Herzegovina. METHODS: Retrospective analysis of medical records from 15 patients treated at the ENT Department, Tuzla University Hospital, between January 1992 and December 1996. RESULTS: Ten flaps were prepared during basic operation ("one step reconstruction of defect") and five flaps three weeks after the removal of tumor (postoperative pharyngocutaneous fistula was the reason for secondary fla

Localization of swimbladder and pectoral motoneurons involved in sound production in pimelodid catfish.

We localized the motoneurons and occipital and true spinal innervation of sound-producing organs in pimelodid catfish. Pimelodids have a stridulatory organ composed of the pectoral girdle and the first pectoral fin ray, a swimbladder with extrinsic muscles to produce drumming sounds, and a tensor tripodis (TT) muscle that inserts on the swimbladder. Sonic muscles are innervated by three branches (rostral, dorsal and caudal) of the occipital nerve (Oc) and the first two true spinal nerves (S1 and 2): pectoral spine muscles (abductor, adductor and ventral rotator) by the rostral branch of Oc and

Right submammarian thoracotomy in female patients with atrial septal defects and anomalous pulmonary venous connections. Comparison between the transpectoral and subpectoral approaches.

A right submammarian incision with anterior thoracotomy was performed in 89 female patients to repair simple and complex forms of atrial septal defects. Patients' ages ranged from 8 to 38 years (mean 21.5 years). Seventy-seven had secundum-type atrial septal defects, one had the primum type, and 11 had an associated partial anomalous pulmonary venous connection. The patients were divided into two groups: 57 patients in group A, in whom, after a breast flap was elevated, a transpectoral approach was used to enter the chest; 32 patients in group B, in whom the pectoral muscle was not divided and

MRI-based assessment for optimal breast cancer local therapy in the era of BI-RADS v2025.

Preoperative breast magnetic resonance imaging (MRI) is the most sensitive modality for assessing disease extent in newly diagnosed breast cancer, detecting additional ipsilateral disease in approximately 20% of patients. While its impact on patients' prognosis is debated, MRI may refine surgical management when it is appropriately evaluated. This review provides a practical framework for optimizing MRI interpretation for individualized surgical planning, incorporating the updated American College of Radiology Breast Imaging Reporting and Data System (BI-RADS) v2025 recommendations. Key update

Breast Reconstruction Using Combined Unilateral Hemiabdominal DIEP Flap with Lipofilling from the Contralateral Abdomen.

The deep inferior epigastric perforator (DIEP) flap is the gold standard breast reconstruction procedure for patients with adequate lower abdominal tissue and is often elevated, including the unilateral DIEP. In patients with a lower midline abdominal surgical scar, the volume of the DIEP flap used for breast reconstruction may be limited because of the cessation of blood perfusion over the scar. Several options have been used for increasing the flap volume using bipedicled flaps; however, they are more invasive for the abdominal donor site and require longer operation time for complex vascula

Integrated analysis of miRNA and mRNA expression profiles associated with wooden breast myopathy in broiler chickens.

Wooden breast (WB) myopathy has raised a worldwide concern among broiler industry during the past decade. Despite progress in understanding its etiology from transcriptional regulation, post-transcriptional mechanisms including the regulation of microRNAs (miRNAs) remain largely unknown. In the current study, we described an integrative analysis between mRNA and miRNA expression profiles of pectoralis major muscle from normal and WB myopathic broilers. A total of 1983 differentially expressed mRNAs (DEmRNAs) and 155 DEmiRNAs were identified in WB. We screened crucial biological processes and c

The QUILT study: quilting sutures in patients undergoing breast cancer surgery: a stepped wedge cluster randomized trial study.

BACKGROUND: Seroma is the most common complication following breast cancer surgery, with reported incidence up to 90%. Seroma causes patient discomfort, is associated with surgical site infections (SSI), often requires treatment and increases healthcare consumption. The quilting suture technique, in which the skin flaps are sutured to the pectoralis muscle, leads to a significant reduction of seroma with a decrease in the number of aspirations and surgical site infections. However, implementation is lagging due to unknown side effects, increase in operation time and cost effectiveness. Main ob

Management of irreparable subscapularis tears: Current concepts.

Primary repair of acute subscapularis (SSC) tears provides excellent results, but tendon retraction, muscle atrophy, fatty infiltration, and humeral head migration may render a more chronic tear irreparable. These irreparable SSC tears present a diagnostic and treatment challenge for orthopaedic surgeons. Careful physical examination and imaging evaluation can help to distinguish those with reparable versus irreparable tears, but they are still not very reliable due to the methodological limitations of current evidence. Therefore, future research using 3D and quantitative measurement technique

Synchronous cervical necrotizing fasciitis and pharyngocutaneous fistula: A case report.

INTRODUCTION AND IMPORTANCE: Necrotizing fasciitis is a fulminant infection that spreads along the fascial planes. It is a rare entity with potentially fatal outcomes. The head and neck involvement is infrequent, with primary source either odontogenic or pharyngeal infection by single or mixed bacterial flora. To our knowledge, synchronous cervical necrotizing fasciitis (CNF) and pharyngocutaneous fistula is rarely reported in pieces of literature. CASE PRESENTATION: We present a 38-years-old female patient who presented with CNF and pharyngocutaneous fistula. Diabetes mellitus was accidental

Analysis of Nipple-Areola Complex Localization Using Male Cadavers: Considerations for Gender-Affirming Surgery.

BACKGROUND: Masculinizing chest reconstruction is the most common gender-affirming surgery in transgender males. Despite the current literature's acknowledgment of the vital role that proper placement of the nipple-areola complex (NAC) plays in a masculine chest contour, there is still much debate regarding the best anatomical landmarks to achieve the desired result. OBJECTIVES: The primary aim of this study is to determine which landmarks for NAC placement can be applied across diverse body types and aid surgeons in creating a masculine chest. METHODS: Twenty-five formaldehyde-embalmed male

Prepectoral Revision Breast Reconstruction for Treatment of Implant-Associated Animation Deformity: A Review of 102 Reconstructions.

BACKGROUND: Animation deformity is a direct consequence of subpectoral implant placement for breast reconstruction following mastectomy. Current treatment options ameliorate but do not address the source of the problem. Moving the implant from subpectoral to prepectoral has the potential to eliminate animation deformity. OBJECTIVES: Describe the technique and outcomes of prepectoral revision reconstruction in over 100 cases and discuss patient selection criteria for a successful outcome. METHODS: Patients who presented with animation deformity following two-stage implant reconstruction were

Open repair of pectus carinatum.

Pectus carinatum is a chest deformity characterized by a protrusion of the sternum and ribs (usually 3-7 ribs). The treatment of these patients varies in relation to age. In younger patients we prefer to use a custom-made brace, surgery is the elective treatment for older patients. The minimally-invasive technique (Abramson procedure) is used rarely and for mild defects, whereas open surgery is still preferred by many surgeons to repair major deformities. In our institution we use a modified Ravitch approach trough a vertical incision, which is performed on top of the most prominent part of th

Psychological acute stress measurement using a wireless adhesive biosensor.

Stress management is essential in this modern civilization to maintain one's stress level low and reduce health risks, since stress is one of the primary causes leading to major chronic health disorders. The present study investigates the validity of stress index (SI) metric that objectively quantifies the psychological acute stress using a disposable adhesive biosensor worn on the chest called as HealthPatch(®). Eleven healthy volunteers (n=11) were attached with one HealthPatch sensor at left pectoralis major muscle along the cardiac axis to record modified Lead-II ECG. The subjects carried

Clinical application of sentinel lymph node biopsy based on axillary anatomy in breast cancer: A single institution experience.

BACKGROUND: Sentinel lymph node biopsy (SLNB), mostly with the use of vital dye or radioisotope, is a method for predicting axillary status in patients with breast cancer. Conventional axillary lymph node dissection (ALND) is used in cases where sentinel lymph node (SLN) is not detected by existing methods, but a series of studies have found that most of SLNs are present in specific anatomical spaces. We attempted to determine the feasibility of SLNB based on axillary anatomy in cases where SLN was not detected by conventional lymphatic mapping methods. METHODS: A retrospective analysis invol

Poland's syndrome in women: 24 cases study and literature review.

BACKGROUND: Since its first description in 1841, numerous variations and treatments of Poland's syndrome (congenital deficiency of the pectoralis major muscle associated with brachysyndactyly) have been reported. None of the reports, however, involved female Chinese patients. METHODS: A retrospective study of 24 female patients was conducted to guide the selection of methods of surgical reconstruction. The patients were divided into three groups according to the degree of thoracic tissue development. Type I (mild): Limited tissue loss which can be treated with simple filling with autologous f

Breast reconstruction with implants, tissue expanders and AlloDerm: predicting volume and maximizing the skin envelope in skin sparing mastectomies.

AlloDerm has been used as a tissue supplement in conjunction with the pectoralis major muscle to provide full coverage over an implant in breast reconstruction. While this method of reconstruction has shown promising results there is little known on the relationship of AlloDerm size and potential immediate expansion volume. A retrospective chart review was completed evaluating all tissue expander or primary implant reconstructions using AlloDerm. Data recorded included: The type/size of implant/expander, dimensions of the AlloDerm used, initial fill volume, number of expansions and time period

Arteriopathy in the high-performance athlete.

Pain occurs frequently in high-performance athletes and is most often due to musculoskeletal injury or strain. However, athletes who participate in sports that require highly frequent, repetitive limb motion can also experience pain from an underlying arteriopathy, which causes exercise-induced ischemia. We reviewed the clinical records and follow-up care of 3 high-performance athletes (mean age, 29.3 yr; range, 16-47 yr) who were admitted consecutively to our institution from January 2002 through May 2003, each with a diagnosis of limb ischemia due to arteriopathy. The study group comprised 3

A prospective randomized trial of single versus multiple drains in the axilla after lymphadenectomy.

Increasing duration and amount of postoperative fluid formation after axillary lymphadenectomy delays final healing. We postulated that multiple drains (instead of a single drain) might decrease postoperative fluid accumulation by their greater proximity to points of leakage. We randomized 65 women with clinical stage I or II carcinoma of the breast to single or multiple drains. They were stratified for axillary dissection or modified radical mastectomy. For axillary dissection, randomization to multiple drains meant placement of four catheters in the axilla, and randomized to the single drain

Results of grafting the anterior and posterior divisions of the upper trunk in complete palsies of the brachial plexus.

PURPOSE: In most complete brachial plexus injuries, at least 1 root still is available for grafting. We report on the results obtained with reconstruction of the brachial plexus using short sural nerve grafts that connect nonavulsed roots to the anterior, posterior, or both divisions of the upper trunk. METHODS: We prospectively studied 22 young adults with complete brachial plexus palsy who had surgical repair an average of 5 months after trauma. Sural nerve grafts connected the C5 root to the anterior division and the C6 root to the posterior division of the upper trunk. When the C6 root wa
