Transanal protrusion of ventriculoperitoneal shunt reflecting asymptomatic perforation of the large bowel.

Perforation into the gastrointestinal tract is a rare complication of ventriculoperitoneal shunt insertion. We present a case of transanal protrusion of the shunt catheter in an otherwise asymptomatic patient, with only transient signs of shunt failure some 2 months prior to presentation, and discuss treatment options to rationalise our decision to treat with laparotomy and preservation of the shunt.

Extrusion of a cochlear implant positioner through the tympanic membrane in a pediatric patient: management of a delayed complication.

OBJECTIVE: This study aimed to present a case of an extruded Advanced Bionics positioner, with the goal of educating a new generation of otologic surgeons on management of the delayed complications associated with this cochlear implant model. METHODS: Retrospective case report in a pediatric patient. RESULTS: We observed the extrusion of an Advanced Bionics positioner 10 years after cochlear implantation. CONCLUSION: Awareness of the Advanced Bionics positioner as a component of certain historical models of cochlear implants is important as the delayed complications associated with this mod

[Defecation after colic transmural migration of a gauze pad left behind during abdomino-pelvic surgery].

We report a case of complete transmural migration through the colic wall, followed by spontaneous defecation, of a surgical sponge left behind in the abdominal cavity (gossypiboma) during a vaginal hysterectomy with oophorectomy. The process results from encapsulation of the surgical sponge with fistulization through the colic wall. No functional damage followed this defecation because either the fistula closed early, or the sponge remained temporarily entrapped in a colic fold. The diagnosis was based on echography and computed tomography.

A rare cause of acute appendicitis: an ingested foreign body.

Various kinds of foreign bodies causing appendicitis have been reported. However, a needle contained in the appendix is very rare, especially in adults. We report an unusual case of a twenty year old man who had ingested a needle ten days prior and presented with signs and symptoms of acute abdominal pain. An abdominal computed tomography scan revealed acute appendicitis. The patient was successfully treated with laparoscopic surgical intervention. In cases of a foreign body in the gastrointestinal tract that cannot be removed endoscopically, the emergence of non-specific symptoms may be early

Consequences of long term indwelling transmural stents in patients with walled off pancreatic necrosis & disconnected pancreatic duct syndrome.

INTRODUCTION: There is limited data on the long term consequences and safety profile of long term indwelling transmural stents after successful treatment of walled off pancreatic necrosis (WOPN). AIM: To retrospectively evaluate consequences of long term indwelling transmural stents in patients with WOPN. METHODS: The records of patients who underwent endoscopic transmural drainage of WOPN and had disconnected pancreatic duct syndrome (DPDS) were analyzed. RESULTS: Thirty patients (26 M; mean age 37.1 ± 7.8 years) with long term indwelling transmural stents and DPDS were followed up for a m

Migrated Essure permanent birth control device: sonographic findings.

We report a case of a migrated Essure permanent birth control device. The correct diagnosis was made on conventional two-dimensional and three-dimensional pelvic sonography 7 years after placement of the device when the patient presented with persistent right-sided pain. The 3-month post placement hysterosalpingogram had shown an appropriately occluded right fallopian tube but had overlooked the abnormal position of the right Essure device, which was too proximal and extending slightly in the uterine cavity.

Endovascular repositioning of a pipeline embolization device dislocated from the vertebral into the basilar artery using a stent-in-stent technique. Practical and technical considerations.

PURPOSE: Stent dislocation is a rarely encountered problem in interventional neuroradiology. This article describes the repositioning of a pipeline embolization device (PED) dislocated from the vertebral artery (VA) into the basilar artery (BA) using a stent-in-stent technique. Based on this case additional in vitro measurements were performed. METHODS: In a patient, a larger PED (4.0 × 20 mm) was partially opened in a PED (3.0 × 20 mm) floating freely within the distal BA. The microcatheter with the partially opened stent was pulled back hereby pulling back the stent-in-stent construct into

Clinical outcomes of palliative self-expanding metallic stents in patients with malignant colorectal obstruction.

OBJECTIVE: Self-expanding metallic stents (SEMS) are useful palliative option and a bridge to surgery in malignant colorectal obstruction. The aim of this study was to evaluate the clinical outcomes of SEMS to palliate colorectal malignant obstruction. METHODS: Malignant colorectal obstructive patients who underwent SEMS insertion at the National Cancer Center, Korea from January 2004 to June 2008 were enrolled in the study. Patients' clinical characteristics, outcomes and complications for palliative SEMS insertion were retrospectively analyzed. RESULTS: A total of 54 patients were enrolled

Measured tube technique for ensuring the correct length of slippery artificial chordae in mitral valvuloplasty.

Mitral valvuloplasty using Gore-Tex (W.L. Gore & Associates, Inc, Flagstaff, AZ) as artificial chordae is often associated with difficulties in determining the length of the artificial chordae, as well as preventing knot slippage, especially for patients with broad anterior leaflet prolapse. We describe a simple technique that enables surgeons to easily determine the correct length of the artificial chordae and tie slippery knots without using a specific device.

Fracture of the talus as a complication of subtalar arthroereisis.

Subtalar joint arthroereisis remains a popular procedure for a flexible flatfoot deformity. Potential complications of the procedure have been discussed in published reports and are often believed to have resulted from shortcomings related to the mechanical properties of the biomaterial, implant size, and/or implant placement. In the present report, we describe the case of a talar neck fracture with migration of the implant after subtalar joint arthroereisis performed 10 years earlier. The 19-year-old patient was treated with implant removal and open reduction internal fixation and bone void f

[The grown-through ring finger].

After performing a right hemicolectomy due to cancer of the colon on a 67-year-old female patient, we detected an ingrown ring on her right ring finger. This had continuously penetrated the proximal phalanx from the flexor side after a trauma 3 years previously. Clinically, a residual flexion function was maintained. In the imaging the bone was found to be eroded, the digital arteries shifted, but the continuity of the deep flexor tendon was preserved.

Stent migration during transcatheter management of coarctation of aorta.

A 13-year-old girl underwent endovascular stent placement for coarctation of aorta. The fully expanded stent migrated to ascending aorta which could be stabilized, recrimped, and repositioned with a 20-mm goose neck snare. Postdilatation was performed from the left brachial route resulting in a good outcome.
