Radiofrequency in arthroscopic shoulder surgery: a systematic review.

BACKGROUND: Radiofrequency has seen an increase in use in orthopedics including cartilage lesion debridement in the hip and knee as well as many applications in arthroscopic shoulder surgery. The purpose of this systematic review is to evaluate the safety and usage of radiofrequency in the shoulder. METHODS: This systematic review was registered with PROSPERO (international registry) and followed the preferred reporting items for systematic review and meta-analysis protocols (PRISMA-P) guidelines. Embase and PubMed were searched using: "shoulder," "rotator cuff," "biceps," "acromion" AND "mon

Non-whiplash soft tissue injuries following low-velocity impact collisions: A retrospective analysis.

Whiplash is the most common injury reported in low-velocity impact road traffic accidents, but claimants report a range of other soft tissue injuries and we looked at the prevalence of these and any patterns. We retrospectively reviewed reports from the medico-legal practice of an orthopaedic surgeon. We collected data on 609 claimants. Sixty-two per cent were women (p < 0.0001). Majority (90.3%) reported an injury to at least one other area than neck, and 48% of claimants reported shoulder pain. Hand/wrist injuries were almost exclusive to drivers (p = 0.0013). Those with knee injuries were o

Recovery of Shoulder, Elbow, and Forearm Movement After Nerve Reconstruction for Neonatal Brachial Plexus Palsy.

BACKGROUND AND OBJECTIVES: There is a relative dearth of published data with respect to recovery of upper extremity movement after nerve reconstruction for neonatal brachial plexus palsy (NBPP). This study aimed to demonstrate long-term recovery of active range of motion (AROM) at the shoulder, elbow, and forearm after nerve reconstruction for NBPP and to compare that with patients managed nonoperatively. METHODS: We interrogated a prospectively collected database of all patients evaluated for NBPP at a single institution from 2005 to 2020. AROM measurements for shoulder, elbow, and forearm m

A lesion of the musculocutaneous nerve. A rare complication of anterior shoulder dislocation.

A 47-year-old male patient with an anterior shoulder dislocation underwent reduction 3 days later. A residual neurologic injury remained, consisting primarily in weakness of shoulder and elbow flexion and sensory involvement of the ventral side of the forearm. Secondary sequelae included muscle atrophy with a concomitant frozen shoulder. The clinical examination and electromyography revealed a lesion of the musculocutaneous nerve, which is a very rare complication in shoulder dislocation.

Influence of Neighborhood Disparities on Traumatic Shoulder Instability Severity and Timing of Care in Adolescents.

BACKGROUND: Identifying and understanding socioeconomic disparities among adolescents with traumatic shoulder instability can help to optimize care for patients by improving differences in the disease burden, disease severity, and awareness of resource limitations. Current studies evaluating disparities among patients with shoulder instability are limited to the adult population or surgical patients. PURPOSE: To evaluate how educational, health/environmental, and social/economic disparities influence the timing of shoulder instability treatment and shoulder instability severity among adolesce

Shoulder Dystocia: Managing an Obstetric Emergency.

Shoulder dystocia is an obstetric emergency in which normal traction on the fetal head does not lead to delivery of the shoulders. This can cause neonatal brachial plexus injuries, hypoxia, and maternal trauma, including damage to the bladder, anal sphincter, and rectum, and postpartum hemorrhage. Although fetal macrosomia, prior shoulder dystocia, and preexisting or gestational diabetes mellitus increases the risk of shoulder dystocia, most cases occur without warning. Labor and delivery teams should always be prepared to recognize and treat this emergency. Training and simulation exercises i

Fracture of the scapular body: functional outcome after conservative treatment.

BACKGROUND: Scapular body fractures, irrespective of the number of fragments, are usually managed non-operatively, with favorable results. We evaluated results after non-operative management to determine the outcomes of scapular body fractures. METHODS: We evaluated 22 patients following a fracture of the scapular body that had been treated non-operatively. To assess the influence of accompanying injuries other than those of the involved limb, we used both patient-oriented general health and shoulder-based outcome measures. RESULTS: In this series the functional outcome, reflected by the Dis

A Sports Medicine Clinician's Guide to the Diagnosis and Management of Distal Clavicular Osteolysis.

Weightlifting associated shoulder injuries have seen a dramatic rise in the last 20 years. Distal clavicular osteolysis, coined weightlifter's shoulder, is one such condition caused by repetitive microtrauma to the distal clavicle with subsequent, painful development of bony erosions and resorption of the distal clavicle. Diagnosis, treatment, and prevention of this condition can be challenging. In this article, we highlight evidence-based clinical recommendations for the diagnosis and management of distal clavicular osteolysis, including specific considerations for atraumatic and posttraumati…

Glenohumeral fusion in adults with sequelae of obstetrical brachial plexus injury: a report of eight cases.

The purpose of this study was to assess the outcomes of glenohumeral fusion performed in adulthood in patients with poor recovery after obstetric brachial plexus injury. We reviewed eight patients with obstetrical brachial plexus injury who had undergone shoulder arthrodesis. The mean age was 28 years (range 16 to 55). All of the patients had active periscapular and elbow flexor muscles. At an average follow-up of 20 months, the active range of abduction and external rotation of the affected shoulder was 67° (range 50° to 85°) and 21° (range 0° to 40°), respectively. All of the patients were s

Thoracic outlet syndrome and the shoulder.

Thoracic outlet syndrome may be the cause of symptoms wrongly attributed to shoulder pathology, or it may accompany or result from shoulder injury. If misdiagnosed, it may leave the physician wondering why relevant shoulder therapy fails to result in a return to full potential, but if correctly diagnosed, it can be cured or ameliorated.

Irreducible anterior dislocation of the shoulder due to soft tissue interposition of subscapularis tendon.

The shoulder joint is the most unstable joint in the body and is easily dislocated. Anterior shoulder dislocation is the commonest and can be associated with glenoid and humeral fractures. Anterior shoulder dislocations are not infrequently associated with cuff tendon tears; however, anterior shoulder dislocation is easily reducible. Irreducible anterior dislocation of the glenohumeral joint is uncommon, and this could be due to bony as well as soft tissue causes. Persistent anterior dislocation due to torn subscapularis interposition in the glenohumeral joint is very rare, and only a few oper

From the unstable painful shoulder to multidirectional instability in the young athlete.

In conclusion, instability as a cause of shoulder pain in the young athlete is a difficult and often missed diagnosis. These young patients often seek treatment of shoulder pain but do not recall any episodes of shoulder instability. As a result, these uncommon, poorly described forms of instability are often misdiagnosed. A heightened clinical suspicion and an accurate, prompt diagnosis of instability is of paramount importance in this athletic group. It dictates appropriate treatment of the condition, avoids treatment delays and failure, provides better outcomes, and ensures timely return to

Causal effect of body mass index and physical activity on the risk of joint sports injuries: Mendelian randomization analysis in the European population.

BACKGROUND: Observational studies can suggest potential associations between variables but cannot establish a causal effect on their own. This study explored the causal associations between body mass index (BMI), physical activity (PA), and joint sports injuries. METHODS: We conducted two-sample Mendelian randomization (MR) using publicly accessed genome-wide association studies (GWAS) datasets to investigate the causal effects of BMI and PA on joint sports injury risk. The inverse-variance weighted method was believed to be the primary MR analysis. Subsequently, sensitivity, pleiotropy, and

Injuries to belted older children in motor vehicle crashes.

CONTEXT: The American Academy of Pediatrics and the National Highway Traffic Safety Administration currently recommend that, unless they are under 57in. in height, 8-12-year-old children use seat belts and all should ride in the rear seats of vehicles. These recommendations assume that the vehicle seat belt should provide adequate protection for these older children in the event of a crash. OBJECTIVES: To describe characteristics of older children in the rear seat using seat belts in crashes, to estimate their risk and body region distribution of injury, and to identify risk factors for injur
