[The Gulf War Syndrome twenty years on].

INTRODUCTION: After Operation Desert Storm which took place in Iraq from August 1990 to July 1991 involving a coalition of 35 countries and a 700,000 strong contingent of mainly American men, some associations of war veterans, the media and researchers described a new diagnostic entity: the Gulf War Syndrome (GWS). LITERATURE FINDINGS: GWS seems to be a new disorder which associates a litany of functional symptoms integrating the musculoskeletal, digestive, tegumentary and neurosensory systems. The symptoms presented do not allow a syndrome already known to be considered and the aetiology of

Screening for psychosocial risk factors in patients with chronic orofacial pain: recent advances.

BACKGROUND: The authors compiled information about recent advances in screening for psychosocial risk factors considered to be yellow flags for potentially poor outcomes among patients with chronic orofacial pain (most commonly temporomandibular disorders). TYPES OF STUDIES REVIEWED: The authors conducted MEDLINE searches for the period 1995 through 2002 using the terms "temporomandibular disorders," "assessment" and "psychological," as well as "primary care," "screening" and "psychological disorders." They also searched personal files for relevant articles. RESULTS: Psychosocial dysfunction

The Thematic Apperception Test and Alzheimer's disease.

The Thematic Apperception Test (TAT; Murray, 1938, 1943) is widely used in personality description and psychological assessment. Similar narrative tasks are also frequently used as part of neuropsychological assessment. Little is empirically known, however, concerning performances of cognitively impaired individuals on thematic tasks when compared with other diagnostic groups. It was predicted that hospitalized patients with dementia of the Alzheimer's type (DAT) would demonstrate impaired performances on the TAT when compared with a group of nondemented psychiatric inpatients. This prediction

Psychological distress following criminal victimization in the general population: cross-sectional, longitudinal, and prospective analyses.

Samples of 105 violent crime victims, 227 property crime victims, and 190 nonvictims provided normative data regarding levels of psychological distress following criminal victimization. At points approximately 3 months, 9 months, and 15 months postcrime, symptoms of depression, somatization, hostility, anxiety, phobic anxiety, fear of crime, and avoidance were assessed. Although crime victims showed substantial improvement between 3 and 9 months, thereafter they did not. Over the course of the study, violent crime victims remained more distressed than did property crime victims who, in turn, r

Locus of control, repression-sensitization, and psychological disorder in chronic pain patients.

Examined the relationship between locus of control and psychological disorder in chronic pain patients controlling for the effects of response sets. Fifty-nine patients with chronic low back pain were given Rotter's Internal-External Locus of Control (I-E) scale and the MMPI. Multiple regression analysis showed that the MMPI K, F, and Pt scales produced the most efficient prediction of I-E scores. Response set tendencies to dramatize problems and traits that involve excessive rumination and self-doubt were related to external locus of control.

Psychiatric diseases presenting as infectious diseases.

Although many psychiatric diseases have somatic manifestations, some focus on fears or delusions of infection. When a patient with a psychiatric basis for an apparent infection presents to an infectious disease physician, the physician may find the problem confusing, amusing, and ultimately frustrating until the psychiatric basis for disease is recognized. Some of these psychiatric disorders can be treated and controlled with medication and psychotherapy, although patients may resist psychiatric referral. This article reviews examples of psychiatric disorders in patients who present to the inf

Comparison of lifetime psychiatric diagnoses in Premenstrual Syndrome Clinic and community samples.

The purpose of this investigation was to obtain information about lifetime psychiatric diagnoses of women seeking treatment for premenstrual syndrome. The National Institute for Mental Health Diagnostic Interview Schedule (DIS) was administered to 223 women attending a premenstrual syndrome clinic. Rates of symptoms and psychiatric disorders were then compared with DIS data collected from an Epidemiologic Catchment Area (ECA) program community sample of 923 women in the same age group from the same geographic location. Women in the Premenstrual Syndrome Clinic sample met DIS/DSM-III criteria f

Changes in the temporomandibular joint clicking and pain disorders after orthognathic surgery: Comparison of orthodontics-first approach and surgery-first approach.

The purposes of this study were to investigate the influence of the orthodontics-first approach (OFA) and surgery-first approach (SFA) on changes in the signs and symptoms of temporomandibular joint disorders (TMDs) and to compare pre- and postoperative orthodontic treatment duration and total treatment duration between the two approaches. This retrospective study recruited 182 adult patients with malocclusions treated with OFA and SFA and recorded variables such as age, gender, skeletal classification, and signs and symptoms of TMD (clicking and pain disorders) before the start of the surgica

Gender-Stratified Prevalence of Psychiatric and Pain Diagnoses in a Primary Care Patient Sample with Fibromyalgia.

OBJECTIVE: Comorbid psychiatric and pain-related conditions are common in patients with fibromyalgia. Most studies in this area have used data from patients in specialty care and may not represent the characteristics of fibromyalgia in primary care patients. We sought to fill gaps in the literature by determining if the association between psychiatric diagnoses, conditions associated with chronic pain, and fibromyalgia differed by gender in a primary care patient population. DESIGN: Retrospective cohort. SETTING AND SUBJECTS: Medical record data obtained from 38,976 patients, ≥18 years of ag

Fearful Attachment Linked to Childhood Abuse, Alexithymia, and Depression in Motor Functional Neurological Disorders.

Insecure attachment is a predisposing risk factor for the development of functional neurological disorder (FND). There is limited research investigating connections between attachment styles, other predisposing vulnerabilities, and symptom severity in patients with motor FND. By using a within-group design with prospective data collection, the authors performed univariate tests followed by multivariate linear regressions to investigate neuropsychiatric factors associated with four attachment styles (secure, fearful, preoccupied, and dismissing) among 56 patients with motor FND (mean age=40.2 y

The Dancing Manias: Psychogenic Illness as a Social Phenomenon.

The dancing mania erupted in the 14th century in the wake of the Black Death, and recurred for centuries in central Europe - particularly Germany, the Netherlands, and Belgium - finally abating in the early 17th century. The term "dancing mania" was derived from "choreomania," a concatenation of choros (dance) and mania (madness). A variant, tarantism, was prevalent in southern Italy from the 15th to the 17th centuries, and was attributed at the time to bites from the tarantula spider. Affected individuals participated in continuous, prolonged, erratic, often frenzied and sometimes erotic, dan

Somatic symptoms in those with performance and interaction anxiety.

This study (n = 304) examined the relationship between somatic symptoms and social anxiety. Significant differences in the experience of somatic symptoms were found among four groups (i.e. performance anxious, interaction anxious, generalized socially anxious, and controls). Post hoc analyses revealed that those who exceeded the clinical cutoff for generalized social anxiety exhibited more somatic symptoms than those who exceeded the clinical cutoff in the other two social anxiety domains or controls. Individuals in each group exhibited more somatic symptoms than controls, but subtypes did not

High doses alprazolam induced amenorrhoea and galactorrhoea.

Alprazolam belongs in the group of anxiolytics, medicaments used for reducing anxiety. As most other drugs, it can cause various adverse events, including hormonal disturbances and imbalance. Prolactin elevation is one such hormonal adverse event that can lead to galactorrhoea, or abnormal milk discharge from the breast and amenorrhoea. In this case report we will present the case of a female patient that developed galactorrhoea while treated with alprazolam, after all physical factors that can also cause these symptoms were excluded.

[Stendhal syndrome: origin, characteristics and presentation in a group of neurologists].

INTRODUCTION: Travelling, when searching for knowledge and emotion, can cause psychic discomfort that occasionally leads the traveller to seek medical attention. The psychiatrist Graziella Magherini described, in tourists visiting Florence, acute attacks including disorders of thought and affects, and even including, anxiety attack. She named it the Stendhal syndrome (SS) remembering the experience of the writer when visiting the Basilica of Santa Croce in Florence. METHODS: We attempt to investigate the incidence of SS or isolated symptoms related to it, in a homogeneous group of travellers.

Non-epileptic attack disorder: a psychological perspective.

Due to the serious medical consequences in failing to recognize non-epileptic attack disorder (NEAD), and the frequency with which neurologists come into contact with such patients, clearly NEAD constitutes a major concern for clinicians in the field of epilepsy. This article presents the psychological characteristics of 185 patients with NEAD. Psychological factors that were identified as being important in the understanding of the development and maintenance of NEAD included: anxiety or stress; physical abuse; significant bereavement; family dysfunctioning; relationship problems; depression;

Imagery rehearsal treatment for chronic nightmares.

Fifty-eight chronic nightmare sufferers were randomly assigned to two groups: treatment (n = 39) and wait-list control (n = 19). Treated Ss were taught a cognitive-behavioral technique called imagery rehearsal in which they learned in a waking state to change a nightmare and then to visualize the new set of images. Subjects were assessed pre-treatment and 3 months followup for nightmare frequency, self-rated distress and subjective sleep quality. Compared to controls, the treatment group showed significant and clinically meaningful decreases in nightmares. Treated Ss decreased nightmares as me

The effect of major railway accidents on the psychological health of train drivers--I. Acute psychological responses to accident.

The acute psychological reactions of 101 train drivers to on-the-track accidents were studied by means of clinical interviews and questionnaires (Impact of Event Scale, GHQ-20 and a questionnaire addressing stress symptoms, pre-accident expectancies and worries). More than half of the train drivers reported moderate to high intrusive distress (mean 11.3) within hours to days after the accident but only 1/3 reported symptoms of acute psychophysiological arousal. Intrusive symptoms related to visual impressions were most frequently reported. Avoidance was less prevalent (mean 8.8). Clinical inte

The comorbidity of DSM-III-R personality disorders in somatization disorder.

In order to understand psychiatric factors that complicate the medical management of somatizing patients, 94 subjects with known somatization disorder (SD) were evaluated for 13 personality disorders with the Structured Clinical Interview for DSM-III-R Personality Disorders. Referred from multiple primary care settings, the patient sample was predominantly female (85%), married (67%), high school graduates (64%), and had a mean age of 43. Structured interviews documented that 23.4% of SD patients had one personality disorder, and 37.2% had two or more disorders. The four most frequently identi

An investigation of 'functional' somatic symptoms among patients attending hospital medical clinics in Pakistan--I. Characteristics of 'non-organic' patients.

Six hundred medical clinic attenders at the Mayo Hospital, Lahore, Pakistan, were screened using the Bradford Somatic Inventory (BSI) in Urdu. The physicians' clinical diagnosis was recorded. The characteristics of mood-disordered, functional and no-diagnosis groups were compared with patients receiving organic diagnoses. Of the non-organic patients, only the mood-disordered group reported significantly more BSI symptoms than the organic group. The functional group did not have especially high scores. All groups reported a greater mean number of BSI symptoms than a student comparison group. Am

[Parent-child relations as a predisposition for psychogenic pain syndrome in adulthood. A controlled, retrospective study in relation to G. L. Engel's "pain-proneness"].

In a retrospective study on 151 chronic pain patients the relevance of childhood factors as described by G. L. Engel ("pain-proneness") are empirically investigated. All patients are studied by a structured interview developed for pain patients. Two groups were clinically differentiated and compared regarding childhood development: A psychogenic pain group (PP) without any somatic pathology (IASP-classification axis 5: 09; n = 75) and a group of chronic pain patients with somatic pathology (OP; n = 35). A "psychosomatic group" (IASP-axis 5: 07, e.g. migraine; n = 41) was excluded from further
