Selective Facial Muscle Activation with Acute and Chronic Multichannel Cuff Electrode Implantation in a Feline Model.

OBJECTIVES: Facial paralysis is a debilitating condition with substantial functional and psychological consequences. This feline-model study evaluates whether facial muscles can be selectively activated in acute and chronic implantation of 16-channel multichannel cuff electrodes (MCE). METHODS: Two cats underwent acute terminal MCE implantation experiments, 2 underwent chronic MCE implantation in uninjured facial nerves (FN) and tested for 6 months, and 2 underwent chronic MCE implantation experiments after FN transection injury and tested for 3 months. The MCE were wrapped around the main tr

Aesthetic considerations in extirpation of melolabial lymphatic malformations in children.

Surgical treatment of lymphatic malformations in the nasolabial region of children has produced disappointing results. Attempts to remove them through a preauricular incision have generally required a synchronous superficial parotidectomy in an attempt to prevent facial nerve injury. However, incomplete removal with this approach is common, leading to multiple surgeries, infections, facial nerve injury, and when parotidectomy is performed, a lateral facial concavity. To avoid these complications and still extirpate the lymphatic malformation, an alternative direct external technique has been e

Preservation of the deep facial vein in reduction malarplasty.

PURPOSE: Many facial bone contouring surgeries, including reduction malarplasty, are performed in Asian countries to give the face an oval shape. When performing reduction malarplasty, a few large vessels can be injured, including the deep facial vein. The aim of this study was to determine the anatomy of the deep facial vein and to investigate the best method to prevent injury of deep facial vein during reduction malarplasty. PATIENTS AND METHODS: A retrospective chart review was performed for all patients who underwent reduction malarplasty with L-shaped osteotomy between March 2009 and Feb

Sensory recovery after infraorbital nerve avulsion injury.

The infraorbital nerve is a branch of the trigeminal nerve. Injury to the infraorbital nerve can be caused by trauma, including various facial fractures. Due to this nerve injury, patients complain of numbness and pain in the entire cheek, the ala of nose, and upper lip. In general, spontaneous sensory recovery is expected after decompressive surgery. If nerve transection is confirmed, however, neurorrhaphy is typically performed. Here, we present a case in which microsurgery was not performed in a patient with Sunderland grade V avulsion injury of the infraorbital nerve due to a facial bone f

Intraoperative monitoring of the facial nerve during parotid gland surgery in Otolaryngology services - Head and Neck Surgery.

INTRODUCTION: Facial nerve injury remains the most severe complication of parotid gland surgery. Due to the increasing evidence about the advantage of the use of intraoperative facial nerve monitoring, a survey was distributed among members of the Spanish Society of Otorhinolaryngology-Head and Neck Surgery with the objective of determining patterns of its use. MATERIAL AND METHODS: A questionnaire which included 12 separate questions in 3 sections was distributed via email through the official email of the Spanish Society of Otorhinolaryngology-Head and Neck Surgery. The first section of que

Methylene blue staining in the parotid surgery: Randomized trial, 144 patients.

OBJECTIVE: To investigate usefulness of the methylene blue staining for parotid gland surgery. STUDY DESIGN: Randomized, prospective controlled trial. METHODS: The study analyzed 144 surgical operations (2000-2014) for parotidectomy. The cases were randomly divided into two groups: Group 1 (n=70) for surgeries with methylene blue staining, and Group 2 (n=74) for surgeries without staining. Surgical complications like temporary facial weakness, permanent facial nerve injury, and Frey's syndrome were taken for comparison between groups as well as rate of tumor recurrence. RESULTS: Temporary f

Facial reanimation according to the postresection defect during lateral skull base surgery.

The vast majority of benign tumors of the cerebellopontine angle, temporal bone, and parotid gland can be successfully resected without permanent injury to the facial nerve. Malignant tumors or recurrent disease may require facial nerve sacrifice, especially if preoperative facial paresis is present. This article will present case examples of the various methods to reconstruct facial animation after lateral skull base resections that require sacrifice of cranial nerve VII, and the associated mimetic facial musculature. Facial mimetic outcome after reanimation was graded using the House-Brackma

Surgical Optimization of Motor Recovery in Face Transplantation.

BACKGROUND: Face transplantation (FT) has emerged as a viable option for treating devastating facial injuries. Most reported outcomes have demonstrated satisfactory motor and sensory restoration despite differences in technique. The authors have developed an algorithm of facial nerve management in these challenging patients. Our principles of management are illustrated by 2 specific patients. METHODS: A retrospective analysis of prospectively collected data on 2 full face transplants was performed. Both patients required nerve grafting during full FT. Patient 1 due to short donor facial nerve…

Traumatic carotid cavernous fistula complicated with intracerebral hemorrhage: case report.

A 45-year-old woman presented with a rare case of traumatic carotid cavernous fistula (CCF) complicated with intracerebral hemorrhage after injury in a car accident. She had multiple injuries including facial bone fracture and slight subarachnoid hemorrhage around the left sylvian fissure. Emergent plastic surgery for the facial deformity was performed. Next day, she suffered intracerebral hemorrhage in the left frontal lobe. Angiography revealed CCF, predominantly draining to the left superficial sylvian vein. The left internal carotid artery was occluded by endovascular treatment. The clinic

An impressive case of complete traumatic maxillofacial degloving.

By definition, degloving is skin and subcutaneous tissue detachment, most often affecting the limbs and extremities and occasionally the scalp. Degloving generally stems from high-energy trauma, but can also be intentional, such as in the case of planned surgical access in the anterior region of the mandible in oral-maxillofacial surgery. This paper describes an extreme case of complete traumatic maxillofacial degloving that caused partial avulsion of the soft tissues and maxilla. This is an extremely rare condition that has not previously been described in the literature, as the patient survi

Static and Motion Facial Analysis for Craniofacial Assessment and Diagnosing Diseases.

Deviation from a normal facial shape and symmetry can arise from numerous sources, including physical injury and congenital birth defects. Such abnormalities can have important aesthetic and functional consequences. Furthermore, in clinical genetics distinctive facial appearances are often associated with clinical or genetic diagnoses; the recognition of a characteristic facial appearance can substantially narrow the search space of potential diagnoses for the clinician. Unusual patterns of facial movement and expression can indicate disturbances to normal mechanical functioning or emotional a

Facial Paralysis Following Core Needle Biopsy of a Deep Lobe Parotid Tumor: A Case Report.

OBJECTIVES:: To report a case of total facial paralysis as the result of a core needle biopsy. METHODS:: Case report and literature review. RESULTS:: A 70-year-old man was diagnosed with a deep lobe parotid tumor on computed tomography. During a core needle biopsy, he developed complete facial paralysis. The cause was discovered to be a direct stab injury to the main trunk of the facial nerve. CONCLUSIONS:: When biopsying parotid lesions adjacent to the main trunk of the facial nerve, the use of smaller-gauge needles and additional patient counseling should be considered.

Treating facial nerve palsy by true termino-lateral hypoglossal-facial nerve anastomosis.

Hypoglossal-facial nerve anastomosis is a time-proven technique for the repair of facial nerve palsy. Efforts have been made to reduce hypoglossal nerve injury, the main drawback of the technique. In this study, the anastomosis is a true termino-lateral neurorrhaphy with only an epineural window in the hypoglossal nerve sheath. A re-routing technique of the temporal facial nerve is also performed to allow a direct anastomosis to the hypoglossal nerve without the need for a jump graft. The first three results reported are very encouraging, with a satisfactory return of facial mimics and without

Anomalous facial nerve: the otologic surgeons' abyss.

Iatrogenic injury to the facial nerve is certainly the most apparent if not the most feared complication faced by the otologic surgeon. The temporal bone with its tightly packed sensory structures is no place for the "occasional operator". A thorough knowledge of temporal bone anatomy is of course required. On occasion, as in other areas of life, one encounters the unexpected and must be prepared to "adjust" to the situation at hand. An instructional case involving an anomalous facial nerve is presented. A review of the developmental anatomy of the facial nerve, preoperative diagnostic imaging

Malformation in the primary and permanent dentitions following trauma prior to tooth eruption: a case report.

Dento-facial injuries that occur prior to the eruption of teeth in the primary dentition are rare, but can result in damage to the primary dentition. We report a rare case where an injury to the anterior maxilla and mandible of an infant prior to primary tooth eruption resulted in hypoplasia, displacement and impaction of the primary dentition and damage to a developing permanent tooth.

Traumatic Facial Diplegia and Horner Syndrome: Case Report.

Traumatic facial diplegia is an uncommon pathology, and is usually associated with bitemporal bone fractures. Traumatic Horner syndrome is mostly associated with carotid artery dissection. We present a case with traumatic facial diplegia and a unilateral Horner syndrome where the mechanisms of injury were unusual. The patient had developed his neurological deficits 9 days following trauma. We discuss the mechanisms of the facial palsy and Horner syndrome and the importance of their diagnosis.

Radiology of maxillofacial gunshot injuries.

The relative advantages and disadvantages of conventional facial radiographic techniques, facial tomography, and carotid arteriography for gunshot injury cases are discussed. The conventional series of skull, facial, or cervical spine projections forms a base line upon which the more complex procedures may be added. Five cases illustrate the radiographic principles involved in the determination of gunshot fragment localization, bone fracture, soft-tissue swelling, and maxillary sinus involvement.

Major congenital ear malformations: surgical management and results.

Fifteen patients with major congenital aural atresia underwent operations using an anterior (trans-atretic bone) approach. Facial nerve monitoring was used in all cases and there were no instances of facial nerve injury. Postoperatively, two thirds of the patients had speech reception thresholds of 30 dB or better; the air conduction threshold (averaged for 500, 1,000, and 2,000 Hz) improved at least 25 dB in 80% of the patients and at least 30 dB in 60% of the patients (follow-up, 10 to 29 months). Minor revision surgery was necessary in three patients. On the basis of this series, several co
