Quetiapine, a second-generation antipsychotic originally used in the treatment of schizophrenia, was also found to be effective as monotherapy or as an adjunctive therapy for acute mania. Delirium in patients treated with quetiapine seems to be a rare phenomenon; however, we report two patients with bipolar disorder who developed delirium when prescribed quetiapine as an adjunct to valproate for acute mania. Controlled trials have demonstrated the efficacy of several classes of drugs for achieving acute response in bipolar mania and depression. For many years, clinical response has been the primary outcome in the majority of short-term efficacy studies. However, there is a growing consensus that the optimal goal in the long-term management of bipolar disorder is remission. The purpose of this article is to briefly summarize the clinical importance of remission in bipolar disorder and to review data on the effectiveness of available treatments for achieving and sustaining remission.

We have shown that plasma apolipoprotein E is significantly decreased in treatment-free subjects with schizophrenia spectrum and bipolar disorder but increases after treatment in bipolar disorder. Levels of apolipoprotein D were not changed by treatment. Hence changed apolipoprotein E could be involved in abnormalities in lipid homeostasis in some subjects with psychiatric diseases.
