Erosive gout: update on mechanisms, impact and treatment.

PURPOSE OF REVIEW: Erosive gout refers to the presence of at least one gouty bone erosion in a person with gout. This review provides an update on the update on mechanisms, impact, and treatment of erosive gout. RECENT FINDINGS: Erosive gout is usually the consequence of long-standing tophaceous disease. However, bone erosions in association with tophi can be detected on ultrasound before onset of clinical symptoms in some people with asymptomatic hyperuricaemia. SPP1 (osteopontin)-expressing macrophages have been implicated in pathogenesis of tophaceous gout and SPP1-mediated cellular intera

[Gout and Its Management in Clinical Practice].

Gout and Its Management in Clinical Practice Abstract. Resolution of an acute attack is usually the prime objective in routine clinical management of gout. Crystal identification in synovial fluid by polarised light microscopy is considered the diagnostic gold standard. Imaging procedures such as high-resolution ultrasonography are also useful. Non-steroidal anti-inflammatory drugs, steroids and colchicine (not approved in Switzerland, available from pharmacies) are used to treat an acute gout attack. Just as important as the diagnosis and treatment of an acute attack is the long-term manageme

Beyond Medical Treatment: Surgical Treatment of Gout.

PURPOSE OF REVIEW: Medical treatment with urate-lowering therapy (ULT) is efficacious. A recent publication suggested that surgery in gout i … performed in Asia and Europe. Most patients were not taking ULT at the time of surgery, leading to hyperuricemia. This can result in tophi reformation post-surgery. The role of surgery should be a last-line treatment and until recently has only been demonstrated through case reports.

Non-invasive determination of uric acid in human saliva in the diagnosis of serious disorders.

This review summarizes and critically evaluates the published approaches and recent trends in sample pre-treatment, as well as both separation and non-separation techniques used for the determination of uric acid (UA) in saliva. UA is the final product of purine nucleotide catabolism in humans. UA concentrations in biological fluids such as serum, plasma, and urine represent an important biomarker of diseases including gout, hyperuricemia, or disorders associated with oxidative stress. Previous studies reported correlation between UA concentrations detected in saliva and in the blood. The inte…

The nomenclature of the basic disease elements of gout: A content analysis of contemporary medical journals.

OBJECTIVES: There is currently no standardised nomenclature for the basic disease elements of gout. This study aimed to identify these elements and examine how they are labelled in contemporary medical literature. METHODS: We analysed articles from the ten highest ranked general rheumatology journals, and five highest ranked general internal medicine journals (by Impact Factor, according to 2015 Thomson-Reuters Journal Citation Reports), published between 1 January 2012 and 31 January 2017. For each journal, articles relevant to gout and hyperuricaemia were identified by the search terms 'gou

An association of smoking with serum urate and gout: A health paradox.

BACKGROUND: The potential effect of cigarette smoking on levels of serum urate and risk of gout has been considered by a large number of studies, either as the primary variable of interest or as a covariate. METHODS: Here we systematically review the published evidence relating to the relationship of smoking with serum urate, hyperuricaemia, and gout. RESULTS: Many studies have reported that smoking reduces serum urate, however, the evidence has not been conclusive with other studies pointing to the opposite or no effect. It has also been suggested that smoking reduces the risk of gout, alth

Review of the ophthalmic manifestations of gout and uric acid crystal deposition.

Gout is a clinical disorder that is characterized by the deposition of monosodium urate crystals (MSU) in joints and tendons, usually in the presence of prolonged hyperuricaemia. Following an asymptomatic phase of hyperuricaemia, gout usually presents as acute monoarthritis followed by periods of remission and exacerbation. Conjunctival hyperaemia and subconjunctival haemorrhage exacerbated by purine intake are two of the more common manifestations that may go unrecognized. Other ocular and adnexal structures can be affected by urate crystal deposition and associated inflammation, with potenti

Axial (spinal) gout.

Gout is a common metabolic disorder resulting from supersaturation of uric acid in extracellular fluid and deposition of monosodium urate crystals in tissues. Gouty arthritis typically affects the peripheral joints of the appendicular skeleton, especially the feet and hands. Gouty involvement of the spine, however, is not as rare as generally perceived. Although it may be asymptomatic, tophaceous axial gout is also a well-documented cause of acute back pain, radiculopathy, and frank cord compression. As with the appendicular skeleton, it takes several years of gout before radiological evidence

Identification of Inflammation-Related Biomarker Pro-ADM for Male Patients With Gout by Comprehensive Analysis.

OBJECTIVE: Gout is a local inflammatory disease caused by the deposition of monosodium urate (MSU) crystals in joints or adjacent tissues. When some gout occurs without hyperuricemia, or its clinical symptoms and signs are not typical, the diagnosis of gout will be delayed, so there is an urgent need to find a new biomarker to predict and diagnose of gout flare. Our research attempts to find the key genes and potential molecular mechanisms of gout through bioinformatics analysis, and collected general data and blood biochemical samples of patients with gout and healthy, then analyzed and compa…

Uric acid does not affect the prevalence and extent of coronary artery disease. Results from a prospective study.

BACKGROUND: Hyperuricemia may be involved in the atherosclerotic process due to endothelial dysfunction and facilitation of smooth muscle cell proliferation. However, debates still exist on the independent role of hyperuricemia, due to its association with several cardiovascular risk factors, such as hypertension, hyperlipidemia, obesity and insulin resistance. Thus, the aim of the current study was to investigate in a consecutive cohort of patients undergoing coronary angiography whether hyperuricemia is associated with the extent of coronary artery disease. METHODS AND RESULTS: Our populati…

Integrating Metabolomics and Network Pharmacology to Identify Xanthine Oxidase Inhibitors from Raspberry Leaf: Optimization by Ultrasound-Assisted Extraction and Kinetic Characterization.

The rising global prevalence of hyperuricemia has intensified the search for natural xanthine oxidase (XO) inhibitors from food-grade botanicals. This study screened leaf extracts from four plant species for XO inhibitory activity and identified raspberry leaf as the most potent source. Among seven extraction and pre-treatment methods, ultrasound-assisted extraction (UAE) outperformed conventional decoction and all alternatives. Box--Behnken response surface methodology optimized UAE parameters to 70 °C, 50 min, and a solid-to-liquid ratio of 1:20 g/mL, yielding an extract with 97.77 ± 0.08% X…

Post-transplant metabolic dysregulation: Insights and implications for kidney graft survival.

Kidney transplantation is the preferred treatment for end-stage kidney disease, improving survival and quality of life. Yet, metabolic disorders including post-transplant diabetes mellitus, dyslipidemia, hyperuricemia, secondary hyperparathyroidism, and obesity are common in kidney transplant recipients and threaten graft function and patient outcomes. Post-transplant diabetes mellitus affects up to 30% of recipients within the first year, while transient hyperglycemia occurs in 60% of nondiabetic patients post-surgery. Dyslipidemia, obesity, and hyperuricemia exacerbate cardiovascular and met…
