Tracheal bronchus is a congenital anatomical variant of patients with accessory bronchus in the right upper lobe deriving directly from the supracarinal trachea. Pre-operative consultation with the anesthesiologist is important for patient safety and can avoid adverse effects induced by endotracheal intubation. In this report, we described a case of tracheal bronchus in which general anesthesia was performed for video-assisted thoracoscopic surgery. We discussed some of the issues surrounding to complications in tracheal bronchus.

The sixteenth century saw a shift in perceptions of the scope of surgery. The medieval focus on elevating the status of surgery had been accompanied by a certain distancing of surgery from manual operations, but the medical humanism of the sixteenth century embraced manual skills as an important part of medicine, most noticeably in the case of anatomy. In the first part of this paper I use accounts of the treatment of ulcers as a way of exploring these changes in perceptions. Ulcers were a well-known surgical ailment in medieval medicine, but in the sixteenth century they were also associated

Upper airway endoscopy in children is a high-risk procedure, which should only be performed by a well trained and fully equipped team. The sequence of procedures, at least at the beginning of the procedure, is always the same and must be performed very rigorously. Communication between the various operators, especially the surgeon, the anaesthetist and the operating room nurse, is essential before starting the procedure, as anticipation of sudden difficulties is the key to effective management. The authors report their experience and endoscopy practices.

Peculiarities of endobronchial intubation and guaranteeing the monopulmonary ventilation parameters in videothoracoscopic interventions were studied up. Various methods of pulmonary collapse achievement were adduced. There were suggested the main principles of medicinal anesthesiological support as well as the perspectives for conditions improving for surgical comfort and the anesthesia optimization.

The traditional training of surgeons focused exclusively on developing knowledge, clinical expertise, and technical (surgical) skills. However, analyses of the reasons for adverse events in surgery have revealed that many underlying causes originate from behavioural or non-technical aspects of performance (eg, poor communication among members of the surgical team) rather than from a lack of surgical (ie, technical) skills. Therefore, technical skills appear to be necessary but not sufficient to ensure patient safety. Paying attention to non-technical skills, such as team working, leadership, s

BACKGROUND: Fresh frozen human cadavers have been used at the Clinical Training and Evaluation Centre, The University of Western Australia, Perth, WA for years and are an excellent model for surgical dissection thanks to their representative tissue quality. Differentiation between artery and vein can be difficult as both collapse post mortem. A historical technique was therefore refined to increase arterial rigidity using gelatine prior to freezing.
METHODS: Two fresh human cadavers were selected after ethical approval. Gelatine was infused into the carotid artery in one, and into the common

OBJECTIVE: To evaluate the impact of surgeon volume on the risk of technical surgical complications after esophageal cancer resection.
SUMMARY BACKGROUND DATA: Severe postoperative complications occur in about half of esophagectomized patients. Of these, technical complications might be particular targets for improvement. Population-based studies of the influence of surgeon volume on technical complications are sparse.
METHODS: A prospective, nationwide, population-based study was conducted in Sweden in 2001 through 2005. Details concerning patient and tumor characteristics, surgical procedu

BACKGROUND: The use of animate training laboratories have been touted as an important part of a surgical resident's training. This study determines if there was any benefit in resident performance and whether that benefit persisted.
METHODS: Twelve senior surgical residents attended a course in advanced laparoscopy with didactic and laboratory components. The residents' skills were tested by having them perform a laparoscopic fundoplication before, immediately after, and 6 months after the course. The procedure was videotaped, and divided into stages that were timed and scored by a single, ma

Oesophageal atresia management has evolved alongside the development of paediatric surgery. An analysis of a 30-year prospective collection of oesophageal atresia cases treated at the Royal Alexandra Hospital for Children, NSW, Australia is presented. There has been a dramatic change in the surgical approach. Fewer infants are undergoing operations of cervical oesophagostomy, gastrostomy and oesophageal replacement. More, including fragile infants, are surviving with a repaired native oesophagus. Fewer infants are suffering morbidity secondary to anastomotic leak. The progression in treatment

The surgical separation of a pair of conjoined twins in the year 1689 by Johannes Fatio was the subject of a recent article in this journal. The reference used as publication of the case was Fatio's book, Der Arzney Doctor, Helvetisch-Vernuftige Wehe-Mutter, published in 1752, although the case was presented in literature by three earlier sources. Two articles were published in the Miscellanea Curiosa sive Ephemeridum Medico-Physicarum Germanicarum Academiae Imperialis Leopoldinae Naturae Curiosorum in 1689 by Emanuel Konig and another in 1690 by Theodor Zwinger who described and illustrated t

The prominent tenets of limb's wound surgery have been improved during the Great War. Before 1914, traditional treatment of war injuries was usually applied on the ground because injuries caused by firearms were supposedly trivial while the threatening infection refrained surgeons from acting. From the very start of 1914, gangrene and septicaemia were the terrible consequences of new types of injuries provoked by shrapnel, all the more dangerous that the wounded men were numerous and the injuries were nursed after a too important delay. New surgical facilities have been organised by the French

Endoscopic laryngoplasty used in reconstructive surgery of the upper respiratory tracts for paralytic laryngeal stenoses is maximally sparing in relation to laryngeal structures. Mastering of the skill of the endoscopic intervention necessitated application of the device for training surgeons. The device simulates the operative situation of microsurgical manipulations on the larynx.

OBJECTIVE: One purpose of this prospective investigation was to assess the frequency of glove perforations and subsequent blood contact associated with selected obstetric procedures. The second purpose was to assess the relative risk of perforation among different members of the surgical team and determine if time of day or urgency of the procedure affected the frequency of perforation.
STUDY DESIGN: Over a 3-month period, obstetric personnel were asked to double glove for all surgical procedures. After surgery, they placed their gloves in plastic bags and noted the type of procedure, time of
