Echographic evaluation of a patient with diabetes and dense vitreous hemorrhage: an avulsed retinal vessel may mimic a tractional retinal detachment.

PURPOSE: To report that an avulsed retinal vessel may appear as a tractional retinal detachment on echographic evaluation. METHODS: Case report. RESULTS: A 57-year-old diabetic woman presented with a nonclearing vitreous hemorrhage of 2 months duration in the left eye. Echography was consistent with a localized tractional retinal detachment on longitudinal sections; transverse sections demonstrated a pinpoint opacity in the vitreous cavity. Intraoperatively, an avulsed retinal vessel was noted in the area of echographic abnormality. CONCLUSION: An avulsed retinal vessel may mimic tractional…

Comparison of Acupuncture and Lumbar Traction for Lumbar Disc Herniation and Subsequent Risk of Lumbar Fusion or Discectomy: A Retrospective Cohort Study.

BACKGROUND: Lumbar disc herniation (LDH) is commonly treated with conservative therapies, but comparative evidence on acupuncture versus lumbar traction and subsequent surgery is limited. We examined whether acupuncture was associated with lower 5-year risks of discectomy or spinal fusion than traction among adults with LDH. METHODS: We conducted a retrospective cohort study using deidentified electronic health records from the TriNetX US Collaborative Network from January 1, 2000, to December 31, 2023. Adults aged 20 years or older with LDH who received acupuncture or traction after diagnosi…

Investigating the structural responses of medial collagen and elastin to traction to elucidate mechanical property differences in porcine aorta and pulmonary artery.

Along with the rising interest in regenerative medicine and tissue engineering comes the necessity to gain a deeper understanding of the underlying nature behind the mechanical responses of various soft tissues. This study investigates the differences in mechanical responses of the aorta and pulmonary artery, both of which share similar trilayered structures, by examining how their microstructural architectures influence their response to traction forces. As such, two-photon microscopy was used to visualize elastin and collagen fibers, via two-photon excited autofluorescence and second-harmoni…

Entire traction using clip-and-nylon ring to facilitate endoscopic submucosal dissection of a laterally spreading tumor with fibrosis in the rectum.

A 78-year-old woman with hematochezia underwent a colonoscopy and found a 2 × 2-cm laterally spreading tumor (LST) in the rectum, 3 cm from the anus. Because of the risk related to anus preservation and the potential operative trauma, the patient refused surgery and was referred for ESD treatment. Here, we applied a novel entire traction method to deal with this subset of tumors.

Pre-operative intravitreal bevacizumab for tractional retinal detachment secondary to proliferative diabetic retinopathy: the Alvaro Rodriguez lecture 2023.

The treatment of proliferative diabetic retinopathy (PDR) has evolved significantly since the initial use of panretinal photocoagulation as a treatment in the 1950s. Vascular endothelial growth factor inhibitors have provided an effective alternative without the risk of peripheral vision loss. Despite this, the risk of complications requiring surgical intervention in PDR remains high. Intravitreal bevacizumab has shown promise as a preoperative adjuvant to vitrectomy for PDR complications, albeit with a purported risk for tractional retinal detachment (TRD) progression in eyes with significant…

Progression of Central-peripheral Band and Traction Bronchiectasis Clusters Leading to Chronic Respiratory Failure in a Patient with Fibrotic Pulmonary Sarcoidosis.

We herein report a rare case of pulmonary sarcoidosis leading to chronic respiratory failure with restrictive ventilatory impairment during a 53-year-long observation period. Nine years after the histological diagnosis of stage I sarcoidosis on chest X-ray in a woman in her 20s, she developed bilateral reticular and granular opacities on chest computed tomography and was started on prednisone for 18 years. Seven years after prednisone withdrawal, these persisting opacities around the bronchovascular bundle, including a central-peripheral band, had progressed, forming traction bronchiectasis cl…
