BACKGROUND/PURPOSE: This study aimed to explore the alteration of bile acid (BA) profiles in patients with choledocholithiasis (CDC) and construct a prediction model for evaluating the risk of common bile duct stone (CBDS) recurrence after endoscopic treatment.
METHODS: A total of 320 patients (218 with CDC and 102 with nonneoplastic polyps) were enrolled. The serum BA profiles were compared between groups. Both diagnostic score of CDC and prognostic risk score of CBDS recurrence based on BAs were established by least absolute shrinkage and selection operator regression. A nomogram model was

BACKGROUND AND STUDY AIMS: Postendoscopic retrograde cholangiopancreatography (ERCP) complications increase with repeated cannulation attempts. We evaluated several advanced biliary cannulation techniques, which have been used when the standard approach fails, to increase the success rate and decrease post-ERCP complications. We aimed to evaluate the use of double-wire technique in terms of success rates and effects on post-ERCP pancreatitis (PEP) and to assess the value of pancreatic duct stenting following needle-knife sphincterotomy in difficult biliary cannulation.
PATIENTS AND METHODS: A…

The most effective and the standard treatment for bile duct stones (BDSs) is endoscopic retrograde cholangiopancreatography (ERCP). However, in 10% to 15% of patients with BDSs, the stones cannot be removed by conventional ERCP, which involves endoscopic sphincterotomy followed by balloon or basket extraction. Additional techniques or devices are often necessary to remove these difficult bileduct stones, including endoscopic papillary large balloon dilatation to make a larger papillary opening and/or mechanical lithotripsy to fragment the stones. Advances in cholangioscopy have made possible e…

Choledocholithiasis is a common disorder that is managed universally by endoscopic retrograde cholangiopancreatography (ERCP). For difficult or complex stones, ERCP with conventional techniques may fail to achieve biliary clearance in 10% to 15% of cases. This review summarizes the literature regarding the current available endoscopic techniques for complex stone disease, including mechanical lithotripsy, endoscopic papillary large balloon dilation, cholangioscopy-guided lithotripsy, and endoscopic ultrasound-guided biliary access.

The article is dedicated to non-typical methods of operation of endoscopical papillothomiya. The article presents the history of elaboration of different methods of non-typical endoscopical papillothomiya, the results of analyze of data from the literature with a survey of discussable items about the necessity, effectiveness and safety of non-typical methods of endocsopical papillothomiya. The authors offered the classification of these methods, defined the conditions of using and gave the data of their own researches of effectiveness and frequency of complications during the process of operat

The results of surgical treatment of 156 patients with chronic calculous cholecystitis complicated by choledocholithiasis have been analysed. In 62 patients, cholecystectomy in combination with choledocholithotomy was performed. Ninety four patients underwent additional drainage operations on the terminal common bile duct. Endoscopic papillosphincterotomy was performed in 90% of cases, other drainage interventions (choledochoduodenostomy and transduodenal papillosphincterotomy)--in 10%.

Langenbecks Archiv fur Chirurgie. Supplement II, Verhandlungen der Deutschen Gesellschaft fur Chirurgie. Deutsche Gesellschaft fur Chirurgie. Kongress1990ger
Cholecystectomy, hitherto the "golden standard" in gallstone treatment, and all the other alternative methods must pass the test of applicability, morbidity, mortality, rate of success and recurrence. According to these criteria, cholecystectomy (removing not only the stones but also the offending gallbladder)--in particular with the advent of the laparoscopic approach--is the therapy of choice.

Endoscopic sphincterotomy (ES) with stone removal is indicated in the post-cholecystectomy patient with retained or recurrent stones and for the very high risk surgical patient. ES has a role in the treatment of severe acute biliary pancreatitis (ABP), but not in mild ABP. Acute suppurative cholangitis refractory to antibiotics can be successfully treated by ES with stone removal or nasobiliary drainage not only in high risk surgical patients. However, concomitant occurrence of abscesses in the liver and other locations, and of gallbladder empyema, calls for an interdisciplinary approach (surg…

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) is a crucial endoscopic procedure for pancreato-biliary diseases for diagnostic and therapeutic purposes. Although commonly performed, difficult biliary cannulation (DBC) cases remain challenging when using conventional technology alone.
OBJECTIVE: This study aimed to assess the efficacy and safety of rescue needle knife papillotomy (RNKP) compared to sphincterotomy only (SPTO) in DBC. Methods: A retrospective observational study with sequential intervention analysis to evaluate outcomes between the SPTO and RNKP groups among …

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) is a major option for common bile duct (CBD) stones. Endoscopic sphincterotomy (EST), endoscopic papillary balloon dilatation (EPBD), and endoscopic sphincterotomy plus balloon dilatation (ESBD) are procedures for opening the bile duct orifice to extract CBD stones during ERCP. The optimal method for extracting small CBD stones (≤ 10 mm) has not yet been proposed. We aimed to compare the efficacy and safety of these three techniques in extracting small CBD stones.
METHODS: ERCP for small stones was performed between January 200…

BACKGROUND: Cholecystectomy after endoscopic sphincterotomy for bile duct stones with concomitant gallstones is known to reduce late biliary complications. Endoscopic papillary balloon dilation for bile duct stones develops fewer late biliary complications than endoscopic sphincterotomy, but no randomized controlled trials have been conducted about the role of cholecystectomy after endoscopic papillary balloon dilation. Therefore, we conducted this propensity score-matched analysis to compare cholecystectomy and wait-and-see approach after endoscopic papillary balloon dilation.
