A multi-centre study on epidemiology and clinical characteristics of human metapneumovirus infection in Sri Lanka from 2021 to 2024.

Introduction. Human metapneumovirus (hMPV), first identified in 2001, is one of the major respiratory pathogens causing acute respiratory tract infections (ARTIs). In Sri Lanka, data on epidemiology and clinical characteristics of hMPV infections are limited. In this study, we aimed to investigate the epidemiology and clinical characteristics of hMPV infection in adults and children with ARTIs in different locations in Sri Lanka from January 2021 to December 2023. Methods. A total of 1,582 respiratory samples from patients with ARTIs were enrolled from four tertiary care hospitals. Nasopharyng

Comparative Effectiveness of Epidural Analgesia and Intravenous Lidocaine for Postoperative Pain in Major Abdominal Surgery: A Systematic Review and Meta-Analysis.

Background: Pain management is an integral part of recovery after major abdominal surgeries. Traditionally, epidural analgesia is used for postoperative pain management in major abdominal surgeries. However, intravenous lidocaine has recently been proven to be a good alternative. However, there is very limited evidence comparing their efficacy in major abdominal surgery. The aim of this review is to compare the effectiveness of epidural analgesia with intravenous lidocaine in reducing pain and opioid consumption following major abdominal surgery. Methods: We searched PubMed and Cochrane Librar

Proton pump inhibition to prevent delayed chemotherapy-induced nausea and vomiting in patients receiving adjuvant or neoadjuvant breast cancer chemotherapy: a phase II, randomised double-blind crossover trial (PantoCIN).

PURPOSE: Delayed chemotherapy-induced nausea and vomiting (CINV) is a distressing effect of chemotherapy for early breast cancer (EBC), with rates exceeding 50%, despite multi-agent prophylaxis. We hypothesised that chemotherapy-induced alterations to the gastric environment may result in delayed CINV, and that pantoprazole, a proton pump inhibitor, may be effective prophylaxis by reducing stomach acid. METHODS: Using a randomised crossover design, we compared pantoprazole 40 mg once daily with placebo for 5 days at the start of each chemotherapy cycle, in addition to standard antiemetic prop

Proximal femoral bionic nail (PFBN) offers comparable functional and clinical outcomes to PFNA and intertan in the treatment of intertrochanteric fractures: a systematic review and meta-analysis.

BACKGROUND: Intertrochanteric fractures represent nearly 45% of hip fractures and are rising due to global aging. Surgical fixation, especially intramedullary nailing with PFNA or InterTAN, is the preferred treatment. However, complications like implant failure persist. The proximal femoral bionic nail (PFBN), designed to mimic natural femoral anatomy, has emerged as a promising alternative. This systematic review and meta-analysis aimed to compare PFBN with PFNA or InterTAN. METHODS: This study followed PRISMA guidelines and was registered on PROSPERO. A comprehensive search of PubMed, Scopu

Applications used to minimize forgotten stents: a systematic review of the literature. Is it the dawn of a new era?

PURPOSE OF REVIEW: Ureteral stents are an integral part of daily urological clinical practice, but in case of ureteral stents inadvertently left in place or forgotten, they can lead to a range of complications. Managing retained DJ stents presents a complex challenge for urologists, involving various aspects such as surgery, legal implications, and financial factors both for the patient as for the health system. Ensuring proper follow-up of patients poses a significant challenge even in nowadays in everyday clinical urological practice as it involves efficient urologist-patient communication.

Emergency Department Risk Factors for Post-Concussion Syndrome After Mild Traumatic Brain Injury: A Systematic Review.

Approximately 16% of patients with mild traumatic brain injury (mTBI) develop a post-concussion syndrome (PCS) with persistent physical, neurological, and behavioral complaints. PCS has a great impact on a patient's quality of life, often decreases the ability to return to work, and henceforth has a great economic impact. Recent studies suggest that early treatment can greatly improve prognosis and prevent long-term effects in these patients. However, early recognition of patients at high risk of PCS remains difficult. The objective of this systematic review is to assess risk factors associate
