Perioperative anesthetic management of patients with hypoplastic left heart syndrome undergoing the comprehensive stage II surgery-A review of 148 cases.

BACKGROUND: Patients with hypoplastic left heart syndrome undergo the comprehensive stage 2 procedure as the second stage in the hybrid approach toward Fontan circulation. The complexity of comprehensive stage 2 procedure is considered a potential limitation, and limited information is available on its anesthetic management. This study aims to address this gap. METHODS: A single-center retrospective cohort study analyzed 148 HLHS patients who underwent comprehensive stage 2 procedure, divided into Group A (stable condition, n = 116) and Group B (requiring preoperative intravenous inotropic th

Right ventricle-to-pulmonary artery shunt versus modified Blalock-Taussig shunt in the Norwood procedure for hypoplastic left heart syndrome - influence on early and late haemodynamic status.

OBJECTIVE: The aim of this study was to assess changes in early and late haemodynamic status after the Norwood procedure (NP), caused by the implementation of right ventricle-to-pulmonary artery shunt (RV-PA). METHODS: A consecutive series of 68 children with hypoplastic left heart syndrome underwent NP: Group 1 (n=31) with the application of a modified Blalock-Taussig shunt and Group 2 (n=37) with RV-PA. Haemodynamic data from the early postoperative period (72 h after the operation) and cardiac catheterisation data, as well as blood tests before the hemi-Fontan procedure (HF) were analysed.…

Is alternative cardiac surgery an option in adults with congenital heart disease referred for thoracic organ transplantation?

OBJECTIVES: We analysed the outcomes of adults with congenital heart disease (ACHD) referred for thoracic organ transplantation who underwent non-transplant cardiac surgery as an alternative management option. METHODS: Adult patients with congenital heart disease assessed for heart or heart-lung transplant were identified from the departmental database. A retrospective analysis of the medical records, transplant assessment data and surgical notes was carried out. RESULTS: One hundred and twenty-six patients were assessed between January 2000 and July 2011. Non-transplant cardiac surgery was

Selection of revision technique for suboptimal total cavopulmonary connection based on computational fluid dynamics: a case report.

BACKGROUND: Unequal pulmonary blood flow in Fontan circulation is a known contributor to development of pulmonary arteriovenous fistula (PAVF). However, optimal revision strategies remain debated, especially in patients with a pedicled autologous pericardial conduit. CASE PRESENTATION: A 23-year-old woman with a history of pulmonary atresia and previous total cavopulmonary connection (TCPC) using a pedicled pericardial extracardiac lateral tunnel developed hemoptysis and cyanosis due to left-sided PAVF. Cardiac catheterization, 3-dimensional computed tomography and 4-dimensional flow magnetic

Plastic bronchitis due to adenoviral infection: a case report.

BACKGROUND: Plastic bronchitis (PB) frequently occurs as a serious postoperative complication of the Fontan procedure. The definitive causes of PB are unknown. CASE PRESENTATION: Herein, we report a pediatric case of PB secondary to adenoviral infection. A 4-year-old girl was admitted to the general pediatric ward for cough since 2 weeks and fever since 11 days. Consolidated lesions were noted in the right upper and both lower lung lobes. Extracorporeal membrane oxygenation was performed because the patient's respiratory failure remained unalleviated despite the use of a ventilator. Bronchial…

Regurgitation through the morphologically pulmonary valve after additional aortopulmonary anastomosis.

Although additional aortopulmonary anastomosis established by creating double outlet from the systemic ventricle and anastomosing the pulmonary trunk to the ascending aorta can undoubtedly provide an unobstructed outlet for the systemic ventricle, postoperative regurgitation of the morphologically pulmonary valve, if it occurs, may spoil the efficacy. To determine whether the pulmonary valve can be regurgitant, 11 patients undergoing this surgical option, six accompanying a Fontan-type operation and five associated with biventricular repair, were reviewed. Postoperative regurgitation was obser

[Assessment of atrial pathologies in children using transesophageal echocardiography].

OBJECTIVE: Transesophageal echocardiography (TEE) is indicated for suspected atrial septal pathology and for monitoring of interventional procedures such as an atrial septal defect (ASD) closure during cardiac catheterization. Transesophageal echocardiography also helps to demonstrate postoperative complications and residual defects of complex congenital cardiac anomalies. METHODS: Transesophageal echocardiography was performed in 112 pediatric patients with or suspected atrial pathology at our institution between 1999-2002, using the standard techniques. The mean age was 8.7+/-4.2 years. RE

Pediatric interventional cardiology: the cardiologist's role and relationship with pediatric cardiothoracic surgery.

The advances in pediatric interventional cardiac catheterization have changed the therapeutic strategy for many patients with CHD. The procedure of choice for valvar stenosis, recoarctation, collateral vessel occlusion, and branch PA stenosis has moved from the operating room to the catheterization laboratory. Effective and safe transcatheter interventions now exist for closure of ASDs, VSDs, and PDAs and are considered viable alternatives to surgical closure. Other interventional catheterization procedures are currently being investigated to complement the surgical management of patients with

3D Electroanatomical Mapping for Complex Atrial Arrhythmias in Adult Congenital Heart Disease-A Single Center Experience.

INTRODUCTION: Adult Congenital Heart Disease (ACHD) patients suffer from multiple, and often atypical, atrial arrhythmias. 3D electro-anatomical mapping systems assist in the identification and ablation of arrhythmogenic substrates critical to the initiating of theses arrhythmias. METHODS: We report on patients with ACHD and atrial arrhythmias treated at our center using the high density Rhythmia (Boston Scientific. Marlborough, MA) mapping system. Procedural success was defined as no sustained arrhythmia inducible at the end of the procedure. Partial success was defined as non-inducibility o

Main branches pulmonary artery stenting in congenital heart diseases: a case series.

BACKGROUND: Branch pulmonary artery (PA) stenosis is a common complication in congenital heart disease (CHD) that can result in unequal pulmonary perfusion, cyanosis, and increased ventricular workload. Although surgical repair remains an option, pulmonary artery stenting has emerged as a less invasive and effective alternative for restoring vessel patency. METHODS: This single-center retrospective study included 18 pediatric patients (median age 5.4 years, range 3 months-17 years) who underwent PA stenting for branch stenosis associated with complex CHD. Patient demographics, procedural char

Extracorporeal Membrane Oxygenation in the Interstage Period After Norwood and Related Stage I Palliations in Infants With Single Ventricle: A Systematic Review of Incidence, Risk Factors, and Outcomes.

BACKGROUND: Neonates with congenital univentricular conditions require staged palliation culminating in a Fontan circulation to support survival. The first-stage Norwood procedure carries substantial risk of myocardial dysfunction, frequently necessitating extracorporeal membrane oxygenation (ECMO) support. The aim of this systematic review was to examine the incidence, risk factors, and outcomes of post-Norwood ECMO use during the interstage period. METHODS: We systematically searched MEDLINE, Embase, Cochrane Library, and ClinicalTrials.gov for studies published between January 1990 and Mar

Transcatheter creation of bidirectional cavopulmonary connections by needle punctures in two patients.

We report on two patients who received a transcatheter cavopulmonary connection by a needle puncture under deep conscious sedation. In both patients, the vessel-to-vessel connection was achieved by a venous access into the superior caval vein and direct needle puncture of the pulmonary artery. The two cavopulmonary anastomoses were held open by a covered stent and a bare-metal stent, respectively.

Neo-aortic insufficiency late after staged reconstruction for hypoplastic left heart syndrome: impact of differences in initial palliative procedures.

The neo-aortic insufficiency in patients with hypoplastic left heart syndrome is an important sequela. We assessed the risks of the neo-aortic valve deterioration by the difference of initial palliations: Group I underwent primary Norwood (Nw) with systemic-to-pulmonary artery shunt (SPS), Group II underwent bilateral pulmonary artery banding (bPAB) and subsequent Nw with SPS (bPAB-Nw/SPS), Group III underwent bPAB and subsequent Nw with bidirectional Glenn (BDG) procedure (bPAB-Nw/BDG). The neo-aortic valve z score changes over time did not reach statistical significance in all groups (p = 0.

Outcome of Tricuspid Valve Plasty in Norwood Stage I Operation.

BACKGROUND: Significant tricuspid valve regurgitation (TR) is considered a poor prognosis factor for patients with hypoplastic left heart syndrome (HLHS). Performing a tricuspid valve plasty (TVP) during Norwood Stage I palliation (S1P) has rarely been reported. We report mid-term results of performing TVP during Norwood S1P. METHODS AND RESULTS: Between December 2004 and July 2013, 48 patients with HLHS or variants underwent Norwood S1P and of them 11 (23%) with TR of a moderate degree or above underwent TVP. The estimated 1- and 5-year survival rates for Norwood S1P were 61% and 54%, respec

Systemic venous collateral channel causing desaturation after bidirectional cavopulmonary anastomosis: percutaneous closure.

An infant with cyanotic cardiac disease that was palliated with a bidirectional cavopulmonary shunt developed progressive and worsening cyanosis 5 years after the surgical procedure. A large venous collateral was found to be decompressing the bidirectional Glenn shunt from the superior caval vein to the inferior caval vein and was percutaneously closed with a vascular plug. The unusually large venous collateral, and the excellent outcome associated with percutaneous procedure are discussed.

Outcome of the Norwood operation in patients with hypoplastic left heart syndrome: a 12-year single-center survey.

OBJECTIVE: Recent advances in perioperative care have led to a decrease in mortality of children with hypoplastic left heart syndrome undergoing the Norwood operation. This study aimed to evaluate the outcome of the Norwood operation in a single center over 12 years and to identify clinical and anatomic risk factors for adverse early and longer term outcome. METHODS: Full data on all 157 patients treated between 1996 and 2007 were analyzed. RESULTS: Thirty-day mortality of the Norwood operation decreased from 21% in the first 3 years to 2.5% in the last 3 years. The estimated exponentially w
