Implementing opt-out hepatitis C virus (HCV) screening in Canadian provincial prisons: A model-based cost-effectiveness analysis.

BACKGROUND: Implementing opt-out hepatitis C virus (HCV) screening across Canadian provincial prisons is crucial to achieving micro-elimination. Given short incarceration lengths, the most cost-effective screening strategy remains unknown. We compared the cost-effectiveness of current standard of care (venipuncture-based HCV-antibody+HCV RNA) and 13 alternative strategies in Quebec's largest provincial prison. METHODS: A prison cohort was simulated with a Markov micro-simulation model. Strategies differed in the biomarkers, sampling methods, and number of tests used. The model considered inca…

A Senior Management Perspective on the Policy Debate of Needle and Syringe Exchange Program Provision in Irish Prisons.

This study examined from an Irish perspective the contentious policy debate about providing clean needles to injecting drug users within prison systems; specifically, it examined the views of six senior managers who were prison health staff and security management. Research participants were generally opposed to the introduction of prison-based needle and syringe exchange programs (PNSP) in Ireland. They argued that (1) PNSP were unnecessary since injecting drug use within Irish prisons has declined significantly, (2) PNSP, by making needles freely available to prisoners, would make prisons ri…

[Psychiatric medicalization in three Brazilian women's prisons: an ethnographic approach to the itineraries of criminalization, pathologization and pharmaceuticalization].

This article analyzes the psychiatric medicalization of women in three Brazilian women's prisons, from the fields of mental health, the anthropology of confinement, and gender studies. A qualitative study employing ethnographic methods was carried out from October 2006 to February 2007, which included participant observation, informal conversations, semi-structured interviews, and life histories. Understood as a state device of power, psychiatric medicalization processes are marked by three itineraries: the mass criminalization of women, pathologization of women's crimes, and psychiatric pharm…

Availability and use of healthcare resources in prisons according to the transference model: a comparative study in Spain.

OBJECTIVE: In most European countries, correctional healthcare provision is under strain or in the process of change regarding its management model and the implications that such modifications may entail for the improvement of inmates' health. This paper compares the functioning and the results of health management in a facility whose healthcare service is integrated within an autonomic health system and others which depend on the department of corrections. MATERIALS AND METHODS: Cross-sectional study where the study unit is the facility itself. From the data collected from the record of Mont

Health, Human Rights, and the Transformation of Punishment: South African Litigation to Address HIV and Tuberculosis in Prisons.

South Africa experiences the world's highest HIV burden and one of the highest burdens for tuberculosis (TB). People in prison are particularly vulnerable to these diseases. Globally, and internally in South Africa, increased attention is being paid to HIV and TB treatment and prevention in prisons, with the public health community arguing for reforms that improve respect for the human rights of incarcerated people, for example, by calling for the reduction of overcrowding and unnecessary incarceration. Despite the retributive rhetoric that is popular among politicians and the public, the cons…

Correspondence analysis to evaluate the transmission of Staphylococcus aureus strains in two New York State maximum-security prisons.

Prisons/jails are thought to amplify the transmission of Staphylococcus aureus (SA) particularly methicillin-resistant SA infection and colonisation. Two independently pooled cross-sectional samples of detainees being admitted or discharged from two New York State maximum-security prisons were used to explore this concept. Private interviews of participants were conducted, during which the anterior nares and oropharynx were sampled and assessed for SA colonisation. Log-binomial regression and correspondence analysis (CA) were used to evaluate the prevalence of colonisation at entry as compared…

The most common reasons and incentives of tendency to addiction in prisons and rehabilitation centres of Zahedan (Iran).

In most European countries the ratio of addicts and normal people is 1 to 5,000, and in some third world countries such as Morocco, Egypt, and South Africa this figure is 1 to 1,000. In Iran, the situation is worse, and there is one drug addict per 100 people. The research method in this study is analytical descriptive. The study population consisted of all rehabilitated addicted men and women who were spending their time in prison. Total sample size were 134 people (99 men and 35 women). A special designed questionnaire used to collect data, which included socio-demographic characteristics. T

Considering treatment-as-prevention scale-up for Australian prisons: a qualitative sub-study of expert stakeholders from the Australian 'surveillance and treatment of prisoners with hepatitis C' project (SToP-C).

BACKGROUND: With direct-acting antivirals dramatically reshaping the public health response to the hepatitis C virus (HCV), prisons are set to play a critical role in elimination efforts. Despite the theoretical demonstration of HCV treatment-as-prevention in prison in mathematical modeling, limited empirical data exist. The Australian 'Surveillance and Treatment of Prisoners with Hepatitis C' project (SToP-C) is the world's first trial of HCV treatment-as-prevention in prison. Drawing on interviews with HCV expert stakeholders, this paper explores the factors respondents identified as crucial…

Evaluating reintegration effectiveness differently: A realist evaluation of sports programs in Belgian male prisons using the Capability Approach.

Reintegration of (formerly) incarcerated people is often assessed through narrow behavioral metrics such as non-recidivism or compliance with supervisory conditions. Such measures overlook crucial dimensions of reintegration - dignity, agency, and access to valued opportunities that shape how people rebuild meaningful lives during and after detention. This study addresses that gap by applying the Capability Approach within a Realist Evaluation of prison sports programs in Flanders (Belgium), examining how, why, and under what conditions sport contributes to reintegration. It focuses on prison

Implementation of the STEPPS program for the treatment of self-harm behaviors in Catalonia's prisons: barriers and facilitators.

BACKGROUND: The Systems Training for Emotional Predictability and Problem Solving (STEPPS) is an evidence-based program that has been shown to be effective in reducing self-harm behaviors in Correctional Settings (CS). However, there is limited understanding of the contextual factors that impact the implementation of this intervention within such complex environments. A comprehensive understanding of the implementation process is crucial for the successful adoption of this program. This study aims to examine the factors that influence the implementation of the STEPPS program in CS, specificall

Changes in hepatitis C virus infections after implementation of an expanded treatment program in New Mexico state prisons.

BACKGROUND: Numerous leading health organizations have developed plans to eliminate hepatitis C virus (HCV), but the success of these efforts can only be assessed through systematic monitoring of infections. People in prison are at very high risk for HCV infection and New Mexico (NM) prisons have one of the highest rates of HCV in the US For nearly two decades NM Corrections Department (NMCD) and Project ECHO® have been partnering to treat HCV in NM state prisons, including a significant expansion in treatment in 2020. Since the start of the treatment scale-up in 2020, 2981 people have been tr…

Comparison of gene targets and sampling regimes for SARS-CoV-2 quantification for wastewater epidemiology in UK prisons.

Prisons are high-risk settings for infectious disease transmission, due to their enclosed and semi-enclosed environments. The proximity between prisoners and staff, and the diversity of prisons reduces the effectiveness of non-pharmaceutical interventions, such as social distancing. Therefore, alternative health monitoring methods, such as wastewater-based epidemiology (WBE), are needed to track pathogens, including SARS-CoV-2. This pilot study assessed WBE to quantify SARS-CoV-2 prevalence in prison wastewater to determine its utility within a health protection system for residents. The study…

Effectiveness of Coronavirus Disease 2019 (COVID-19) Vaccines Among Incarcerated People in California State Prisons: Retrospective Cohort Study.

BACKGROUND: Prisons and jails are high-risk settings for coronavirus disease 2019 (COVID-19). Vaccines may substantially reduce these risks, but evidence is needed on COVID-19 vaccine effectiveness for incarcerated people, who are confined in large, risky congregate settings. METHODS: We conducted a retrospective cohort study to estimate effectiveness of messenger RNA (mRNA) vaccines, BNT162b2 (Pfizer-BioNTech) and mRNA-1273 (Moderna), against confirmed severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infections among incarcerated people in California prisons from 22 December 2020…
