Malignant transformation of monostotic fibrous dysplasia in the mandible.

Malignant transformation of fibrous dysplasia (FD), which was first introduced by Coley and Steward is very rare. We present a case of malignant transformation of monostotic mandibular FD after 2 surgical excisions, 20 and 15 years ago, respectively. The treatment choice of FD is usually conservative. With the advanced surgical techniques, radical excisions followed by immediate reconstruction have been reported. However, the decision criteria are still controversial and we might think that radical treatment is a good method for recurrent lesions based on the recent literatures. The case discu

Multiple complications due to osteoradionecrosis in a patient with thromboangiitis obliterans.

A case is presented of a patient with thromboangiitis obliterans (TAO) who developed severe necrosis of the intraoral soft tissues and maxillary and mandibular bone after radiotherapy for a cT2N0M0 squamous cell carcinoma of the soft palate. Multiple surgical procedures including partial resection of the mandible and maxilla and reconstruction of intraoral and extraoral defects with a pectoralis major myocutaneous flap and anterolateral thigh flap were performed with partial success. Although a causal relationship between TAO and the described complications cannot be verified, close monitoring

Fibromatosis of the mandible in a child.

Fibromatosis represents a group of fibrous tumors showing clinic and biologic features between benign fibrous lesions and fibrosarcoma. These locally aggressive tumors have high recurrence rates (20% to 70%). A four-year-old boy presented with mandibular fibromatosis occupying the mandible completely and extending to the submandibular gland and soft tissues. Complete hemimandibulectomy and submandibular gland excision were performed followed by reconstruction with a curved Kirschner wire. No signs of recurrence was observed during a follow-up period of 18 months. In addition, no limitations in

Development of a novel method for estimating the missing mandible using three-dimensional homologous models.

BACKGROUND AND AIM: Skeletal remains are sometimes discovered with the mandible missing from the skull due to external factors such as environmental conditions or scavenger activity. The absence of the mandible reduces the accuracy of forensic identification methods, including facial reconstruction. Although we previously established a two-dimensional method for estimating mandibular measurements (e.g., bicondylar breadth) commonly used in forensic practice, no method has yet been developed for estimating the entire three-dimensional mandibular shape. Therefore, the aim of this study was to es

Bone Mass, Microarchitecture, and Morphometric Insights on a Right Unilateral Bifid Mandibular Condyle: A Micro-CT Analysis Report and Literature Review.

Background/Objectives: The bifid mandibular condyle (BMC) is a rare anatomical variation characterized by a division of the mandibular condyle into two distinct heads. Although frequently identified through radiographic studies or in dry skulls, its etiology remains unclear, and few studies have examined its internal bone structure. This study aimed to perform a detailed morphologic and microarchitectural analysis of a right unilateral bifid mandibular condyle using micro-CT and to contrast the findings with the relevant morphological and clinical literature. Case Presentation: A human mandibl

Navigating Coding Challenges in Craniofacial Surgery: A National Survey Analysis on CPT Variability.

ObjectiveAccurate current PROCEDURAL TERMINOLOGY (CPT) coding is essential to healthcare costs and fair reimbursement. In craniofacial (CF) surgery, specific codes for procedures like mandibular distraction and cranial remodeling are lacking, leading to unclear standards that may undervalue complex techniques. This study hypothesizes variability in billing exists and aims to characterize these trends.Design & SettingA 21-question survey was distributed via email.ParticipantsMembers of the American Society of Maxillofacial Surgeons and American Society of Craniofacial Surgeons.Main Outcomes Mea

Review of the Literature on the Current State of Periosteum-Mediated Craniofacial Bone Regeneration.

STUDY DESIGN: This is an article review on the current state of periosteum-mediated bone regeneration (PMBR). It is a known mandibular reconstruction option in children, and though poorly understood and unpredictable, the concerns of developmental changes to donor and recipient tissues shared by other treatment options are nonexistent. The definitive role of periosteum during bone regeneration remains largely unknown. OBJECTIVE: The objective is to review the literature on the clinical and molecular mechanism evidence of this event. METHODS: Our search methodology was modeled after the PRISM…

Mapping the expression of beam hardening artefacts produced by metal posts positioned in different regions of the dental arch.

OBJECTIVES: To objectively assess the expression and direction of artefacts in the vicinity of metal posts positioned in different mandibular regions. MATERIALS AND METHODS: A human mandible had two sockets prepared-anterior and posterior regions-to accommodate a single-rooted tooth. Two CBCT units, Picasso Trio and OP300, were used. CBCT images of the tooth without metal post, and with silver-palladium, nickel-chromium, or cobalt-chromium posts (experimental groups) were individually acquired. Then, 8 lines of interest (LOIs) were determined around the root canal, in an axial reconstruction:

Microsurgical Reconstruction of Extensive Lower Lip Defects: An Algorithm for Free Flap Selection and Functional Outcomes.

BACKGROUND: Large defects of the lower lip pose a difficult challenge for any reconstructive surgeon. When there is limited local tissue available to resurface the defects, free flaps are the preferred option. AIM AND OBJECTIVES: We reported our experience in the reconstruction of extensive lower lip defects. The authors propose a new algorithm for microsurgical technique selection and assessment of the functional outcomes obtained. MATERIALS AND METHODS: A retrospective review of all microsurgical reconstructions of extensive lower lip defects by the senior author for 10 years was performed

Custom made onlay implants in peek in maxillofacial surgery: a volumetric study.

PURPOSE: Facially malformed patients often present a variable degree of facial imbalance even after basal bone correction, and resolution of the residual hard- and soft-tissue deficiencies and asymmetries of the face are of great importance for achieving a satisfactory post-implant procedure result. The use of polyether ether ketone (PEEK) implants for maxillofacial reconstruction has been documented in the literature, but the number of patients who have received them is limited. The aim of this study was to evaluate the overall volumetric results in patients with facial imbalance after onlay

Lip reconstruction using a functioning serratus anterior free flap: preliminary study.

Reconstructive surgery to the lips requires the replacement of defective tissues with similarly functioning tissues. While non-dynamic free tissue transfers provide adequate lower lip reconstruction, improved benefits may be obtained with innervated free muscle flaps. This study reports the use of innervated serratus anterior muscle free flaps for lip reconstruction in five patients. All patients had squamous cell carcinoma of either the upper or lower lip. After resection of the tumours, the resultant defects comprised two-thirds of the lips. The innervated serratus anterior muscle free flap

[Microsurgical procedures in reconstructive otorhinolaryngologic surgery].

For reconstruction of bony and soft tissue defects numerous free flaps are available to the head and neck surgeon. Different tissues, as from skin, mucosa, fat, muscle or bone can be transferred by microvascular transplantation. Microvascular tissue transfer has completely replaced tissue transfer from distant donor sites and in daily routine now competes with pedicled tissue transfer. Most of the reconstructive problems can be solved by using a limited number of reliable and versatile flaps, e.g. forearm, scapular, latissimus dorsi and jejunal flap. The scapular flap offers advantages in reco

Primary versus secondary microvascular bony jaw reconstruction in patients with oral squamous cell carcinoma and adjuvant therapy - a retrospective cohort study.

BACKGROUND: There are two established timepoints for microvascular bone reconstruction of the jaw in patients with oral squamous cell carcinoma (OSCC): 1. Primary reconstruction together with tumour resection and 2. Secondary reconstruction after successful adjuvant therapy. However, the decision between primary and secondary reconstruction remains controversial and is often made on a case-by-case basis. OBJECTIVE: This study aimed to compare clinical outcomes of primary versus secondary osseous jaw reconstruction in patients with OSCC. The aim of this study was to retrospectively describe an

Immediate Free Fibula Flap After Maxillectomy With Simultaneous Full-arch Dental Implant-Supported Prosthetic Rehabilitation of the Midface and Mandible: The "Super Reconstruction Concept".

Immediate reconstruction and dental rehabilitation for ablative defects of the head and neck in one surgery is now a commonly used technique among reconstructive surgeons. Often patients are edentulous or partially edentulous in the jaw opposing that with pathology. In this case, only rehabilitating the dental arch affected by pathology would not render the patient with a functional occlusion. We introduce the "super reconstruction" concept which includes not only rehabilitating the dental arch affected by pathology but also the opposing arch, giving the patient a fully functional occlusion at

Early Mandibular Distraction in Craniofacial Microsomia and Need for Orthognathic Correction at Skeletal Maturity: A Comparative Long-Term Follow-Up Study.

BACKGROUND: Controversy exists regarding the treatment of mandibular hypoplasia in craniofacial microsomia patients, notably the role of mandibular distraction osteogenesis. The authors compared the need for orthognathic surgery in skeletally mature craniofacial microsomia subjects who either did (study group) or did not (control group) undergo early mandibular distraction osteogenesis. METHODS: A retrospective review was conducted of all craniofacial microsomia patients evaluated between January of 1993 and March of 2017. This study included patients with a Kaban-Pruzansky grade I to III man

A Modified Approach to Extensive Oromandibular Reconstruction Using Free Fibula Flaps.

In select patients with advanced disease resulting in large composite tissue defects, consideration is often given to multiple flap reconstruction. The authors propose an alternative option. Using virtual surgical planning the authors demonstrate how modest sacrifice in projection translates into a substantial decrease in the volume and surface area of soft tissue needed, in turn maximizing soft tissue coverage with a single fibula free flap. The authors used 3-dimensional virtual surgery to simulate angle-to-angle reconstructions using free fibula flaps. The reference 3-segment reconstruction
