Early excision of full-thickness hand and digit burns: factors affecting morbidity.

Full-thickness dorsal hand and digit burns result in serious acute and chronic functional disability. Early wound closure is paramount to minimize functional impairment and improve ultimate cosmetic appearance. Fifty patients were followed for 2 to 6 years after excision and immediate autografting of full-thickness hand and digit burns. Based on the depth of injury distal to the metacarpal phalangeal joints, two groups of hand burns emerged in this series. Group I: Patients had uninjured and intact extensor mechanisms. Near normal range of motion was obtained by the end of the second postopera

Trabecular bone score and phalangeal quantitative ultrasound are associated with muscle strength and fracture risk in hemodialysis patients.

There is growing interest in the relationship between chronic kidney disease (CKD) and fragility fracture risk. Bone mineral density (BMD) is a major determinant of bone strength, although its role as a predictor of fracture in advanced CKD and hemodialysis is still under debate. We aimed to further investigate surrogates of bone quality and their associations with muscle strength and fracture risk in hemodialysis. Multiple clinical risk factors for fracture and an estimated 10-year probability of fracture, BMD at lumbar spine and femur, trabecular bone score (TBS), X-ray vertebral morphometry

Factors affecting healing following percutaneous intramedullary fixation of metacarpal fractures.

Although percutaneous intramedullary nailing of metacarpal fractures is a straightforward and reliable technique, it is not without complications, and patients experience different outcomes. This study analyzed factors affecting fracture healing time and complication rates in patients who underwent percutaneous intramedullary fixation of metacarpal fractures. This study was a retrospective review of the 25 patients who underwent retrograde percutaneous Kirschner wire (K-wire) nailing for fracture of the metacarpal shaft or neck at a military hospital between May 2016 and October 2018. [Aseptic osteonecrosis of the head of the metacarpal (Mauclaire's disease)--case report and review of the literature].

Aseptic osteonecrosis of the head of metacarpal is a rare disease with an unknown aetiology. It was first described by Mauclaire 1927. We now report on the treatment of 2 patients with Mauclaire's disease via transplantation of the calotte of the head of the 2nd metatarsal. At 26 and 30 months, respectively, after surgery both patients were free of complaints in activities of daily live and had free function of their fingers. There was no discomfort in the resection area at the 2nd metatarsal. Based on this experience, the method seems to be a recommendable option in young adults. A prerequisi

Operative treatment of fractures and dislocations of the hand and wrist region in children.

The indications for open treatment of skeletal injury of the pediatric hand and wrist region are limited, but failure to surgically treat these fractures and dislocations will cause later morbidity. Irreducible dislocations of the small joints of a child's hand require open reduction. Angulated fractures of the middle and proximal phalanx in the region of the neck or distal shaft also require exact reduction because of the poor remodeling capacity in that region. Irreducible Salter-Harris II fractures of the distal radius and unstable Galeazzi type fracture-dislocations of the distal radius in

[From hematopoietic suppression to therapy resistance: the dual outcomes of leukemia-induced remodeling of the bone marrow vascular microenvironment].

The bone marrow vascular microenvironment is primarily composed of endothelial cells (ECs) and mesenchymal stem/stromal cells (MSCs), which maintain hematopoietic homeostasis by regulating the quiescence, activation, proliferation, and differentiation of hematopoietic stem cells (HSCs). During leukemogenesis and disease progression, malignant cells remodel the vascular microenvironment, leading to aberrant vascular architecture, disruption of endothelial barrier integrity, phenotypic conversion of MSCs toward a pro-inflammatory/pro-survival state, and the dismantling of normal signaling networ

Appraising efficiency of OpSite as coolant in drilling of bone.

BACKGROUND: During drilling of bone, which is common in clinical surgeries, heat generation increases local temperature in the drilling site. Transmission of excessive heat to the surrounding bone tissue can cause thermal osteonecrosis. Consequently, it may lead to failure of implants and fixation screws or delay in healing process. Using cooling is a method for limiting temperature elevation. MATERIALS AND METHODS: In this study, through comparing three conditions of drilling without cooling, external cooling with normal saline, and external cooling with OpSite spray, the efficiency of OpSit

A Thai mother and son with distal symphalangism, hypoplastic carpal bones, microdontia, dental pulp stones, and narrowing of the zygomatic arch: a new distal symphalangism syndrome?

A Thai mother and son with distal symphalangism and other associated abnormalities are reported. Distal and middle phalanges of fingers and toes 2-5 were either aplastic/hypoplastic or fused between the corresponding digits. The second fingers and fourth fingernails were most severely affected in both patients. The mother's hands were less severely affected; the middle and distal phalanges of her hands were malformed and fused. Besides the absence of fusion lines, the shape of the fused middle and distal phalanges was quite different from that of other types of fusion, i.e., fused bones in bot

[Endoprosthesis and bone reaction].

Prosthetic implants cause a reaction on the part of the surrounding bone. Bone reaction can be interpreted as a dynamic response of the internal bone architecture to stress and strain. On the other hand, instability at the bone-implant interface or biological reactions to wear particles may lead to bone resorption. Standard X-ray examination has remained the most important tool for evaluation of the bone-implant relationship. Knowledge of the radiological features of bone remodelling and bone reaction is therefore a prerequisite for the orthopaedic surgeon. Based on eight case reports after pr

A patient-matched prosthesis for thumb amputations: Design, mechanical and functional evaluation.

Thumb amputations strongly affect hand functionality in daily activities. The currently available solutions, such as microsurgical treatments and external vacuum prostheses present disadvantages, which can be successfully addressed through the osseointegration technique. However, despite its widespread use in oral applications, only a few osseointegrated solutions for the treatment of hand-finger amputations are available. Bone remaining limbs may have different lengths, diameters, and conditions and no patient-matched osseointegrated medical devices are available on the market. The manuscript
