Traumatic Optic Neuropathy-Craniofacial Fracture Patterns and Management.

PURPOSE: Up to half of patients with traumatic optic neuropathy (TON) develop permanent vision loss. The diagnosis and management of TON are challenging. Here, we aim to identify craniofacial fractures associated with TON and compare visual outcomes in TON patients under observation versus steroid-based management. METHODS: We reviewed CT scans of patients after acute facial trauma at the R Adams Cowley Shock Trauma Center from 2018 to 2022 to determine craniofacial fractures associated with TON. TON patient outcomes between 2013 and 2022 were analyzed to compare steroid treatment versus obse

Outcomes After Open Reduction With Internal Fixation of Mandible Fractures.

INTRODUCTION/HYPOTHESIS: Mandible fractures contribute substantially to morbidity after blunt trauma. Controversy exists surrounding the appropriate timing of surgical intervention and benefit from routine postreduction imaging. METHODS: The authors retrospectively reviewed 146 patients who sustained traumatic mandible fractures at a level 2 trauma center over a 5-year period, between January 2012 and December 2016. The authors excluded all patients who did not undergo surgery, underwent operative closed reduction only, sustained other significant maxillofacial injuries, penetrating mechanism…

Surgical management of orbital wall fractures in juveniles: a European multicenter study.

INTRODUCTION: Isolated orbital floor fractures in juveniles are a challenge, as children and adolescents may experience diplopia and limited ocular motility, even with little to no periorbital edema or visible signs of injury to the eye. The aim of the present study is to assess the demographic and clinical variables, and the outcomes of pediatric patients affected by isolated orbital wall fractures at several European oral and maxillofacial surgery centers. METHODS: Pediatric patients (less than 18 years) with hospitalized and surgically treated for unilateral isolated orbital blow out fract

Current therapy: complications associated with rigid internal fixation of facial fractures.

Rigid internal fixation (RIF) is commonly used for the anatomical reduction and fixation of facial fractures. This technique has many advantages over more traditional methods, such as wire osteosynthesis and maxillomandibular fixation. Properly placed RIF ensures a stable anatomical reduction and allows for immediate or early restoration of function. RIF has a number of complications associated with its use in facial trauma surgery including metal sensitivity, infection, neurologic injury, dental trauma, stress shielding, and malocclusion. Complication rates appear to be inversely proportional

Use of Vascularized Myo-Osseous Fibula Free Flap to Reconstruct a Hemimandibular Defect With a Concomitant Skull Defect Arising From Stock Condylar Prosthesis Displacement Into the Middle Cranial Fossa.

Functional reconstruction of the temporomandibular joint (TMJ) is a controversial topic among oral and maxillofacial surgeons; this controversy becomes more complicated when one dives into the dilemma of the ideal reconstructive modality. TMJ defects might result from various etiologies, such as blunt or penetrating traumatic injuries, advanced degenerative joint disease, or various pathologic conditions, including benign and malignant conditions, that might arise from the TMJ or adjacent tissues. Reconstruction of the TMJ is vital because of its essential function in mastication, articulation

Cranioplasty complications following wartime decompressive craniectomy.

OBJECT: In support of Operation Iraqi Freedom (OIF) and Operation Enduring Freedom-Afghanistan (OEF-A), military neurosurgeons in the combat theater are faced with the daunting task of stabilizing patients in such a way as to prevent irreversible neurological injury from cerebral edema while simultaneously allowing for prolonged transport stateside (5000-7000 miles). It is in this setting that decompressive craniectomy has become a mainstay of far-forward neurosurgical management of traumatic brain injury (TBI). As such, institutional experience with cranioplasty at the Walter Reed Army Medica

Retrospective review of 78 rehabilitated head and neck postoncological patients: a new classification method.

BACKGROUND: Numerous studies have been published about the prosthetic rehabilitation of the postoncological maxillo-facial patient, but the guidelines that emerge lack a correlation between the anatomical classification of the treated site, which generally is preparatory upon surgery, and the type of prosthetic rehabilitation appropriate to the new anatomical and functional condition. With this correlation, it would be possible to obtain a multidisciplinary and predictable therapeutic process, able to identify from the beginning the best type of prosthetic rehabilitation. METHODS: The authors

Applications of coronoid process as a bone graft in maxillofacial surgery.

The coronoid process can be easily harvested as a donor bone by an intraoral approach during many maxillofacial surgery procedures. The purpose of this study was to evaluate the utility of autogenous coronoid process bone grafts for maxillofacial reconstructive surgery. Twelve patients, who underwent coronoid process grafts for reconstruction of maxillofacial deformities due to trauma, alveolar atrophy, or temporomandibular joint ankylosis, were included in the study. There were 3 orbital defects after extended maxillectomy, 1 blowout fracture of the orbit, 2 cases of reconstruction after temp

Spontaneous Repositioning of a Traumatic Intraorbital Encephalocele.

Fractures of the orbital roof are relatively rare facial fractures that are often caused by high-energy trauma and are frequently seen in combination with intracranial injury and other facial fractures. Surgical reconstruction should be considered in cases with threatened visual function, significant displacement of fracture fragments, dural laceration, or additional facial fractures requiring repositioning. A rare complication of an orbital roof fracture, described in only a few case reports, is a traumatic intraorbital encephalocele (TIOE). This occurs when intracranial swelling disrupts the

[Cooperation between the plastic surgery and the head & neck surgery departments in the treatment of unusual cases, Veszprém, Hungary, 2010-2015].

INTRODUCTION: Handling problems in the head and neck region often requires a close cooperation between allied professions, just as in the challenging cases presented by us. Cases and methodology: With the first patient, we performed a radical surgery on the left side of the face due to recidivious basal cell carcinoma, followed by reconstruction using a Type I chimeric anterolateral thigh flap (ALT), while in the case of the second patient, we carried out a radical surgery on the right side of the face due to epithelial carcinoma and reconstruction with a Type I chimeric thoracodorsal flap. Diagnosis of jaw and dentoalveolar fractures in a traumatized patient with cone beam computed tomography.

The purpose of this case report is to discuss and illustrate the clinical usage of Cone Beam Computed Tomography (CBCT) for the diagnosis of maxillofacial fractures in a traumatized patient. In this presentation, a 30-year-old male patient who was referred to Oral Diagnosis and Radiology Department with a limitation of mouth opening was reported. The history of the patient revealed a traumatic injury on his face because of a fall. The patient was initially examined by a medical practitioner in the emergency department of a public hospital. According to 2D cephalometric analysis, no fracture ex

Sensory disturbances associated with implant surgery.

PURPOSE: This paper presents a critical review of the literature on neurosensory disturbances associated with implant surgery. MATERIALS AND METHODS: The reviewed literature includes mainly retrospective implant studies and patients' surveys, as well as a few prospective psychophysical studies on sensory disorders following maxillofacial surgeries. RESULTS: The available data suggest that injury to the peripheral branches of the trigeminal nerve and subsequent sensory disturbances are potential complications following implant surgery. Cross-sectional studies suggest that gross tactile sensat

Different effects of abnormal mechanical stress on temporomandibular joint cartilage, subchondral bone, and discs.

BACKGROUND: Temporomandibular joint disorder (TMJD) is a group of diseases occurring in the temporomandibular joint (TMJ) with clinical manifestations of pain in the joint area, mastication disorders, and restriction of mouth opening, which is one of the most common diseases of the oral and maxillofacial region, and its specific etiology has not yet been fully elucidated. As a biomechanical orchestrator, the TMJ mediates dynamic transduction of masticatory forces during the functional loading cycle. Notably, as a secondary cartilage type, the condylar cartilage exhibits postnatal remodeling th

The "low preauricular" transmasseteric anteroparotid (TMAP) technique as a standard way to treat extracapsular condylar fractures.

Condylar fracture treatment is a debated topic among maxillofacial surgeons. Various surgical techniques are used today, each one with advantages and disadvantages. The aim of this study is to present and evaluate our technique adopted for treatment of any type of extracapsular condylar fractures. Between 2020 and 2022, 16 condylar fractures were treated. In two patients with bilateral condylar fractures, the present technique was compared to the mini-retromandibular approach. All the patients were checked for clinical and radiological outcomes, facial nerve injury, scar visibility and presenc
