Towards a Better Understanding of Genotype-Phenotype Correlations and Therapeutic Targets for Cardiocutaneous Genes: The Importance of Functional Studies above Prediction.

Genetic variants in gene-encoding proteins involved in cell−cell connecting structures, such as desmosomes and gap junctions, may cause a skin and/or cardiac phenotype, of which the combination is called cardiocutaneous syndrome. The cardiac phenotype is characterized by cardiomyopathy and/or arrhythmias, while the skin particularly displays phenotypes such as keratoderma, hair abnormalities and skin fragility. The reported variants associated with cardiocutaneous syndrome, in genes DSP, JUP, DSC2, KLHL24, GJA1, are classified by interpretation guidelines from the American College of Medical G

The wind of change in the management of autosomal dominant polycystic kidney disease in childhood.

Significant progress has been made in understanding the genetic basis of autosomal dominant polycystic kidney disease (ADPKD), quantifying disease manifestations in children, exploring very-early onset ADPKD as well as pharmacological delay of disease progression in adults. At least 20% of children with ADPKD have relevant, yet mainly asymptomatic disease manifestations such as hypertension or proteinuria (in line with findings in adults with ADPKD, where hypertension and cardiovascular damage precede decline in kidney function). We propose an algorithm for work-up and management based on curr

Splicing impact of deep exonic missense variants in CAPN3 explored systematically by minigene functional assay.

Improving the accuracy of variant interpretation during diagnostic sequencing is a major goal for genomic medicine. To explore an often-overlooked splicing effect of missense variants, we developed the functional assay ("minigene") for the majority of exons of CAPN3, the gene responsible for limb girdle muscular dystrophy. By systematically screening 21 missense variants distributed along the gene, we found that eight clinically relevant missense variants located at a certain distance from the exon-intron borders (deep exonic missense variants) disrupted normal splicing of CAPN3 exons. Several

Phenotype analysis impacts testing strategy in patients with Currarino syndrome.

Currarino syndrome (OMIM 175450) presents with sacral, anorectal, and intraspinal anomalies and presacral meningocele or teratoma. Autosomal dominant loss-of-function mutations in the MNX1 gene cause nearly all familial and 30% of sporadic cases. Less frequently, a complex phenotype of Currarino syndrome can be caused by microdeletions of 7q containing MNX1. Here, we report one familial and three sporadic cases of Currarino syndrome. To determine the most efficient genetic testing approach for these patients, we have compared results from MNX1 sequencing, chromosomal microarray, and performed

Perspectives of gene expression profiling for diagnosis and therapy in haematological malignancies.

Considering the heterogeneity of leukaemias and the widening spectrum of therapeutic strategies, novel diagnostic methods are urgently needed for haematological malignancies. For a decade, gene expression profiling (GEP) has been applied in leukaemia research. Thus, various studies demonstrated worldwide that the majority of genetically defined leukaemia subtypes are accurately predictable by GEP, for example, with respect to reciprocal rearrangements in acute myeloid leukaemia (AML). Moreover, novel prognostically relevant gene classifiers were developed as, for example, in normal karyotype A

Single-probe Pd-modified ZnO-polymer fluorescent sensor array for discrimination of thymidine analogues in aqueous media.

Thymidine analogues are widely used as antiviral and anticancer agents; however, improper dosage and prolonged exposure can lead to severe hematological and genetic complications, necessitating reliable methods for their detection and discrimination. We report an array-based fluorescence sensing platform for the discrimination of structurally related thymidine analogues in aqueous media using a Pd-modified ZnO-polymer nanocomposite as a single cross-reactive probe. The nanocomposite was synthesized by functionalizing ZnO nanoparticles with a KG3 polymer matrix followed by palladium ion modific

Updates in hereditary angioedema and chronic spontaneous urticaria.

Hereditary angioedema (HAE) is a genetic disorder caused by either deficiency of C1 esterase inhibitor (85% of cases) or normal to high levels of nonfunctional C1 esterase inhibitor. Both defects lead to dysregulation of the kallikrein pathway, unregulated bradykinin production, and episodic swelling involving the skin, abdomen, and, in up to 50% of patients, the airway sometime during their life-span. A newer phenotype, HAE with normal C1 inhibitor (HAE type III), has also been recognized, although its mechanisms remain incompletely defined and diagnosis is largely based on consensus algorith

From inflammation to carcinogenesis: Distinct pathways and clinical implications of IBD-associated colorectal cancer compared with sporadic CRC.

Inflammatory bowel disease-associated colorectal cancer (IBD-CRC) represents a distinct clinical and molecular entity compared with sporadic colorectal cancer (CRC). While sporadic CRC arises through the adenoma-carcinoma sequence, IBD-CRC follows an inflammation-dysplasia-carcinoma pathway, characterized by early TP53 alterations, multifocality, and flat lesions that challenge detection. Contemporary epidemiology indicates declining IBD-CRC incidence in high-income regions due to improved surveillance, though risk remains elevated in subgroups with long-standing colitis, primary sclerosing ch
