Review of metamaterials principles and methods in ventilation ducts: 1928-2024.

Acoustic metamaterials (AMMs) have emerged as a promising strategy for low-frequency noise control in ventilation ducts, offering subwavelength, tunable solutions that overcome key limitations of conventional silencers, such as high pressure drop and reduced long-term performance. This review presents a structured analysis of 54 studies on AMMs for duct acoustics, with civil, mechanical, aerospace, and biomedical engineering applications. AMM unit designs are categorized by physical mechanism, including resonant cavities, acoustic membranes, Herschel-Quincke tubes, Fano-like interference struc

Percussion assisted radiation therapy in Hodgkin lymphoma allows a marked reduction in heart dose.

BACKGROUND AND PURPOSE: Early-stage Hodgkin lymphoma (HL) is a highly curable disease but the treatment can induce late complications many years later. Irradiation of the healthy heart is inevitable during radiation treatment of mediastinal sites. We developed a novel method to induce a prolonged apnea-like state that can help decrease the dose to organs at risk during radiation therapy. We present the results of the first 8 HL patients treated routinely with percussion assisted radiation therapy (PART) in our clinic. MATERIAL AND METHODS: We used a newly developed high-frequency non-invasive

Twin pregnancy in a patient on chronic haemodialysis who already had three pregnancies.

Pregnancy in women with end-stage renal disease is rare. Multiple pregnancies carry a high risk of complications even in healthy individuals. We report the case of a 36-year-old woman who had four pregnancies while she was on dialysis, including one twin pregnancy. The last pregnancy occurred while in the 14th year of hemodialysis. At 8 weeks of gestation (WG), ultrasonography diagnosed a dichorionic diamniotic twin pregnancy. The frequency of dialysis was increased from 3 to 6 times a week and each session lasted 4 h. At 22 WG, polyhydramnios was diagnosed. At 25 WG, the patient presented res

Guillian-Barre syndrome.

Guillian-Barre syndrome (GBS) is the most common cause of acute flaccid paralysis. All age groups can be affected, more common in elderly. Campylobacter jejuni, a major cause of bacterial gastroenteritis worldwide has become recognized as a most frequent antecedent pathogen for GBS. A prospective case-controlled study showed, positive C. Jejuni serology was found in an unprecedented high frequency of 57% as compared to 8% in family controls and 3% in control patients with other neurological diseases. In GBS there is molecular mimicry between epitops found in the cell walls of some micro-organi

Nihilism in the 1990s: the true mortality of congenital diaphragmatic hernia.

OBJECTIVE: Reported survival in congenital diaphragmatic hernia (CDH) fails to allow for case selection bias. This study reports the incidence of CDH in a geographically defined population over 11 years and assesses the effect of new therapies (high-frequency oscillatory ventilation, extracorporeal membrane oxygenation, inhaled nitric oxide, and delayed surgery) on survival when case selection is avoided. METHODS: A retrospective review of cases from a regional case registry, the Northern Region Congenital Anomaly Survey, was conducted. RESULTS: A total of 185 cases were identified. Mortalit

Misalignment of lung vessels: diagnostic role of conventional histology and immunohistochemistry.

Misalignment of lung vessels represents a rare congenital anomaly that may cause respiratory failure in the newborn. It is characterized by abnormal position of pulmonary veins and venules that lie adjacent to arteries and bronchi, and it is usually associated with a decreased number of alveolar capillaries (i.e., alveolar capillary dysplasia), although these two conditions have been separately described. Awareness of this anomaly is required by pathologists because it can be easily overlooked on lung biopsy or autopsy, and because definite diagnosis relies on histology. We report the case of

Noninvasive clearance of airway secretions.

Airway clearance techniques are indicated for specific diseases that have known clearance abnormalities (Table 2). Murray and others have commented that such techniques are required only for patients with a daily sputum production of greater than 30 mL. The authors have observed that patients with diseases known to cause clearance abnormalities can have sputum clearance with some techniques, such as positive expiratory pressure, autogenic drainage, and active cycle of breathing techniques, when PDPV has not been effective. Hasani et al has shown that use of the forced exhalatory technique in p

The occurrence of high-risk factors for hearing loss in very-low-birth-weight neonates: a retrospective exploratory study of targeted hearing screening.

The current study aimed at determining the type and frequency of high-risk factors for hearing loss in a group of very-low-birth-weight (VLBW) neonates in a tertiary hospital in South Africa with the objective of collating evidence that could be used in arguing for or against revisiting targeted hearing screening in developing countries. Furthermore, the study aimed at investigating the relationship between the identified high-risk factors and hearing screening results. In a retrospective data review design, data were collated from files from the VLBW project; this included hearing screening r

Diaphragmatic ultrasound and thoracic fluid content for prediction of non-invasive ventilation failure in neonates: a randomized controlled trial.

UNLABELLED: Non-invasive ventilation (NIV) remains the standard of care for preterm infants with respiratory distress syndrome (RDS); however, NIV failure is frequent and often linked to adverse outcomes. This study evaluated diaphragmatic ultrasound and thoracic fluid content (TFC) as early predictors of NIV failure in preterm neonates. In this prospective randomized controlled trial, 90 preterm neonates (28-34 gestational weeks) with RDS requiring NIV as initial respiratory support were randomized into three groups: (1) nasal continuous positive airway pressure (nCPAP), (2) nasal intermitten

A Clinical Study on Congenital Diaphragmatic Hernia in Neonates: Our Institutional Experience.

BACKGROUND: Congenital diaphragmatic hernia (CDH) is a complex developmental defect having a multifactorial etiology; i majority of cases (~80%), the cause is not known. Survival rates for patients with CDH have increased over the past decade with early prenatal detection and better postnatal management including surgery. Clinical profile and the outcome of 83 CDH neonates were studied and analyzed over a period of 12 years in our institute. AIMS AND OBJECTIVES: The clinical study was to analyze the clinical profile and outcome of CDH among the neonates in a tertiary care referral neonatal an

Viable virus aerosol propagation by positive airway pressure circuit leak and mitigation with a ventilated patient hood.

INTRODUCTION: Nosocomial transmission of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has been a major feature of the COVID-19 pandemic. Evidence suggests patients can auto-emit aerosols containing viable viruses; these aerosols could be further propagated when patients undergo certain treatments, including continuous positive airway pressure (PAP) therapy. Our aim was to assess 1) the degree of viable virus propagated from PAP circuit mask leak and 2) the efficacy of a ventilated plastic canopy to mitigate virus propagation. METHODS: Bacteriophage phiX174 (108 copies·mL-1) wa

Surgical procedures performed in the neonatal intensive care unit on critically ill neonates: feasibility and safety.

BACKGROUND AND OBJECTIVE: Transferring unstable, ill neonates to and from the operating room carries significant risks and can lead to morbidity. We report on our experience in performing certain procedures in critically ill neonates in the neonatal intensive care unit (NICU). We examined the feasibility and safety of such an approach. METHODS: All surgical procedures performed in the the NICU between January 1999 and December 2005 were analyzed in terms of demographic data, diagnosis, preoperative stability of the patient, procedures performed, complications and outcome. Operations were perf

Improving newborn health in countries exposed to political violence: An assessment of the availability, accessibility, and distribution of neonatal health services at Palestinian hospitals.

INTRODUCTION: Geopolitical segregation of Palestine has left a fragile healthcare system with an unequal distribution of services. Data from the Gaza Strip reflect an increase in infant mortality that coincided with a significant increase in neonatal mortality (12.0 to 20.3 per 1,000 live births). OBJECTIVE: A baseline study was carried out to evaluate available resources in neonatal units throughout Palestine. STUDY DESIGN: A cross-sectional, hospital-based study was conducted in 2017 using the World Health Organization's "Hospital care for mothers and newborn babies: quality assessment and

A real-time machine learning framework for improved intraoperative risk predictions in cardiac surgery patients.

BACKGROUND AND OBJECTIVE: Cardiac surgeries are associated with a range of serious postoperative complications, and current predictive models often rely on static summaries of preoperative and intraoperative data compiled after the case. Such development limits their utility in real-time clinical settings. To address this challenge, we developed TE-Transformer, an end-to-end deep learning framework that continuously ingests heterogeneous electronic health record (EHR) data-including preoperative variables, high-frequency vital signs, medication administration records, and brief surgical notes
