Pathophysiology of stroke and stroke-induced retinal ischemia: emerging role of stem cells.

The current review focuses on pathophysiology, animal models and molecular analysis of stroke and retinal ischemia, and the role of stem cells in recovery of these disease conditions. Research findings associated with ischemic stroke and retinal ischemia have been discussed, and efforts towards prevention and limiting the recurrence of ischemic diseases, as well as emerging treatment possibilities with endothelial progenitor cells (EPCs) in ischemic diseases, are presented. Although most neurological diseases are still not completely understood and reliable treatment is lacking, animal models

Severe vaso-occlusive retinopathy as the primary manifestation in a patient with systemic lupus erythematosus.

Severe vaso-occlusive retinopathy as the initial manifestation of systemic lupus erythematosus (SLE) is rare. We report a 16-year-old female who developed bilateral visual impairment. Fundus examinations showed bilateral "cherry-red spot" appearance, multiple confluent cotton wool spots, and widespread arterial occlusion. Laboratory studies showed leukopenia, antinuclear antibody (+), and anti-double-stranded DNA antibody (+). Malar rashes, oral ulcers, and bilateral knee joint tenderness were noted during physical examination. SLE was diagnosed and pulse therapy started immediately. Best corr

Therapy insight: The recognition and treatment of retinal manifestations of systemic vasculitis.

A variety of retinal signs can occur in patients who have systemic vasculitides, or who experience complications of these diseases or their treatment. Although treatment of these retinal manifestations is usually the treatment of the systemic disease, specific treatment is occasionally indicated to preserve vision. The more prevalent of the systemic vasculitides are giant cell arteritis, polyarteritis nodosa, Wegener's granulomatosis, Churg-Strauss syndrome, relapsing polychondritis and systemic lupus erythematosus. Less frequently occurring vasculitides include Takayasu's arteritis, Goodpastu

Blood supply to the retina in the laboratory shrew (Suncus murinus).

The blood supply to both retinae was studied light microscopically and by scanning electron microscopy in 48 adult laboratory shrews (Suncus murinus) of both sexes. Thirty-eight of the animals were injected into the left ventricle with Neoprene latex (Du Pont. 601A) or with Mercox (Dai Nippon Ink Ltd., CL-2R) to elucidate the blood supply to the retina from the ophthalmic artery. The remaining animals were kept for histological study of the retina. The central retinal artery, originating from the ophthalmic artery in the muscular part of the orbit, enters the optic nerve, passes through the op

A point prevalence study of 150 patients with idiopathic retinal vasculitis: 2. Clinical relevance of antiretinal autoimmunity and circulating immune complexes.

This study describes the occurrence of antiretinal antibodies and circulating immune complexes in the sera of a large series of patients with idiopathic retinal vasculitis whose ophthalmological and clinical features are presented in Part 1. Antiretinal antibodies were measured by indirect immunofluorescence and passive haemagglutination, and circulating immune complexes were measured by polyethylene glycol precipitation and Clq binding. The occurrence of antiretinal antibodies and that of circulating immune complexes were analysed in relation to each other, to severity of retinal disease, to

Ion channels and myogenic activity in retinal arterioles.

Retinal pressure autoregulation is an important mechanism that protects the retina by stabilizing retinal blood flow during changes in arterial or intraocular pressure. Similar to other vascular beds, retinal pressure autoregulation is thought to be mediated largely through the myogenic response of small arteries and arterioles which constrict when transmural pressure increases or dilate when it decreases. Over recent years, we and others have investigated the signaling pathways underlying the myogenic response in retinal arterioles, with particular emphasis on the involvement of different ion

Impact of 85 kHz versus 125 kHz SHIFT OCTA scan speeds on image quality in retinal diseases and diagnostic reliability of choroidal neovascular membranes.

The purpose of the study was to compare Optical Coherence Tomography Angiography (OCTA) scan speeds of 85 kHz and 125 kHz with respect to image quality, diagnostic reliability, and scan time in patients with retinal diseases. In this prospective cohort study, OCTA images were obtained at both scan speeds in 70 eyes from 40 patients with retinal diseases. Masked expert graders evaluated qualitative parameters including clinical utility, artifacts, and overall image quality. Quantitative parameters including scan time, Heidelberg Q-score, and OCTA-Q score were recorded. In 46 eyes with visible c

Fluorescein angiography of retinal vascular involution after intravitreal injection of ranibizumab for retinopathy of prematurity.

AIM: To describe the involution patterns of vessel growth of retina through fluorescein angiography (FA) of children, who had been under treatment up to 1y previously intravitreal ranibizumab (IVR) as monotherapy for retinopathy of prematurity (ROP). METHODS: This is a retrospective study. The medical information and FA of 17 children (34 eyes) whose area of avascular retina from the ora serrata was more than two disc diameters (DD) were analyzed. RESULTS: Among 34 eyes, all were the presence of finger-shaped vessels and arteriolar-venular shunts (100%, 34/34 eyes). Popcorn abnormalities wer

Spatial and temporal localization of caveolin-1 protein in the developing retina.

Caveolin-1 (Cav-1), the signature protein of caveolae is expressed in several cell types in the adult retina and is linked to ocular pathologies including uveitis, diabetic retinopathy, and primary open angle glaucoma. Genetic ablation of Cav-1 causes retinal functional deficits due to disruptions in environmental homeostasis. To better understand Cav-1 function in the retina, we examined its expression/localization during postnatal retinal development. From P0-P5, Cav-1 was detected only in the developing superficial retinal vessels, in hyaloid and choroidal vasculature, and in the retinal pi…

[The most important signs of a glaucomatous disc].

Glaucomatous optic atrophy is the result of both primary and secondary glaucomas leading to functional defects. Loss of axons and ganglion cells leads to a specific atrophy of the optic nerve head. Increased cup size and depth with loss of neuroretinal rim tissue are accompanied by changes of the retinal vessels and juxtapapillary retinal pigment epithelium. Changes of the retinal nerve fibre layer, caused by ganglion cell loss may be earliest signs of glaucomatous atrophy. The marked variability of optic disc size and form may influence the quality of ophthalmoscopic evaluation in extreme var…

Coats' disease and persistent hyperplastic primary vitreous. Role of MR imaging and CT.

Coats' disease is an idiopathic disorder in which telangiectatic and aneurysmal retinal vessels leak a lipoproteinaceous exudate, with consequent bullous retinal detachment. It is a diagnostic challenge, and CT and MR imaging provide valuable information to differentiate it from other pathologies, particularly from retinoblastoma. Typical, advanced Coats' disease shows on CT a denser substance posterior to the vitreous, which on MR is hyperintense on all pulse sequences. Contrast administration on both CT and MR might give slight linear enhancement at the boundary between vitreous and exudatio…

Retinal involvement in familial exudative vitreoretinopathy.

34 cases of familial exudative vitreoretinopathy were seen during a 40-month period ending April 1981. There were 30 cases from 11 families and 4 sporadic cases. The familial cases showed the mode of inheritance compatible with the autosomal-dominant inheritance. Ophthalmoscopy and super-wide panoramic fluorescein angiography revealed characteristic retinal vascular abnormalities in all cases, embracing supernumerous vascular branchings, arteriovenous shunt formation in the extreme periphery, generalized hyperpermeability of vessels and incomplete arteriovenous interdigitating pattern. Vitreor

Assessment of the Optic Disc and Retinal Microvasculature by Optical Coherence Tomography Angiography in Patients With Pediatric Migraine.

BACKGROUND: Migraine, as a chronic neurovascular disease, is known to be a risk factor for retinal and optic nerve head damage. Herein, we aimed to evaluate the optic disc and retinal microvasculature in pediatric migraine (PM) patients using optical coherence tomography angiography (OCTA). METHODS: Forty-six eyes of 23 patients with PM without aura (PM group) and 46 eyes of 23 age- and sex-matched healthy subjects (control group) were included in this cross-sectional prospective study. Demographic features and ophthalmological examination including OCTA measurements were evaluated. OCTA was

Narrowing of the retinal arterioles in descending optic atrophy. A quantitative clinical study.

Measurements were made on fundus photographs of the diameters of retinal arterioles in a series of cases with various nonvascular lesions of the anterior visual pathways, before and after the development of fundus signs of optic atrophy. A simple method was devised for reliable estimation of the diameter of the central retinal artery whenever this vessel could not be measured directly: this allowed comparisons to be made between different eyes, irrespective of individual variations in arteriolar branching patterns. Atrophy was consistently associated with a decrease in central retinal artery c

Looking Beyond the Surface: Multimodal Assessment of Subclinical Microvascular Changes in Juvenile SLE Using OCTA and Nailfold Capillaroscopy.

PURPOSE: Juvenile systemic lupus erythematosus (jSLE) is characterized by immune-mediated microvascular injury. Optical coherence tomography angiography (OCTA) and nailfold videocapillaroscopy (NVC) enable noninvasive assessment of microcirculation. This study aimed to evaluate retinal structural and microvascular alterations in jSLE using OCT/OCTA and investigate their association with peripheral microvascular findings evaluated by NVC. METHODS: The study included 31 patients with jSLE and 43 healthy controls. Retinal structural parameters, vessel density of the superficial (SCP) and deep ca
