Uncertain call likelihood negatively affects sleep and next-day cognitive performance while on-call in a laboratory environment.

On-call working arrangements are employed in a number of industries to manage unpredictable events, and often involve tasks that are safety- or time-critical. This study investigated the effects of call likelihood during an overnight on-call shift on self-reported pre-bed anxiety, sleep and next-day cognitive performance. A four-night laboratory-based protocol was employed, with an adaptation, a control and two counterbalanced on-call nights. On one on-call night, participants were instructed that they would definitely be called during the night, while on the other on-call night they were told

The Effectiveness of Low-Level Light Therapy in Attenuating Vocal Fatigue.

OBJECTIVES: Low-level light therapy (LLLT) is effective in reducing inflammation, promoting wound healing, and preventing tissue damage, but has not yet been studied in the treatment of voice disorders. The objective of this study was to investigate the possible effectiveness of LLLT in attenuating symptoms of vocal fatigue created by a vocal loading task as measured by acoustic, aerodynamic, and self-reported vocal effort. METHODS: In a randomized, prospective study, 16 vocally healthy adults divided into four groups underwent a 1-hour vocal loading procedure, followed by infrared wavelength

Affective reactivity differences in pregnant and postpartum women.

Reactions to emotional cues, termed affective reactivity, promote adaptation and survival. Shifts in affective reactivity during pregnancy and postpartum may invoke altered responses to environmental and biological changes. The development and testing of affective reactivity tasks, with published normative ratings for use in studies of affective reactivity, has been based on responses provided by healthy college students. A comparison of the healthy norms with ratings provided by peripartum women has yet to be conducted, despite its value in highlighting critical differences in affective react

The burden of microstructural damage modulates cortical activation in elderly subjects with MCI and leuko-araiosis. A DTI and fMRI study.

The term leuko-araiosis (LA) describes a common chronic affection of the cerebral white matter (WM) in the elderly due to small vessel disease with variable clinical correlates. To explore whether severity of LA entails some adaptive reorganization in the cerebral cortex we evaluated with functional MRI (fMRI) the cortical activation pattern during a simple motor task in 60 subjects with mild cognitive impairment and moderate or severe (moderate-to-severe LA group, n = 46) and mild (mild LA group, n = 14) LA extension on visual rating. The microstructural damage associated with LA was measured

Freezing in Parkinson's disease: a spatiotemporal motor disorder beyond gait.

Freezing of gait (FOG) is an incapacitating problem in Parkinson's disease that is difficult to manage therapeutically. We tested the hypothesis that impaired rhythm and amplitude control is a common mechanism of freezing which is also present during other rhythmic tasks. Therefore, we compared the occurrence and spatiotemporal profiles of freezing episodes during upper limb motion, lower limb motion, and FOG. Eleven freezers, 12 non-freezers, and 11 controls performed a rhythmic bilateral finger movement task. The triggering effect of movement speed, amplitude, and coordination pattern was ev

Preparatory activity in occipital cortex in early blind humans predicts auditory perceptual performance.

Early onset blindness leads to a dramatic alteration in the way the world is perceived, a change that is detectable in the organization of the brain. Several studies have confirmed that blindness leads to functional alterations in occipital cortices that normally serve visual functions. These reorganized brain regions respond to a variety of tasks and stimuli, but their specific functions are unclear. In sighted individuals, several studies have reported preparatory activity in retinotopic areas, which enhances perceptual sensitivity. "Baseline shifts," changes in activity associated with a cu

Neural substrates of word generation during stroke recovery: the influence of cortical hypoperfusion.

Several studies have demonstrated reorganization of cognitive and motor function caused by stroke. This study examined the influence of hypoperfused brain regions, in addition to the area of the infarct itself, on reorganization of the cognitive processes underlying word generation in stroke patients. In addition, we also sought to determine the influence of hypoperfusion on the blood oxygen level dependent/(BOLD) effect. Subjects with left and right subacute or chronic subcortical strokes, along with normal controls, were imaged while performing a verbal fluency task (word generation). The st

Parietal rTMS distorts the mental number line: simulating 'spatial' neglect in healthy subjects.

Patients with left-sided visuospatial neglect, typically after damage to the right parietal lobe, show a systematic bias towards larger numbers when asked to bisect a numerical interval. This has been taken as further evidence for a spatial representation of numbers, perhaps akin to a mental number line with smaller numbers represented to the left and larger numbers to the right. Previously, contralateral neglect-like symptoms in physical line bisection have been induced in healthy subjects with repetitive transcranial magnetic stimulation (rTMS) over right posterior parietal lobe. Here we use

Therapeutic orphans no more: Revisiting the ethical justification for excluding pregnant women from pharmaceutical clinical trials.

For six decades, pregnant women have been routinely excluded from pharmaceutical clinical trials, a practice rooted in a protectionist ethic that emerged after the thalidomide tragedy. This narrative review examines whether that exclusion remains ethically justifiable, drawing on the obstetric, pharmacological, and bioethical literature together with guidance from the Council for International Organizations of Medical Sciences, the US Food and Drug Administration, and the European Medicines Agency. Far from eliminating risk, systematic exclusion has produced a critical evidence gap: most medic

Basis-driven learnable operator for MLP-mixers.

INTRODUCTION: Existing multi-layer perceptron (MLP)-mixer architectures either rely heavily on extensive training data or employ rigid, hand-engineered mixing operations. This study introduces a Basis-Driven Learnable Operator (BDLO) for MLP-mixers, targeting a balance between high-capacity flexible learning and computationally efficient, yet constrained, handcrafted solutions. METHODS: BDLO is a drop-in replacement for the shifting block of shifting-based MLP-mixers. It approximates shifting-based mixing operations by learning the coefficients of a real, complete, discrete basis, yielding an

The effects of different sensory augmentation on weight-shifting balance exercises in Parkinson's disease and healthy elderly people: a proof-of-concept study.

BACKGROUND: Earlier versions of biofeedback systems for balance-related applications were intended primarily to provide "alarm" signals about body tilt rather than to guide rehabilitation exercise motion. Additionally, there have been few attempts to evaluate guidance modalities for balance rehabilitation exercises. The purpose of this proof-of-concept study is to evaluate the effects of guidance modalities during common dynamic weight-shifting exercises used in clinical settings. METHODS: A motion guidance system providing visual biofeedback, vibrotactile biofeedback, or both, was used durin

Prevalence of person-centered maternity care during childbirth and its predictors among women who gave birth in health facilities of East Africa: A systematic review and meta-analysis.

Poor quality care significantly contributes to preventable maternal morbidity and mortality in sub-Saharan Africa, where respectful and responsive childbirth care remains variable. Person-centered maternity care (PCMC) is a crucial element of the WHO maternal and newborn quality-of-care framework; however, region-specific synthesis is lacking. This review aimed to estimate the pooled mean PCMC score and identify its predictors among postpartum women in East Africa. We systematically searched PubMed, Research4Life, ScienceDirect, DOAJ, and Google Scholar for observational studies conducted from

Methodologies for studying depression in persons with tuberculosis: A scoping review.

Depression is prevalent among people with tuberculosis (PWTB) and negatively affects diagnosis, treatment adherence, and clinical outcomes. However, the methodological approaches used to study depression among PWTB have not been systematically mapped. This scoping review aimed to evaluate methodologies used to study depression among PWTB and identify gaps in methodological approaches. We conducted a systematic scoping review following PRISMA-ScR guidelines. Nine databases were searched without date restrictions. Two reviewers independently screened titles, abstracts, and full texts. Inclusion

From policy to practice: perspectives on youth-friendly sexual and reproductive health service delivery in Southeast Nigeria.

BACKGROUND: Young people in Nigeria face significant barriers in accessing sexual and reproductive health (SRH) services, resulting in unmet … youth-centred service delivery, including expanding the healthcare workforce through employment and task shifting, upgrading health infrastructure, and subsidising SRH services by integrating them into basic healthcare packages. These actions are essential to advancing young people’s SRH rights and improving service delivery.

"If you want to have sex you can, if you don't, you don't have to": a qualitative evaluation of the Unite for a Better Life couples program to prevent intimate partner violence in rural Ethiopia.

BACKGROUND: Gender-transformative programing targeting couples is an important strategy to reduce intimate partner violence (IPV), but questions around efficacy and safety remain. This qualitative study explores indications of change and unintended consequences of the gender-transformative Unite for a Better Life (UBL) intervention, using data from the couples’ and control arms of a cluster randomized controlled trial (cRCT) in Ethiopia. The cRCT found no impact on physical and psychological IPV and weak evidence of reductions in sexual IPV in the couples’ arm. METHODS: We analyzed 33 facilita

The role of sexual fantasy on sexual desire, distress, and sexual worries: a randomized controlled study.

BACKGROUND: Sexual fantasies may enhance sexual desire and reduce sexual distress by redirecting focus toward emotionally salient and arousing imagery. Despite promising evidence, few experimental studies have tested whether repeated engagement in sexual fantasies can improve sexual functioning, reduce sexual distress and reduce cognitive distractions during sexual activity. OBJECTIVES: This randomized controlled trial tested the preregistered hypotheses that a four-week sexual fantasy task increases sexual desire, reduces sexual distress, and reduces worries with body image and performance d

WEDGE-Net: Wavelet-Driven Memory-Efficient Anomaly Detection for Industrial Edge Computing.

As deep learning-based Anomaly Detection (AD) transitions from theoretical research to industrial application, the focus is shifting towards operational efficiency and economic viability on edge devices. While recent studies have achieved remarkable detection accuracy on standard benchmarks, they often rely on heavy memory banks or complex backbones, which pose challenges for deployment in resource-constrained manufacturing environments. Furthermore, real-world inspection lines often present distinct challenges-such as environmental noise and strict latency requirements-that are not fully addr
