ICLH knee arthroplasty. A consecutive study of 108 knees with uncemented tibial component fixation.

One hundred eight ICLH knee arthroplasties with cementless tibial component fixation were performed from 1978 through 1982 in Gothenburg, Sweden. The patients were followed for 2-8 years (mean, 4 years) with clinical and radiographic examinations at regular intervals, and the data were computer-analyzed. Patellar subluxation-dislocation, often accompanied by pain, occurred in 50% of the knees. Eleven arthroplasties failed due to aseptic loosening. Survival rate analysis performed using three different definitions of failure resulted in cumulative success rates after 8 years of 86%, 78%, and 72

Anterior subluxation of the lateral tibial plateau. A diagnostic test and operative repair.

Recurrent anterior subluxation of the lateral tibial plateau is a common type of chronic knee instability resulting from trauma. It can be reproduced by the clinical test described and corrected by a surgical procedure called the sling and reef operation, in which a strip of iliotibial tract is used to create a sling and to reef the posterolateral capsule. From 1971 to 1978, eighty-four patients were operated on, of whom fifty had been evaluated at one to six and one-half years after operation. The results were: forty-one good, six fair, and three poor. The lesions found in the thirty-seven kn

[Plate osteosynthesis of patellar fractures].

OBJECTIVE: Anatomic reconstruction of the retropatellar articular surface and repair of the extensor mechanism of the knee joint. The osteosynthesis should allow immediate mobilization as part of an early functional postoperative rehabilitation protocol. INDICATIONS: Displaced fractures of the patella, especially multifragment and comminuted fractures with a retropatellar incongruity or dislocation of >2 mm. CONTRAINDICATIONS: Critical local soft tissue because of the risk of postoperative infection. SURGICAL TECHNIQUE: Median skin incision. For simple (transverse) fractures, preservation o

Endovascular graft bail-out post reconstruction of popliteal artery injury. First case report.

A male patient who sustained a blunt trauma to the right knee and chest resulting in posterior dislocation of the knee joint and avulsion injury to the right popliteal artery (segments II and III) had a vascular repair by interposition saphenous vein graft. Postoperatively an episode of hypotension resulted in thrombotic occlusion of the graft and acute limb ischemia. Graft and distal thrombectomy, though successful, resulted in a stenotic segment with two perforations. To shorten the operating time this was managed by Viabahn Endoprothesis stent-graft through both native popliteal artery and

Tibial Allograft Transfer for Medial Tibial Plateau Resurfacing.

A large cartilage lesion of the tibial plateau with a deficient meniscus in a young patient is a challenging pathology for the orthopaedic surgeon due to the limited options available. While hemiarthroplasty procedures can be an option, the risk of revision in a young patient is high, and therefore a reconstructive procedure is advocated. The purpose of this Technical Note is to describe our technique for tibial plateau resurfacing, including the preparation and implantation of a medial tibial plateau and medial meniscal allograft, in a patient with a post-traumatic KD-4 knee dislocation with …

[Patellofemoral instability in trisomy 21: MPFL-Reconstruction as a single procedure].

BACKGROUND: Patellar instability is a common orthopaedic condition which is often seen in younger individuals. Biomechanical studies have shown that the medial patellofemoral ligament (MPFL) is the most important soft tissue that restrains lateral subluxation of the patella in the beginning of flexion of the knee joint. METHODS: MPFL reconstruction is an effective procedure to treat recurrent patellar dislocation. Double-bundle and single-bundle procedures have been described. If double-bundle reconstruction is not possible, there are good postoperative outcomes with single-bundle procedure a

[Tuberculous arthritis and chondrocalcinosis. Apropos of 2 cases].

Two cases of tuberculous arthritis in a joint affected with chondrocalcinosis are reported. No similar cases have been published. Diagnosis was established by demonstration of the tubercle bacillus and calcium pyrophosphate crystals in the joint fluid. Both patients were elderly French females. One patient with involvement of a knee required amputation. The other patient had involvement of a shoulder and developed inferior dislocation of the humeral head and drooping shoulder despite antituberculous therapy. Concomitant occurrence of the two conditions was apparently coincidental but may have

Delayed Presentation of Popliteal Artery Injury after Salter-Harris III Proximal Tibia Fracture.

INTRODUCTION: Proximal tibia physeal fractures in children are not very common but can be dangerous because they can harm popliteal fossa structures, especially the popliteal artery. Popliteal artery injuries (PAI) are most commonly the result of trauma to the lower extremity, including blunt force, hyperextension injuries, complex fractures, and knee dislocations that can compromise popliteal neurovascular structures. Case Presentation. A 14-year-old boy presents to the emergency department after being transferred from an outside hospital 24 hours after a left lower extremity hyperextension i…

Treatment of the Lower Extremity Contracture/Deformities.

Lower extremity deformities of patients with arthrogryposis multiplex congenita present a wide spectrum of severity and deformity combinations. Treatment goals range from merely ensuring comfortable seating and shoe wear, to fully independent and active ambulation, but the overarching intention is to help realize the patient's greatest potential for independence and function. Treatment of hip and knee contractures and dislocations has become more interventional, whereas treatment of foot deformities has paradoxically become much less surgical. This article synopsizes the treatment strategies p

[Pathology of injuries due to snow surfing].

The amount of injured snowboarders is increasing each year. The reason is due to the success of snowboarding for teenagers. Accidents occur more during the first hours of practice. Upper limb is the first location: fore-arm breaks and wrist fractures are very common. Elbow and shoulder dislocations also occur often for snowboarders. The location of most injuries for lower leg is the ankle; we found many ankle sprains and astragalus's breaks. The knee sprain is rare unlike alpine ski. Prevention has to be developed to give a better image of this sport, in fact the attitude of the participants i

Knee angles and rickets in nigerian children.

Knee angles of 2,036 normal Nigerian children up to 12 years old were measured directly or from photographs. The knees were bowed (varus) in the first 6 months. At 21 to 23 months, the distribution of angles became strongly bimodal: about half were varus and half were valgus (knock-kneed), with few in between. After this they were all valgus, with few exceptions. Hence, the change from varus to valgus in individual infants must be sudden (a few weeks), although the changeover of the whole population appears smooth and gradual. They became maximally and uniformly knock-kneed (-7.1 degrees +/- 1

Associated joint pathology in the anterior cruciate ligament-deficient knee with emphasis on a classification system and injuries to the meniscocapsular ligament-musculotendinous unit complex.

This article defines the anterior cruciate ligament-deficient knee in terms of capsular instabilities. This definition lends itself to a better assessment of the associated pathology consisting of torn capsular ligaments, torn medial and lateral menisci, chondral fractures, subluxation of the patella, and acute contusion of the peroneal nerve. The specific incidence of torn medial meniscus was 70 per cent, with a torn lateral meniscus identified 77 per cent of the time.

Walking ability in patients with arthrogryposis multiplex congenita.

BACKGROUND: Arthrogryposis multiplex congenita (AMC) is a multiple joint condition which affects both lower and upper extremities and thus affects ambulation. Multiple surgeries are needed to correct limb deformity in order to promote walking. The objective of this study is to identify the most critical residual deformity that diminishes the ambulatory status. MATERIALS AND METHODS: 51 patients were included in this study, 14 patients were nonambulatory. The mean age at first surgery was 4.1 years (range 2-16 years). The mean length of followup was 44.0 months (range 22-168 months). Type of p

The anatomy and reconstruction of the medial patellofemoral ligament.

INTRODUCTION: Reconstruction of the MPFL for recurrent and acute lateral dislocation of the patella is becoming more accepted. This study aimed to further define the medial patellofemoral ligament (MPFL) anatomy and to describe the most suitable sites for graft attachment when reconstructing the MPFL. MATERIALS AND METHODS: Anatomical dissection was performed on 25 embalmed knee specimens. Four separate specimens were used to assess graft fixation points. Sites were tested for isometricity from 0 to 120 degrees of knee flexion. Length patterns were considered isometric if there was less than
