Renal osteodystrophy represents a spectrum of skeletal lesions that range from high-turnover to low-turnover bone disease. Similar factors are involved in the pathogenesis of renal osteodystrophy in adult and pediatric patients with chronic kidney disease (CKD). However, growth retardation and the development of bone deformities are specific complications that occurred in pediatric patients with CKD. Metabolic acidosis, renal osteodystrophy, malnutrition, and disturbances in the insulin growth factor (IGF)/growth hormone (GH) are among the main factors involved and they are discussed briefly i

To date the cause of growth retardation of children who have suffered physical abuse and emotional deprivation is unknown. Hypophyseal disturbances in these patients have been proposed of the cause but there are still several concerns on the dynamics of growth hormone secretion in these children. In this study, eleven out of sixteen patients had a low height without important diminution of corporal weight. Growth hormone under basal conditions was found to be elevated in battered children compared with a control group (15.2 +/- 4.7 ng/mL vs 9.6 +/- 1.9 ng/mL, p = 0.025). Two weeks after hospit

Polyaminopathies are a family of rare genetic developmental disorders caused by pathogenic variants in genes involved in polyamine homeostasis and metabolism, and related biological pathways, including the hypusine biosynthesis pathway. DOHH is the enzyme catalyzing the last step of hypusine biosynthesis. DOHH-related disorders, caused by biallelic variants in the DOHH gene, were first described in 2022 in five patients exhibiting severe neurodevelopmental delay, congenital heart defects, and growth restriction. We report on a 10-year-old boy, the first child of consanguineous parents, whose c

OBJECTIVE: To assess the role of Doppler uterine artery screening in the prediction of recurring hypertensive disorders in a high-risk population.
METHODS: Ninety-four women with a history of hypertensive disorders in previous pregnancies underwent ultrasound color Doppler to analyze blood flow in the uterine arteries at 21-22 weeks of gestation. We evaluated the performance of the Pulsatility Index (PI) as well as the diastolic notch to predict recurring hypertensive disorders. Outcome measures were the recurrence of hypertensive disorders, and poor pregnancy outcome due to intrauterine deat

The insulin tolerance test (ITT) is the most reliable method for diagnosing growth hormone deficiency (GHD). Our study aimed to determine a new glycemic threshold that allows for an adequate growth hormone (GH) response without increasing the risk of deleterious outcomes. This study was conducted retrospectively on children who were suspected of GHD. There were two groups of children: group 1 (G1) consisted of children who underwent an ITT and reached a hypoglycemic level of 2.2 mmol/l, and group 2 (G2) included those who did not reach biochemical hypoglycemia but exhibited clinical hypoglycem

BACKGROUND: MIRAGE syndrome, a rare autosomal dominant disorder, is caused by heterozygous gain-of-function mutations in the SAMD9 gene. A key characteristic of MIRAGE syndrome is growth restriction. Although initially thought to stem mainly from prenatal and systemic factors, this growth restriction can also be a consequence of panhypopituitarism, leading to growth hormone deficiency (GHD). The use of recombinant human growth hormone (rhGH) to treat the characteristic severe growth failure is controversial due to an inherent risk of myelodysplastic syndrome (MDS) and acute myeloid leukemia.

CONTEXT: Growth hormone (GH) enhances muscle strength and aerobic capacity in adults with GH deficiency, but GH effects in skeletal muscle are less well characterized in youth.
OBJECTIVE: To investigate the impact of GH on skeletal muscle in boys with significant short stature (SS) compared with age-matched, normally growing controls.
METHODS: This was an open-label comparator at an outpatient endocrine clinic. Participants were 45 prepubertal boys, SS, n = 30 (isolated GH deficiency or idiopathic SS) (mean [SE] age: 8.9 ± 0.3 years; height SDS: -2.3 ± 0.1); controls, n = 15 (8.8 ± 0.4 years

Prompt diagnosis and early treatment are key goals to optimize the outcomes of children with growth hormone deficiency (GHD) and attain the genetically expected adult height. Nonetheless, several barriers can hinder prompt diagnosis and treatment of GHD, including payer-related issues. In Saudi Arabia, moderate-to-severe short stature was reported in 13.1 and 11.7 % of healthy boys and girls, respectively. Several access and payer barriers can face pediatric endocrinologists during the diagnosis and treatment of GHD in Saudi Arabia. Insurance coverage policies can restrict access to diagnostic

OBJECTIVE: The aim of this study was to explore the impact of growth hormone (GH) therapy on the onset and progression of puberty in girls with idiopathic short stature.
METHODS: This study included 541 girls aged between 4.5 and 10.6 years who were receiving GH treatment, monitored over a 22-year follow-up period. Of these, 126 girls have been followed up to the onset of menarche. The participants were divided into two groups: a ISS control group (n = 66) and a group receiving daily GH treatment at a dose of 0.15 iu/kg (n = 60). We assessed the pubertal development and GH usage of these girl

BACKGROUND: Short stature is a frequent complication after pediatric kidney transplantation (KT). Whether the type of transplantation and prior treatment with recombinant human growth hormone (GH) affects post-transplant growth, is unclear.
METHODS: Body height, leg length, sitting height, and sitting height index (as a measure of body proportions) were prospectively investigated in 148 prepubertal patients enrolled in the CKD Growth and Development study with a median follow-up of 5.0 years. We used linear mixed-effects models to identify predictors for body dimensions.
RESULTS: Pre-transpl

Specific phenotypic features of subjects affected with genetic syndromes depend on peculiarities of expression of each discrete mutation and on extent of its divergence from normal physiology. In this context, and when studying the GH/IGF-I axis of subjects with two different syndromes that include severe short stature (SSS), we noticed different metabolic phenotypes in each cohort. Subjects with Laron syndrome (LS), who have GH insensitivity (GHI), display obesity, increased body fat, enhanced insulin sensitivity and diminished incidence of diabetes mellitus. Subjects with a new syndrome (NS)

The use of appropriate growth standards/references is of significant clinical importance in assessing the height of children with short stat … nces. This study highlights that, although no growth reference has 100% sensitivity for identifying growth disorders, the most recent national or regional growth charts may offer the most appropriate tool for monitoring childhood growth in Europe.
