Transesophageal atrial pacing threshold: role of interelectrode spacing, pulse width and catheter insertion depth.

This study evaluated the role of interelectrode spacing, pulse widths greater than 10 ms, and depth of esophageal insertion on minimizing transesophageal atrial pacing threshold in 30 patients aged 1 day to 77 years. Interelectrode spacings of 15, 22 and 28 mm were evaluated by establishing strength-duration curves in 2 or more serial studies in 12 patients; electrode spacing had no effect on pacing threshold. In 23 patients studied with 22-mm electrode spacing, pulse widths of 15 and 20 ms had no significant effect on current threshold requirements compared with 10-ms pulse widths. In 20 pati…

Determination of manganese at ng ml levels in natural waters by differential pulse polarography.

A procedure based on differential pulse polarography is described for the determination of manganese at ng ml levels in fresh, estuarine and sea-water buffered at pH 9.5 with a citrate-borate solution that also serves as supporting electrolyte. The procedure is unaffected by the potentially interfering compounds most likely to occur in natural waters. Furthermore, iron (in ascorbate-borate buffer, pH 9.5) or chromium (in ascorbate-ammonia-ammonium-chloride buffer, pH 9.8) can be determined together with manganese. Some results for the concentration of manganese, iron and chromium in the River …

Successful management of pacing system malfunctions without surgery: the role of programmable pulse generators.

The value of programmable pulse generators for correcting pacing system malfunction without surgical revision was assessed in 293 patients. Twenty-five patients (8.5%) developed malfunctions from one day to 38 months after implant. Of these, 16 or 64% were successfully managed by programming alone, while nine patients required lead repositioning or a new lead. The majority (75%) of isolated pacing and sensing malfunctions were corrected by programming, but programming restored normal pacing function in only one of five patients who had combined sensing and pacing malfunctions which appeared ea…

Effects of femorofemoral grafts on postoperative sexual function: correlation with penile pulse volume recordings.

Twenty-four male patients underwent 24 femorofemoral bypass graft procedures for severely limiting claudication or limb threat. These patients were assessed for sexual function preoperatively and postoperatively. Twenty-three patients reported impotence preoperatively. Eighteen patients were studied with sequential penile pulse volume recording (PVRS), and in each case of impotence an abnormal penile PVR curve was noted. Seventeen of these 23 patients regained satisfying sexual function after femorofemoral bypass, and in most cases this correlated with an improved PVR curve. This study demonst…

Penile flow index utilizing a Doppler pulse wave analysis to identify penile vascular insufficiency.

A Doppler ultrasound probe was used in 42 men to evaluate penile circulation. Twenty-six patients had confirmation of vascular anatomy by pelvic arteriography. The average penile acceleration (peak velocity over pulse rise time) relative to the radial artery acceleration was expressed as the penile flow index. Eleven potent men had a penile flow index of 3.4 plus or minus 0.4 standard error, while 13 impotent men with vascular disease on arteriography had a penile flow index of 21.7 plus or minus 5 standard error (p less than 0.01). Patients with intermediate penile flow index values had varia…

Pre-hospital pulse pressure as an early predictor of active hemorrhage in trauma patients.

BACKGROUND: Early identification of active hemorrhage (AH) is a critical challenge in trauma care. Narrowed pulse pressure (PP) has been associated with AH at the time of emergency department arrival; however, its role in the prehospital setting remains unclear. The aim of this study was to evaluate the utility of prehospital PP as an early predictor of AH in trauma. METHODS: We conducted a retrospective multi-centre study (January 2017 - January 2020) including adult trauma patients at all three level 1 trauma centers in Alberta, Canada. AH was defined as either operative or interventional r…

Exposure to pulse modulated radiofrequency electromagnetic fields does not affect Sternberg working memory performance.

PURPOSE: To replicate a previously reported finding that radiofrequency electromagnetic field (RF-EMF) exposure improves performance on the Sternberg working memory task, as indicated by decreased reaction time, and to test whether this effect, if verified, can also be induced by exposure to an infrared electromagnetic field (IR-EMF). MATERIALS AND METHODS: Forty-eight healthy adults participated in a randomized, counterbalanced provocation study. Working memory performance was assessed using a modified Sternberg working memory task, which was calibrated to individual performance levels in a

Pulse width-dependent Monte Carlo source modeling for ultra-high dose rate electron beams.

Objective. The emergence of ultra-high dose rate (UHDR) electron beams has highlighted the need for accurate Monte Carlo (MC) source models after recent measurements revealed pulse width (PW)-dependent discrepancies in dose profiles. This work aims to develop and validate a MC source model for conventional and FLASH modes using GATE 10, including PW dependence.Approach.Percentage depth dose (PDD) curves and lateral dose profiles were measured in water using a flashDiamond detector for PWs of 0.5-5s. These data were used to optimize the parametric source model throughminimization and gamma analy…

Exercise modality influences oxygen pulse flattening during incremental cardiopulmonary exercise testing in healthy adults.

PURPOSE: To compare oxygen pulse (O₂P) responses during cardiopulmonary exercise testing (CPET) in healthy adults using two ergometers, Treadmill and Cycle ergometer, and to determine the prevalence of O₂P flattening and the modality-specific characteristics of O₂P curve dynamics. METHODS: Thirty-four healthy adults (29.4 ± 6.9 years; 35% female) completed two maximal CPETs on both Cycle and Treadmill, in a random order within three weeks. O₂P flattening, defined as the absence of a further increase in O₂P despite increasing workload, was assessed visually and confirmed quantitatively. RESUL…

Reclassifying transcranial pulse stimulation as TNUS: Nonlinear mechanics necessitate departure from the ITRUSST framework.

BACKGROUND: Transcranial ultrasound has emerged as a promising non-invasive neuromodulation modality for Alzheimer's disease (AD). However, its clinical translation is hindered by inconsistent biophysical classification between quasi-linear low-intensity focused ultrasound (LIFU) and nonlinear pulse-based approaches. OBJECTIVE: To propose a Transcranial Nonlinear Ultrasound Stimulation (TNUS) framework for the formal reclassification of Transcranial Pulse Stimulation (TPS), enabling clear differentiation of nonlinear wave mechanics from quasi-linear acoustics. METHODS: This perspective revie…

Exploring haemoglobin Köln: a rare cause of pulse oximetry and arterial oxygen saturation discrepancy.

Pulse oximetry guides the management of respiratory conditions through non-invasively monitoring arterial oxygen saturation. Unstable haemoglobin variants may produce inaccurate readings. We describe a man in his 60s with chronic obstructive pulmonary disease admitted to hospital with a lower respiratory tract infection and pulmonary emboli. His pulse oximetry saturation was 88% on 3 L nasal cannula. However, an arterial blood gas (ABG) showed his oxygen saturation (sO2) at 96.7% and his partial pressure of carbon dioxide raised to 6.0 kPa (45.0 mm Hg). His oxygen was removed, and his pulse ox…

Pulse Oximetry Screening in the Newborn: Can Modifications to the Screening Algorithm Improve Detection?

Pulse oximetry screening (POS) is a simple non-invasive tool which enables the early detection of critical congenital heart defects (CCHDs). POS has moderate sensitivity and high specificity; it is cost-effective, readily accepted by parents and healthcare professionals, and its introduction significantly reduces mortality from CCHDs. As a result, most high-income countries and many middle-income countries have introduced, or are considering introducing, POS. Despite the international uptake of POS, there is still no consensus regarding the most appropriate screening algorithm, although severa…

High-Energy-Density Plasma Nanorod by Collisionless Absorption of Ultrafast Laser Pulse Inside Dielectrics.

The generation of energetic and dense plasmas by femtosecond laser pulses within the bulk of solids can pave the way to study warm dense matter, shocks, extreme UV radiation, or the synthesis of new material phases. However, this has remained elusive because of the intrinsic dynamical effects of defocusing by the laser-generated plasma. Here, we demonstrate the generation of overcritical plasma densities inside transparent solids over long distances. We identify with experiments in bulk sapphire and first-principles simulations that femtosecond conical interference via a Bessel beam creates a …

Norepinephrine Dose and Pulse Oximetry Agreement During Critical Illness: A Prospective Repeated-Measures Study.

Background and Objectives: Pulse oximetry is essential in ICUs but may be less accurate when peripheral perfusion is impaired. Norepinephrine, the primary vasopressor for shock, may exacerbate vasoconstriction and microcirculatory alterations, increasing discrepancy between pulse oximetry saturation (SpO2) and arterial oxygen saturation (SaO2). This study aimed to evaluate whether norepinephrine dose is associated with disagreement between SpO2 and SaO2, with particular attention to measurement variability and large discrepancies. Materials and Methods: We analyzed 1050 paired SpO2-SaO2 measur…
