Lengthening temporalis myoplasty and lip reanimation.

The authors report a new surgical technique of myoplasty of the temporalis muscle applied to the treatment of permanent peripheral facial paralysis. It consists of a muscle lengthening, using the totality of the temporalis muscle and the transfer of its tendon attached to the coronoid process directly to the lips, therefore modifying the temporal fixed point and respecting the deep temporal pedicles. Ten cadaver dissections were performed for a better understanding of the muscle fiber redistribution during surgery to measure the distance between coronoid process and lip commissure and to measu

Surgical correction of foot deformities after stroke.

Of 104 patients with corrective surgery for foot deformities subsequent to a cerebrovascular accident from 1980 until 1983, 53 patients returned for clinical examination and 22 patients were evaluated by questionnaire and telephone interview. The average follow-up period was 6.4 years. The operative techniques were tenotomy of the toe flexors for hammer-toe deformity, lengthening of the aponeurosis of the gastrocnemius for equinus deformity, and transfer of the anterior tibial tendon or the posterior tibial tendon or the long toe flexors for varus deformity. In 74% of patients, correction was

Orthopaedic issues in the musculoskeletal care of adults with cerebral palsy.

AIMS: Orthopaedic care of adults with cerebral palsy (CP) has not been well documented in orthopaedic literature. This paper focuses on some … inovarus. The surgical correction of this is most often facilitated through a split anterior tibial tendon transfer. Posterior tibial transfers are rarely indicated. Residual equinus deformities contribute to a pes planus deformity. The split anterior tibial tendon transfer is usually combined with gastrocnemius-soleus recession and plantar release. Transfer of t …

Biceps rerouting and derotational radial osteotomy: A technique for correcting forearm supination in NBPBP.

INTRODUCTION: Forearm supination deformity is a recognized late sequela of neonatal brachial plexus birth palsy (NBPBP) that can impair functional positioning of the hand and limit activities requiring forearm pronation. Surgical options include forearm osteotomy, tendon rerouting, tendon transfer, and combined procedures. We describe a modified technique combining derotational radial osteotomy with biceps tendon rerouting using cortical button fixation. METHODS: This was a single-patient technical case report. An 18-year-old male with longstanding fixed forearm supination deformity secondary…

Facial reanimation: evolving from static procedures to free tissue transfer in head and neck surgery.

PURPOSE OF REVIEW: The purpose of this article is to review and evaluate the surgical options for treating patients with facial paralysis, c … eric nerve or contralateral facial nerve using a cross-face nerve graft. The orthodromic temporalis tendon transfer has minimal donor site morbidity and acceptable reported outcomes. Static procedures continue to be used alone and in combination with other paradigms for facial nerve reanimation.

Peroneus Longus overload caused by soft tissue deficiencies associated with early adult acquired flatfoot: A finite element analysis.

BACKGROUND: Peroneus Longus tendinopathy has been related to overload from cavus and ankle instability. The etiology of isolated Peroneus Longus tendon synovitis has not been elucidated. Loss of foot arch integrity as a cause of isolated Peroneus Longus overload is difficult to establish using cadaver modeling. Our objective was to analyze Peroneus Longus stress changes in pathological scenarios related to flatfoot development. METHODS: A three-dimensional finite element foot model which included the foot bones and main soft tissues that maintain the arch was used. Simulations were performed

Triceps to biceps transfer to restore elbow flexion in three patients with brachial plexus palsy.

Between April 1994 and April 1998, triceps to biceps transfers were done for three men with post-traumatic lesions of the brachial plexus and consequent loss of elbow flexion. Their mean age at the time of their accidents was 33 years (range 19-41) and at the time of muscle transfer 40 years (28-46), with a mean observation period of 21 months (12-31). The transfer resulted in active elbow flexion in all patients with a mean of 113 degrees (90 degrees-130 degrees) and a degree of strength 4-5 (contraction against resistance) with no remaining deficit of passive extension. Two patients were sat

Anterior transfer of the toe flexors for equinovarus deformity of the foot.

Anterior transfer of the long toe flexors was carried out for the treatment of spastic equinovarus foot deformity in both adults and children. Adults included those with hemiplegia subsequent to a stroke, spastic hemiplegia due to cerebral palsy and spastic spinal paraplegia. Most of the children had cerebral palsy. The transfer was indicated for an equinovarus foot with persistent activity of the toe flexors, which produced curling of the toes in the swing phase of the gait or a fixed hammer toe deformity. Fifty six patients were followed up for more than four years. In all cases correction o

Applying a hexapod circular frame to gradually correct persistent knee flexion due to congenital aplasia of the quadriceps-a case report and review of the literature.

Congenital aplasia or hypoplasia of the quadriceps muscle are rare conditions that significantly impair the extension of the knee joint, resulting in a sustained flexion deformity. These conditions alter overall gait quality, impede daily activities and require effective, timely intervention. Surgical techniques for correction have been described, including soft-tissue release, guided growth, tendon transfer and external fixators. Among the external fixators available, hexapod circular frames are probably the most effective because they enable the precise, progressive correction of complex mul…

Impact of Treadmill Running Intensity on Lubricin Levels in Rat Achilles Tendons.

PURPOSE OF THE STUDY: The Achilles tendon is crucial for transferring force from the calf muscles to the heel bone. Lubricin, a key glycoprotein, ensures smooth tendon gliding. This study investigated the effects of different treadmill running intensities on lubricin levels in rat Achilles tendons and explored the associated molecular mechanisms. MATERIAL AND METHODS: Eighteen rats were divided into three groups: Strenuous Treadmill Running (STR), Moderate Treadmill Running (MTR), and a sedentary Control group (CON). The running protocols were conducted over eight weeks. Post-intervention, te

The use of latissimus dorsi muscle flap in the aesthetical reconstruction of heat-press injury of the hand.

We present a successful case of aesthetic reconstruction utilizing free latissimus dorsi muscle flap transfer. A large quantity of skin of the dorsum of hand and finger was lost. The dorsum of the index, long and ring fingers was severely damaged, such that extensor tendons were necrotic and all digital bones and the second metatarsal bone were exposed with partial necrosis. In addition, the proximal interphalangeal joints (PIP) were also exposed. To cover exposed bones and the tendons of dorsum of the hand, a free latissimus dorsi muscle flap was transferred, and then meshed skin covered the
