Genetic Risk Factors and Clinical Implications of Glaucoma in the Saudi Population: A Review.

Most glaucoma genetic data derive from European and East Asian cohorts, leaving high-consanguinity Middle Eastern populations under-characterized. This review synthesizes 33 Saudi-specific genetic studies (2014-2024, >9000 participants) to define a population-level glaucoma genetic architecture that diverges substantially from global models and carries direct precision medicine implications. Three findings distinguish the Saudi landscape. First, CYP1B1 functions as the dominant causal gene across both primary congenital glaucoma (PCG) and juvenile-onset open-angle glaucoma (JOAG), accounting f

Prevalence of zonulopathy in primary angle closure disease.

BACKGROUND: To determine the prevalence of zonulopathy in a large cohort of eyes with primary angle closure disease (PACD) that underwent cataract surgery. METHODS: Retrospective consecutive case series of PACD eyes (including primary angle closure suspect, primary angle closure, and primary angle closure glaucoma) that underwent phacoemulsification cataract surgery or clear lens extraction between 2009 and 2020 at a single ophthalmology centre. Those with risk factors for zonulopathy such as history of trauma, pseudoexfoliation syndrome, intraocular surgery, retinitis pigmentosa or connectiv

Lensectomy as treatment for refractory or progressive retinopathy of prematurity narrow-angle glaucoma.

We present the case of a patient with a history of laser-treated retinopathy of prematurity (ROP) who developed narrow angles and intermittent angle closure. Despite laser peripheral iridotomy/iridoplasty, 1 year later, the patient had recurrent narrowing that resolved following clear lens extraction with intraocular lens placement. This case highlights the importance of continued monitoring for narrow angles in patients with ROP history.

Imaging iridotrabecular contact in angle closure.

BACKGROUND: With ultrasound biomicroscopy (UBM), radial scans are taken by using a typical ciliary process, to show the details of the iridocorneal angle, the anterior surface of the ciliary body, and its relation to the posterior iris. Appositional closure represents potentially reversible contact between the peripheral iris and trabecular meshwork. The appositional closure can further be classified according to the configuration of iridotrabecular contact (ITC). UBM can be performed in dark and light conditions, which has been shown to be useful for detecting changes in iridocorneal angle co

Factors defining the intraocular pressure-lowering effect of phacoemulsification and laser peripheral iridotomy in patients with primary angle closure : Factors defining IOP lowering effect of PE and LPI in PAC.

To identify whether gonioscopic ciliary body visibility and factors including anterior chamber depth (ACD), axial length (AL), acute attack history, and preoperative intraocular pressure (IOP) had any effect on IOP decrease in primary angle closure (PAC) and primary angle-closure glaucoma (PACG) after phacoemulsification (PE) and/or laser peripheral iridotomy (LPI). This retrospective-design study was conducted with 81 eyes with PAC and PACG; 33 eyes whose IOP was controlled with LPI and PE constituted group 1, and 48 eyes whose IOP was controlled using LPI alone comprised group 2. The effects

Angle-closure assessment in anterior segment OCT images via deep learning.

Precise characterization and analysis of anterior chamber angle (ACA) are of great importance in facilitating clinical examination and diagnosis of angle-closure disease. Currently, the gold standard for diagnostic angle assessment is observation of ACA by gonioscopy. However, gonioscopy requires direct contact between the gonioscope and patients' eye, which is uncomfortable for patients and may deform the ACA, leading to false results. To this end, in this paper, we explore a potential way for grading ACAs into open-, appositional- and synechial angles by Anterior Segment Optical Coherence To

Multimodal imaging of indapamide-induced bilateral choroidal effusion: a case report.

BACKGROUND: Indapamide, a sulfonamide diuretic used to treat hypertension, has been reported to have ocular side effects of acute angle-closure glaucoma, transient myopia and choroidal effusion whose immediate etiology is uncertain. This report aims to clarify the nature of indapamide-induced edema of the entire eyeball using multimodal imaging. CASE PRESENTATION: A 60-year-old woman who was following a long-term carbohydrate-restricted diet and receiving oral treatment for hypertension was referred to our department for eye pain. Indapamide (1 mg daily) was prescribed for uncontrolled hypert

Ocular manifestations of pulmonary hypertension.

Pulmonary hypertension, if left untreated, may result in increasing cardiac back pressures and lead to right heart failure and death. An increase in venous pressure in cases of pulmonary hypertension influences other organs. Ocular complications occur as a result of elevated venous pressure in the superior vena cava and in the ophthalmic veins, which cause dilation of the ocular veins, resulting in congestion of the choroid and leading to complications such as ciliary detachment, central retinal vein occlusion, acute serous retinal detachment, macular edema, retinal neovascularization, choroid

Intravitreal luxated lens stuck on the optic disc: a case report.

BACKGROUND: We encountered a rare patient with lens luxation in which the lens had become stuck on the optic disc. Findings obtained during vitreous surgery suggested that the luxated lens had become stuck on the optic disc via residual vitreous gel. CASE PRESENTATION: An 88-year-old Japanese man experienced lens luxation into the inferior vitreous cavity while undergoing treatment for glaucoma in his left eye. Because no inflammation was present upon examination, we observed the patient without prescribing any additional medications except for the eye drop treatment for glaucoma. Two years l

Efficacy of Penetrating Canaloplasty versus Trabeculectomy in Patients with Bilateral Primary Glaucoma: A Self-Control Study.

INTRODUCTION: Schlemm's canal-based minimally invasive glaucoma surgeries (MIGSs) are gaining increasing recognition and acceptance, but majority of these MIGSs are recommended in eyes with open angle. Bleb-independent penetrating canaloplasty (PCP) restores physiological aqueous outflow in eyes with both open and closed angles. We aimed to compare the efficacy, safety, and postoperative management of PCP versus trabeculectomy (TRAB) in patients with bilateral primary glaucoma. METHODS: This study is prospective self-control case series in a tertiary hospital in China. A total of 54 eyes from

The effect of flat anterior chamber on the success of trabeculectomy.

The authors reviewed 70 eyes of 59 patients who underwent trabeculectomy. We divided the eyes into the flat anterior chamber group (23 eyes) and formed anterior chamber group (47 eyes, control group). The influence of clinical variables on the development of flat anterior chamber, and the effect of flat anterior chamber on the success of trabeculectomy were evaluated. The two groups were similar in terms of many variables, but the preoperative intraocular pressure (27.1 +/- 15.4 vs. 18.1 +/- 8.7 mmHg), incidence of cataract progression (9/18 vs. 10/44) and choroidal detachment (5/23 vs. 2/47)

Utility of the color pattern-electroretinogram (PERG) in glaucoma.

Pattern-onset electroretinograms (PERGs) with red-green color contrast (CC) and green-"black" luminance contrast (LC) stripe patterns (0.3 c/deg) were recorded in a group of 80 control subjects and in a group of 42 patients having glaucomas of varying etiology and severity. The PERG data were correlated with the results of static perimetry and optic disc morphometry. In the glaucoma group the PERG was reduced significantly and by relatively similar amounts with both CC and LC stimuli. A significant correlation of the PERG reduction with visual field loss was found only with the CC, not with th
