COVID-19 AND CHILDREN: COMPLICATIONS AND LATE OUTCOMES.

Since December 2019, Covid-19 has become a challenge for doctors around the world, including pediatricians. In most infected children, the disease manifests itself in a mild or is char- acterized by a subclinical course. At the same time, in some cases, a severe clinical picture of the so-called late Covid disease may develop, in the form of a multisystem syndrome and other complications. In 2020-2021 at the Academic Pediatric Clinic named after G. Zhvania of Tbilisi State Medical University, we observed 60 children with post-Covid complications and late Covid syn- drome. More than half (32 ch

Intestinal Lymphangiectasia: Insights on Management and Literature Review.

BACKGROUND Intestinal lymphangiectasia (IL) is a rare disease characterized by a dilatation of the intestinal lymphatics and loss of lymph fluid into the gastrointestinal tract leading to hypoproteinemia, edema, lymphocytopenia, hypogammaglobinemia, and immunological abnormalities. Iron, calcium, and other serum components (e.g., lipids, fat soluble vitamins) may also be depleted. A literature search revealed more than 200 reported cases of IL. Herein, we report our observations of a patient diagnosed with IL; we also present our conclusion for our review of the published literature. CASE REPO

Tumour grade on core biopsy and evidence of axillary involvement on ultrasound predicts response in elderly co-morbid patients treated with primary hormone therapy for oestrogen receptor positive breast carcinoma.

INTRODUCTION: Treatment of women with oestrogen-receptor positive breast cancer who are high risk for general anaesthetic remains controversial. Current guidance is based on studies pre-dating aromatase inhibitors (AIs) which may have also included hormone-receptor negative patients. Such studies have demonstrated improved disease-free survival and local disease control following surgery when compared with primary hormone therapy (PHT) alone. However uncertainty persists regarding benefit of surgery over optimal hormone treatment in patients with significant co-morbidity. METHOD: Retrospectiv

A huge lymphovenous malformation in the retroperitoneum.

Vascular malformations are localized or diffuse errors of embryonic development resulting in collections of abnormal vessels with normal endothelium. Many authors have pointed out the confusing terminology of vascular anomalies that has led to improper diagnosis and therapy. This still remains confused in clinical practice, especially in cases occurring at rare regions or presenting atypical radiologic findings. We present a huge retroperitoneal lymphovenous malformation, which exhibited unusual radiologic findings and was difficult to confirm the diagnosis that occurred in a 12-year-old male.

Cerebrospinal fluid mechanics across CNS barriers: from production, circulation, and clearance to mechanomedicine.

BACKGROUND: Cerebrospinal fluid (CSF) is often framed as a transport medium, yet its motion and pressure dynamics impose continuous mechanical loading on central nervous system (CNS) barrier and interface systems. These cues span scales-from cilia-scale near-wall shear to craniospinal compliance-driven pulsatility-and may shape barrier phenotypes, immune programs, and clearance efficiency. In simple terms, this review asks how abnormal CSF motion is converted into barrier dysfunction and disease-relevant outcomes. MAIN BODY: We synthesize evidence that CSF mechanics is spatially heterogeneous

A gaint mesenteric cystic lymphangioma of small intestinal in an adult: A case report and literature review.

RATIONALE: Lymphangioma (LA) is a benign tumor that predominantly occurs in children, and is characterized by abnormal proliferation of lymphatic vessels. It is commonly present in areas such as the head, neck, and axilla. Mesenteric cystic lymphangioma (MCL) is relatively rare in adults. We reported a case of an elderly patient with a massive cystic LA of the mesentery that exerted pressure on surrounding tissues, resulting in both local and systemic symptoms. PATIENT CONCERNS: The patient is a 66-year-old male who presented to our hospital with a history of recurrent abdominal pain of unkno

Sjögren's syndome and extragonadal sex steroid formation: a clue to a better disease control?

Sjögren's syndrome (SS) is an autoimmune disease characterized by lymphoplasmacytoid focal adenitis leading to mucosal dryness, with 9:1 female dominance and peak incidence at menopause. Due to autoimmune adenitis it can be speculated that the normal epithelial cell renewal has failed, possibly as a result of local intracrine failure to process dehydroepiandrosterone (DHEA) to dihydrotestosterone (DHT). Local intracrine/-cellular DHT deficiency seems to predispose to SS if estrogens are low, in menopausal women and in men. This intracrine failure could be the initial noxious stimulus, factor X

Increased detection of clinically significant prostate cancer by additional sampling from the anterior lateral horns of the peripheral zone in combination with the standard sextant biopsy.

BACKGROUND: The objective of the present study was to investigate whether obtaining an increased number of biopsy cores by sampling additional areas, along with the standard sextant biopsy, results in a higher rate of detection of potentially insignificant prostate cancer. METHODS: We included 130 patients who underwent radical retropubic prostatectomy at our institution between January 1999 and June 2003 after being diagnosed as having prostate cancer based on systematic prostate biopsies that included the areas examined by standard sextant biopsies and the bilateral anterior lateral horns (

Childhood abdominal cystic lymphangioma.

BACKGROUND: Abdominal lymphangioma is a rare benign congenital malformation of the mesenteric and/or retroperitoneal lymphatics. Clinical presentation is variable and may be misleading; therefore, complex imaging studies are necessary in the evaluation of this condition. US and CT have a major role in the correct preoperative diagnosis and provide important information regarding location, size, adjacent organ involvement, and expected complications. OBJECTIVE: To evaluate the clinical and imaging findings of seven children with proven abdominal cystic lymphangioma. MATERIALS AND METHODS: Cli

The histopathology of lepromatous leprosy in the nose.

On the basis of clinical, histological and bacteriological assessments, 31 patients in Central India were selected and classified as having active but early lepromatous leprosy and 4 patients as having early borderline leprosy. From the nose of each patient an average of 4 biopsies were taken from particular sites of the septum and turbinates either by punch biopsy or dissection with a scalpel. The nasal tissues from all the lepromatous patients contained many acid-fast bacilli; no bacilli or abnormalities were seen in nasal tissues from the borderline patients. The histopathology of these hig

Abnormal spindle-like microcephaly-associated protein promotes proliferation by regulating cell cycle in epithelial ovarian cancer.

BACKGROUND: Epithelial ovarian cancer (EOC) ranks first for female gynecological tumor-related deaths. Due to the limited efficacy of traditional chemotherapy strategies, potential therapeutic targets are urgently needed. Previous studies have reported a relationship between abnormal spindle-like microcephaly-associated protein (ASPM) and ovarian cancer based on immunohistochemistry (IHC) and bioinformatics analysis. However, the potential role of ASPM in the proliferation of ovarian cancer cells and its molecular mechanism remain to be elucidated. Therefore, we aimed to further investigate th

In normal rat, intraventricularly administered insulin-like growth factor-1 is rapidly cleared from CSF with limited distribution into brain.

BACKGROUND: Putatively active drugs are often intraventricularly administered to gain direct access to brain and circumvent the blood-brain barrier. A few studies on the normal central nervous system (CNS) have shown, however, that the distribution of materials after intraventricular injections is much more limited than presumed and their exit from cerebrospinal fluid (CSF) is more rapid than generally believed. In this study, we report the intracranial distribution and the clearance from CSF and adjacent CNS tissue of radiolabeled insulin-like growth factor-1 after injection into one lateral

[Role of X-ray CT and magnetic resonance imaging in the diagnosis of gynecological malignant tumor].

Diagnostic imaging is important in differentiating benign and malignant pelvic tumors and in staging malignant tumors. Many imaging techniques are now available. We describe computed tomographic (CT) and magnetic resonance imaging (MRI) features of gynecologic tumors. The following nine CT parameters were evaluated in 251 cases of cervical cancer (the incidence of each feature is given in parentheses): 1) enlargement of the cervix (58%), 2) low density area(s) (LDA) in the cervical region (28%), 3) presence of a necrotic cavity (11%), 4) pyometra (16%), 5) irregularity or indistinctness of the

Percutaneous drainage and ablation as first line therapy for macrocystic and microcystic orbital lymphatic malformations.

PURPOSE: To review the management of orbital lymphangiomas and to propose a new treatment for both macrocystic and microcystic lymphatic malformations of the orbit. METHODS: A retrospective case series of all patients from the authors' practice from 2001 to the present who met the histopathologic and/or diagnostic imaging criteria for orbital lymphatic malformation was reviewed. Lymphatic malformation was diagnosed if there was a multilobulated pattern on CT or a cystic internal structure on ultrasonography. In patients that were treated, macrocysts (>1 cm) were treated with dual-drug chemoab
