We describe a posterolateral transfibular neck approach to the proximal tibia. This approach was developed as an alternative to the anterolateral approach to the tibial plateau for the treatment of two fracture subtypes: depressed and split depressed fractures in which the comminution and depression are located in the posterior half of the lateral tibial condyle. Clinodactyly, the angulation of a digit in the anteroposterior plane, is often due to a longitudinal epiphyseal bracket on the radial side of the middle phalanx of the little finger. Treatment options include observation, osteotomy, and epiphyseal bar resection. Epiphyseal bar resection is a simple surgery that requires neither postoperative pin fixation nor immobilization. The most appropriate indications are in children 3 to 6 years old with radial deviation of at least 25 degrees. The procedure reliably diminishes the extent of deformity.

BACKGROUND: Metopic synostosis, apart from the pointed forehead, typically is characterized by hypotelorism with egg-shaped orbits on cephalography and the frontoorbital axis parallel or even converging superiorly. The frontoorbital axis angle is a novel parameter for analyzing and describing the orientation of the orbits. The recognized treatment of persistent congenital trigger thumb (CTT) is surgical release of the flexor pollicis longus (FPL) tendon by transection of the first annular (A1) pulley at the thumb metacarpal head. Different modifications of flaps based on the dorsal metacarpal arterial system have been reported as reliable tools for reconstruction of dorsal and palmar phalangeal soft tissue defects. For simultaneous reconstruction of the extensor aponeurosis and joint in complex injuries of the dorsal index, we used a distally based pedicled fasciotendocutaneus second dorsal metacarpal flap, including the complete vascularized extensor indicis tendon in 2 cases. Clinical results of this procedure have not been reported so far.
