Transconjunctival surgery provides excellent exposure of the inferior orbit, leaves no visible scar, and poses less risk of the complications typically associated with transcutaneous techniques. The transconjunctival approach can be combined with lateral canthal techniques to increase exposure and surgical options. We review applications of the transconjunctival approach in blepharoplasty, the repair of orbital floor fractures, socket reconstruction, inferior orbital tumor excision, and orbital decompression.

Experience with thirty-two patients with a low rectovaginal fistula with or without attendant sphincter damage is reported. The technic used is advancement of the anterior rectal wall with excision of the infected anal glandular tissue and repair of muscle tissue when indicated. Anorectal infection and childbirth injuries were the common causes. An acceptable recurrence rate was achieved. Colostomy was not used in this series.

Quadricuspid aortic valve is an uncommon congenital anomaly that often causes aortic regurgitation in adulthood, necessitating valve replacement. A 54-year-old woman presented with symptoms and signs of aortic regurgitation, but during surgery a quadricuspid aortic valve was identified. The abnormal cusp was fibrotic, short, thick, and fenestrated. Excision of the fourth cusp was performed and valve repair carried out successfully, with only minimal regurgitation.

A variety of surgical procedures exist for early repair of the nerve injury in obstetrical brachial plexus palsy, including neuroma excision and nerve grafting, neurolysis and neurotization. Secondary deformities of the shoulder, forearm, and hand can similarly be reconstructed using soft tissue and skeletal procedures. This review describes our surgical approach to maximize the ultimate functional outcome in infants and children with obstetrical brachial plexus palsy.

The level of sensory recovery was studied in 13 consecutive patients who had undergone lingual nerve repair after a delay of 7-32 (mean 16) months since the initial injury. In all patients the damaged segment of nerve was excised and the cut ends directly apposed by 6-10 (mean 7) epineurial sutures. The final outcome was assessed 12-24 months after repair. We review the literature and report a series of eight cases of balanitis xerotica obliterans (BXO) developing in patients following primary hypospadias repair. The ages of these patients ranged from 8 to 25 years with BXO developing from 1 to 16 years postoperatively. We have isolated a human excision repair gene ERCC5 which complements the defect of the mouse UV-sensitive mutant XL216 (rodent complementation group 5). Here we report cDNA cloning of human and mouse ERCC5 genes using an exon containing an ERCC5 fragment as a probe.
