Enteric alimentation in specialized gastrointestinal problems: an alternative to total parenteral nutrition.

This review has focused on the enteric nutritional management of infants with abnormalities of volume tolerance, digestion, and/or absorption. The necessity of individually assessing the patient's needs vis-à-vis the alternatives cannot be overemphasized. In some patients, enteral feedings are precluded and IV alimentation is the only alternative. For a discussion of IV nutrition options we refer the physician to an excellent review. We also wish to emphasize that in a review of this nature we cannot discuss the relative virtues of closely related formulas. We therefore encourage the physician

Enteral feeding of premature infants with Saccharomyces boulardii.

BACKGROUND: Saccharomyces boulardii (SB) is a yeast that acts both as a probiotic and as a polyamine producer. Probiotics prevent the overgrowth of pathogens in the gut while polyamines enhance intestinal maturation. The aim of this randomized study was to investigate the ability of SB to modify the gut microbial ecology and its function. METHODS: A total of 87 healthy babies with gestational age 28-32 weeks were studied. They were randomly assigned to receive a preterm formula to which SB or maltodextrins was added for 30 days. Evaluations were made on the following: SB tolerance and weight

Changes in growth and metabolism in very low birthweight infants fed with fortified breast milk.

Human milk is often inadequate nutritionally for preterm infants. We investigated the effect of adding a commercially prepared milk fortifie … is increase in LLLvel was comparable to that achieved by a group of babies given a standard preterm infant formula (p < 0.001). By comparison, the control group's change in LLLvel was more modest (p = 0.04). Babies who received human milk with the fortifier added had the lowest serum levels of alkaline phosphatase at the end of the study period when compared to …

Cow's milk allergy.

PURPOSE OF REVIEW: To highlight the most recent insights on cow's milk allergy (CMA), its treatment, and management. RECENT FINDINGS: CMA is one of the most common food allergies among children. Burdened by the risk for fatal reaction, CMA may imply also a severe impairment of health-related quality of life at individual and family level as well as well as individual and societal costs. The updated Diagnosis and Rationale for Action against Cow's Milk Allergy series is going to provide a series of manuscripts that will offer a comprehensive state-of-the-art specifically on CMA, including inte

Comparative Effect of Feeding Human Milk as Opposed to Formula on Visual Function and Ocular Anatomy.

Background/Objective: Performance of ocular examinations on children who were breastfed, fed with formula, and combination of the two for the first 6 months of age. Subsequently, refractive errors, allergic conjunctivitis, and retinal nerve fiber layer (RNFL) thickness were evaluated. Materials and Methods: The present study included a total of 242 eyes of 121 children (aged 60-84 months, 65 males, 56 females) admitted to the outpatient clinic of our institution. The patients were divided into three groups according to their feeding pattern during their first 6 months postdelivery: breastfed c

Polyamine levels in breast milk are associated with mothers' dietary intake and are higher in preterm than full-term human milk and formulas.

BACKGROUND: Polyamine intake from milk is considered essential for post-natal maturation of the immune system and small intestine. The present study aimed to determine polyamine content in human milk after preterm delivery and the association with mothers' dietary intake. In comparison, the polyamine levels were compared with those in term breast milk and some corresponding formulas. METHODS: Transitional breast milk was collected from 40 mothers delivering after 24-36 weeks of gestation, and from 12 mothers delivering after full term. Food intake was assessed in mothers delivering preterm ba

Allergic diseases among very preterm infants according to nutrition after hospital discharge.

To determine whether a cow's milk-based human milk fortifier (HMF) added to mother's milk while breastfeeding or a cow's milk-based preterm formula compared to exclusively mother's milk after hospital discharge, increases the incidence of developing allergic diseases among very preterm infants (VPI) during the first year of life. Of a cohort of 324 VPI (gestational age 24-32 wk), the exclusively breastfed VPI were shortly before discharge randomized to breastfeeding without fortification or supplementing with a fortifier. Those not breastfed were fed a preterm formula. The intervention period

Heavy metal exposures in aerodigestive clinic cohort of infants with reflux or dysphagia.

Infant cereals (rice/oatmeal), purees, and anti-reflux formulas are often first line treatments for thickening in infants with reflux and oropharyngeal dysphagia. However, there has been growing concern about heavy metal contamination, especially arsenic, in these commonly used thickeners. This is a particular concern in infants who may be more susceptible to heavy metal neurotoxicity. The study aimed to assess whether there are differences in heavy metal levels, particularly arsenic, in infants with reflux or oropharyngeal dysphagia while on different thickeners. We performed a single center

Bovine Colostrum for Preterm Infants in the First Days of Life: A Randomized Controlled Pilot Trial.

OBJECTIVES: Mother's own milk (MM) is the best nutrition for newborn preterm infants, but is often limited in supply just after birth. Pasteurized human donor milk (DM), and especially preterm infant formula (IF) are less optimal diets than MM. We hypothesized that it is feasible to use bovine colostrum (BC), the first milk from cows, as a supplement to MM, during the first weeks after preterm birth. METHODS: In an open-label, randomized, controlled pilot safety trial, supplementation of MM with BC was compared with DM supplementation (Danish unit) or IF supplementation (Chinese unit). If MM …

The study of breast milk IGF-1, leptin, ghrelin and adiponectin levels as possible reasons of high weight gain in breast-fed infants.

BACKGROUND: Excessive consumption of protein that leads to increased blood levels of insulin-like growth factor-1 (IGF-1) is an important risk factor for high growth velocity and obesity in formula-fed infants. However, it is not clear whether these factors can explain the high growth velocity in breast-fed infants. AIM: To study the possible links between the growth velocity in breast-fed infants and the levels of protein, IGF-1 and other hormones, which regulate energy homeostasis, in mothers' breast milk. METHODS: We studied 103 mother-infant pairs. Their daily breast milk intake and leve

Long-chain polyunsaturated fatty acids (LCPUFAs) are of nutritional interest because they are crucial for normal development of the central … d milk with a docosahexaenoic acid (DHA) content 2-3 times higher than the current concentration in infant formulas. Overall, these studies show that providing larger amounts of DHA supplements, especially to the smallest infants, is associated with better neurologic outcomes in early life. Observed and calculated cerebral critical closing pressure are highly correlated in preterm infants.

BACKGROUND: Cerebrovascular critical closing pressure (CrCP) is the arterial blood pressure (ABP) at which cerebral blood flow ceases. Preterm ABP is low and close to CrCP. The diastolic closing margin (diastolic ABP minus CrCP) has been associated with intraventricular hemorrhage in preterm infants. CrCP is estimated from middle cerebral artery cerebral blood flow velocity (CBFV) and ABP waveforms. However, these estimations have not been validated due to a lack of gold standard. Direct observation of the CrCP in preterm infants with hypotension is an opportunity to validate synchronously est

Gastroesophageal Reflux Disease.

Gastroesophageal reflux (GER) is a normal physiologic process. It is important to distinguish GER from GER disease (GERD) since GER does not require treatment. Although a diagnosis of GERD can largely be based on history and physical alone, endoscopy and pH impedance studies can help make the diagnosis when there in atypical presentation. In children and adolescents, lifestyle changes and acid suppression are first-line treatments for GERD. In infants, acid suppression is not effective, but a trial of hydrolyzed formula can be considered, as milk protein sensitivity can be difficult to differe

Breast milk-associated physiological hypercalcaemia: an observational study of clinical, biochemical and radiological outcomes.

INTRODUCTION: Asymptomatic physiological hypercalcaemia in exclusively breastfed infants is recognised but has not been systematically characterised when breastfeeding is continued. OBJECTIVE: To evaluate the clinical, biochemical and radiological outcomes of breast milk-associated physiological hypercalcaemia. METHODS: A multi-centre retrospective study over 5 years included infants with hypercalcaemia (serum corrected calcium (cCa) ≥2.8 mmol/L) who were exclusively breastfed and had no other identifiable causes of hypercalcaemia. Clinical, biochemical and radiological data were analysed.

GLP-1 and IGF-I levels are elevated in late infancy in low birth weight infants, independently of GLP-1 receptor polymorphisms and neonatal nutrition.

Low birth weight followed by rapid postnatal weight gain is associated with increased risks for obesity and diabetes in adulthood. Modulation of glucagon-like-peptide 1 (GLP-1) secretion by (epi)genetic mechanisms or nutrition may, in part, influence this risk. Formula-fed infants born small-for-gestational-age (SGA) have higher circulating GLP-1 at age 4 months than breastfed SGA or appropriate-for-gestational-age (AGA) infants. Here we assessed GLP-1 concentrations in healthy AGA (n=149) and SGA (n=107) subjects at age 12 months and their association with endocrine-metabolic and body composi
